Data from: Randomized study of the impact of a therapeutic education program on patients suffering from chronic low-back pain who are treated with transcutaneous electrical nerve stimulation

Background: Transcutaneous electrical nerve stimulation (TENS) is often used for the treatment of low-back pain (LBP). However, its effectiveness is controversial. Objective: To determine the efficacy of TENS in the treatment LBP when associated to a therapeutic education program (TEP). Design: Open randomized monocentric study. Setting: University hospital between 2010 and 2014. Patients: A total of 97 patients suffering from LBP. Interventions: Routine care (TENS group) or routine care plus a therapeutic education program (TENS-TEP group) based on consultation support by a pain resource nurse Main outcome measures: EIFEL and Dallas Pain Questionnaire scores. Results: Twenty-two patients (44%) were still assessable at the end-of-study visit, whereas 33 (70%) were assessable at the same time point in the TENS-TEP group (P = 0.013). The EIFEL score and the Dallas score had a similar evolution over time between groups (p = 0.18 and p = 0.50 respectively). Similarly, there were no significant differences between the groups with respect to resting pain scores (p = 0.94 for back pain and p = 0.16 for leg pain) and movement pain scores (p = 0.52 for back pain and p = 0.56 for leg pain). At Month 6, there was no significant difference between the groups (p = 0.85) with regard to analgesics and social impact. Two patients presented a serious adverse event during the study (one in each group) but non-attributable to the treatment studied. Conclusion: This study does not support the use of TENS in the treatment of patients with chronic LBP even though patients benefited from a therapeutic education program by a pain resource nurse. However, the higher number of premature withdrawals in the TENS group may be due to early withdrawal of patients who did not experience improvement of their symptoms.

Effectiveness of community-based health education and home support program to reduce blood pressure among patients with uncontrolled hypertension in Nepal: A cluster-randomized trial

<p><b>Background: </b>Hypertension is a major global public health problem. Elevated blood pressure can cause cardiovascular and kidney diseases. We assessed the effectiveness of health education sessions and home support programs in reducing blood pressure among patients with uncontrolled hypertension in a suburban community of Nepal.</p> <p><b>Methods</b>: We conducted a community-based, open-level, parallel-group, cluster randomized controlled trial in Birendranagar municipality of Surkhet, Nepal. We randomly assigned four clusters (wards) into intervention and control arms. We provided four health education sessions, frequent home and usual care for intervention groups over six months. The participants of the control arm received only usual care from health facilities. The primary outcome of this study was the proportion of controlled systolic blood pressure (SBP). The analysis included all participants who completed follow-up at six months.</p> <p><b>Results</b>: 125 participants were assigned to either the intervention (n=63) or the control (n=62) group. Of them, 60 participants in each group completed six months follow-up. The proportion of controlled SBP was significantly higher among the intervention participants compared to the control (58.3% vs. 40%). Odds ratio of this was 2.1 with 95% CI: 1.01-4.35 (p=0.046) and that of controlled diastolic blood pressure (DBP) was 1.31 (0.63-2.72) (p=0.600). The mean change (follow-up minus baseline) in SBP was significantly higher in the intervention than in the usual care (-18.7 mmHg vs. -11.2 mmHg, p=0.041). Such mean change of DBP was also higher in the intervention (-10.95 mmHg vs. -5.53 mmHg, p=0.065). The knowledge score on hypertension improved by 2.38 (SD 2.4) in the intervention arm, which was significantly different from that of the control group, 0.13 (1.8) (p&lt;0.001).</p> <p><b>Conclusions</b>: Multiple health education sessions complemented by frequent household visits by health volunteers can effectively improve knowledge on hypertension and reduce blood pressure among uncontrolled hypertensive patients at the community level in Nepal. </p> <p><b>Keywords:</b> Hypertension; Blood pressure; Systolic Blood Pressure; Diastolic Blood Pressure; Cardiovascular disease; Health Education; Home Support</p> <p><b>Trial Registration: </b>ClinicalTrial.gov: <span>NCT02981251</span></p>

Data from: Graduate health professions education programs as they choose to represent themselves: A website review

<p>Introduction: In an age of increasingly face-to-face, blended, and online Health Professions Education, students have more selections of where they will receive a degree. For an applicant, oftentimes, the first step is to learn more about a program through its website. Websites allow programs to convey their unique voice and to share their mission and values with others, such as applicants, researchers, and academics. Additionally, as the number of Health Professions Education programs rapidly grows, websites can share the priorities of these programs. </p> <p>Methods: In this study, we conducted a website review of 158 Health Professions Education websites to explore their geographical distributions, missions, educational concentrations, and various programmatic components.</p> <p>Results: We compiled this information and synthesized pertinent aspects, such as program similarities and differences, or highlighted the omission of critical data.</p> <p>Conclusion: Given that websites are often the first point of contact for prospective applicants, curious collaborators, and potential faculty, the digital image of HPE programs matters. We believe our findings demonstrate opportunities for growth within institutions and assist the field in identifying the priorities of HPE programs. As programs begin to shape their websites with more intentionality, they can reflect their relative divergence/convergence compared to other programs as they see fit and, therefore, attract individuals to best match this identity. Periodic reviews of the breadth of programs, such as those undergone here, are necessary to capture diversifying goals, and serve to help advance the field of Health Professions Education as a whole.</p>
