Orthostatic hypotension, dizziness, neurology outcomes, and death in older adults, supplement methods & tables, STROND checklist
<p>Objective To test the hypothesis that orthostatic hypotension (OH) might cause cerebral hypoperfusion and injury, we examined the longitudinal relationship between orthostatic hypotension (OH) or orthostatic symptoms and incident neurologic outcomes in a community population of older adults. Methods Cardiovascular Health Study (CHS) participants (≥65yrs) without dementia or stroke had blood pressure (BP) measured after lying 20-minutes and after standing 3-minutes. Participants reported dizziness immediately upon standing and any dizziness in the past 2wks. OH was defined as a drop in standing systolic/diastolic BP ≥20/≥10mmHg. We determined the association between OH or dizziness with (1) MRI brain findings (ventricular size, white matter hyperintensities, brain infarcts) using linear or logistic regression, (2) cognitive function (baseline and over time) using generalized estimating equations, and (3) prospective adjudicated events (dementia, stroke, death) using Cox models. Models were adjusted for demographic characteristics and OH risk factors. We used multiple imputation to account for missing OH or dizziness (N=534). Results Prior to imputation, there were 5,007 participants (mean age 72.7±5.5yrs, 57.6% women, 10.9% black, 16% with OH). OH was modestly associated with death (HR=1.11; 95%CI:1.02,1.20), but not MRI findings, cognition, dementia, or stroke. In contrast, dizziness upon standing was associated with lower baseline cognition (β=-1.20;-1.94,-0.47), incident dementia (HR=1.32;1.04,1.62), incident stroke (HR=1.22;1.06,1.41), and death (HR=1.13; 1.06,1.21). Similarly, dizziness over the past two weeks was associated with higher white matter grade (β=0.16;0.03,0.30), brain infarcts (OR=1.31;1.06,1.63), lower baseline cognition (β=-1.18;-2.01,-0.34), and death (HR=1.13;1.04,1.22). Conclusions Dizziness was more consistently associated with neurologic outcomes than OH 3-minutes after standing. Delayed OH assessments may miss pathologic information related to cerebral injury.</p>
Reception of a physical activity intervention program in German older adults. Data from a feedback analysis of PROMOTE II.
<p>As part of the<strong> AEQUIPA </strong>prevention research network, the aim of the <strong>PROMOTE II</strong> study was to gain insights into primary prevention for healthy aging. One focus was on the development and dissemination of ready-to-use tools and work aids to promote physical activity.</p><p>As part of a nine-month randomized trial with a cross-over design among adults aged 60 and above, eligible participants from 14 districts in Bremen in proximity to the two study centers (one in the North and one in the East of the city of Bremen, Germany) were randomly assigned to either a) a print-based intervention (PRINT) or b) a web-based intervention (WEB). Randomly selected thirty percent of the web-based intervention group received an activity tracker in addition (WEB+).</p><p>Usage of program components, attendance of group sessions and acceptance of the intervention were assessed via self-administered paper-based questionnaires three and nine months after baseline as well as feedback interviews and class recordings. Participant feedback was analyzed using quantitative methods on a descriptive basis, as well as using qualitative content analysis of open-ended questions, group-interviews and protocols based on the socio-ecological model as coding framework.</p><p>This publication provides the coding matrix, including the participant feedback data.</p>
Monitoring mobility in older adults using a global positioning system (GPS) smartwatch and accelerometer: A validation study
<p><strong>Background</strong></p> <p>There is interest in identifying the most reliable method for detecting early mobility limitations. Accelerometry and Global Positioning System (GPS) could provide insight into declines in mobility, but few studies have used this multi-sensor approach to monitor mobility in older adults. </p> <p><strong>Methods</strong></p> <p>Thirty-two volunteers (66.2±6.3 years) agreed to participate in our validation study. We conducted two experiments to determine the validity of the TicWatch S2 and Pro 3 Ultra GPS models against the Qstarz receiver in measuring life-space mobility, trip frequency, duration, and mode. We also assessed the accuracy of the TicWatch in measuring step count and agreement with the ActiGraph wGT3X-BT for activity counts and sedentary behavior. Participants wore devices simultaneously for three consecutive days and recorded activity and trip information.</p> <p><strong><span>Results</span></strong></p> <p>The TicWatch Pro 3 Ultra GPS performed better than the S2 model and was similar to the Qstarz in all tested trip-related measures, and it was able to estimate both passive and active trip modes. Both models showed similar results to the Qstarz in life-space-related measures. The TicWatch S2 demonstrated good to excellent overall agreement with the ActiGraph algorithms for the time spent in sedentary and non-sedentary activities, with 84% and 87% agreement rates, respectively. Under supervised conditions, the TicWatch Pro 3 Ultra GPS measured step count consistently with the gold standard observer, with a bias of 0.4 steps. The thigh-worn ActiGraph algorithm accurately classified sitting and lying postures (97%) and standing postures (90%).</p> <p><strong>Conclusion</strong></p> <p>Our multi-sensor approach to monitoring mobility has the potential to capture both accelerometer-derived movement data and trip/life-space data only available through GPS. In this study, we found that the TicWatch models are valid devices for capturing GPS and raw accelerometer data, making them useful tools for assessing real-world mobility in older adults and advancing our knowledge of early mobility decline.</p>
Young and older adult vowel categorization responses
<p>Age-related changes in auditory processing may reduce physiological coding of acoustic cues, contributing to older adults' difficulty perceiving speech in background noise. This study investigated whether older adults differed from young adults in patterns of acoustic cue weighting for categorizing vowels in quiet and in noise. All participants relied primarily on spectral quality to categorize /Ꜫ/ and /æ/ sounds in both listening conditions. However, relative to young adults, older adults exhibited greater reliance on duration and less reliance on spectral quality. These results suggest that aging alters patterns of perceptual cue weights that may influence speech recognition abilities.</p>

Dementia knowledge and misconceptions: a cross-sectional study in Swiss and Italian young adults, adults, and older adults
<p>The dataset was generated during a cross sectional survey conducted in September-December 2019 aimed at quantifying dementia knowledge in the general population. Data come from a sample of 1500 young adults, adults, and older adults living in Northern Italy and Southern Switzerland. We registered socio-demographic characteristics including whether the participant had contact with a person living with dementia, and measured dementia knowledge with the Dementia Knowledge Assessment Survey (DKAS). </p>
Effect Of Pulsed Electromagnetic Field And Microwave Therapy On Pain And Physical Function In Older Adults With Knee Osteoarthritis: A Randomized Clinical Trial.
<p><strong><span>Background and purpose:</span></strong><span> </span><span>Pulsed electromagnetic field (PEMF) therapy and microwaves (MW) are two electrotherapy modalities that have shown to improve pain and function in patients with knee osteoarthritis (KOA). Nevertheless, the effectiveness of these therapies is controversial due to diversity in the application parameters and treatment protocols in these patients. The objective is to compare the effectiveness of active PEMF versus MW, as well as sham PEMF, in addressing pain and improving functionality for treating KOA.</span></p> <p><strong><span>Methods:</span></strong><span> </span><span>Double-blind, placebo-controlled, randomized clinical trial. Participants diagnosed with KOA were assigned to an intervention combining an exercise program with active PEMF, MW, or sham PEMF, delivered in three weekly sessions over four weeks. The main outcomes were pain, reported on a 0–10 cm visual analogue scale (VAS), and functionality, as evaluated with the Western Ontario and McMaster Universities Arthritis (WOMAC) questionnaire, and the Timed Up and Go test. The outcomes were measured at pre-intervention, immediately post-intervention, and one and four months after the intervention.</span></p>

Raw Data for the article: Mortality after transjugular intrahepatic portosystemic shunt in older adult patients with cirrhosis: A validated prediction model
<p><strong>Background and aims: </strong>Implantation of a transjugular intrahepatic portosystemic shunt (TIPS) improves survival in patients with cirrhosis with refractory ascites and portal hypertensive bleeding. However, the indication for TIPS in older adult patients (greater than or equal to 70 years) is debated, and a specific prediction model developed in this particular setting is lacking. The aim of this study was to develop and validate a multivariable model for an accurate prediction of mortality in older adults.</p> <p><strong>Approach and results: </strong>We prospectively enrolled 411 consecutive patients observed at four referral centers with de novo TIPS implantation for refractory ascites or secondary prophylaxis of variceal bleeding (derivation cohort) and an external cohort of 415 patients with similar indications for TIPS (validation cohort). Older adult patients in the two cohorts were 99 and 76, respectively. A cause-specific Cox competing risks model was used to predict liver-related mortality, with orthotopic liver transplant and death for extrahepatic causes as competing events. Age, alcoholic etiology, creatinine levels, and international normalized ratio in the overall cohort, and creatinine and sodium levels in older adults were independent risk factors for liver-related death by multivariable analysis.</p> <p><strong>Conclusions: </strong>After TIPS implantation, mortality is increased by aging, but TIPS placement should not be precluded in patients older than 70 years. In older adults, creatinine and sodium levels are useful predictors for decision making. Further efforts to update the prediction model with larger sample size are warranted.</p>

<p>Data, supplementary material and scripts for the paper "Structure and drivers of social networks and their links with health in older adults"</p> <p>Social network is an important factor in promoting healthy aging. However, the mechanisms linking social capital to health are complex. Moreover, most of the social network analysis studies on older adults consider only participants’ relationships and not how these relationships are themselves connected. In this study, we went further than current ego-centered network studies by determining global social network metrics and the structure of relationships among older adult participants of the RECORD Cohort using the Veritas-Social questionnaire. The aim of this study is to identify key dimensions of social networks of older adults, and to evaluate how these dimensions relate to depressive symptoms, life satisfaction, and well-being. Using Principal Component Analyses (PCA), we identified four social network dimensions with psychological meanings. Dimension 1 (homophily) was positively linked with perceived accessibility to services in one’s residential neighborhood but negatively linked with the level of study. Dimension 2 (social integration) as Dimension 3 (social support) was only linked to the number of people living with ego. Dimension 4 was linked with perceived accessibility to local services. Finally, and rather surprisingly, we found that none of the four network dimensions, even the degree, was linked to the three health status metrics.</p>
