Data from: Small volume plasma exchange for Guillain-Barré syndrome in resource-limited settings: a phase II safety and feasibility study
OBJECTIVE: To assess the safety and feasibility of small volume plasma exchange (SVPE) as an alternative to standard plasma exchange (PE) or intravenous immunoglobulin (IVIg) for Guillain-Barré syndrome (GBS) patients. DESIGN: Non-randomized, single arm, interventional trial. SETTING: National Institute of Neurosciences and Hospital, Dhaka, Bangladesh. PARTICIPANTS: Twenty adult (>18 years) patients with GBS presented within 2 weeks of onset of weakness who were unable to walk unaided for more than 10 meters. INTERVENTIONS: SVPE involves blood cell sedimentation in a blood bag and removal of supernatant plasma after blood cells are re-transfused. This procedure was repeated three to six times a day, for eight consecutive days. OUTCOME MEASURES: Serious adverse events (SAE) were defined as severe sepsis and deep venous thrombosis related to the central vein catheter (CVC) used during SVPE. SVPE was considered safe if less than 5/20 patients experienced a SAE, and feasible if 8 L plasma could be removed within 8 days in at least 15/20 patients. RESULTS Median patient age 33 years (IQR 23-46; range 18-55); 13 (65%) were male. Median MRC sum score was 20 (IQR 0-29; range 0-36); three (15%) patients required mechanical ventilation. One patient developed SAE (severe sepsis, possibly related to CVC). Minor adverse effects were transient hypotension in 10 (50%) patients; CVC-associated bleeding in 10 (50%); transfusion reaction to fresh frozen plasma in 4 (20%); and hypo-albuminemia, anaemia or electrolyte imbalance in 4 (20%). Removal of 8 L plasma was possible in 15 (75%) patients. GBS disability score improved by at least one grade in 14 (70%) patients four weeks after SVPE started. No patients died. CONCLUSION: SVPE seems a safe and feasible alternative treatment to standard PE or IVIg for GBS; further studies of clinical efficacy in low-resource developing countries are warranted. TRIAL REGISTRATION: Clinicaltrials.gov NCT02780570 on May 23, 2016. Strength and limitations of the study: 1. The strength of this study underlies the novel and simple technique of SVPE, which is much less expensive than conventional immunotherapies (plasma exchange and intravenous immunoglobulin). 2. SVPE is corroborated as safe and feasible for the first time in a prospective and standardized cohort of patients with Guillain-Barré syndrome (GBS). 3. The intrinsic limitations of this study are its non-randomized, single arm nature, which is conducted in a single center with a limited sample size of GBS patients. 4. Clinical efficacy of SVPE on patients with GBS was a secondary end-point assessment and therefore deserves a randomized controlled trial in future to assess the clinical efficacy of SVPE for the patients with GBS.
Data from: The scaling of parasite biomass with host biomass in lake ecosystems: are parasites limited by host resources?
The standing crop biomass of different populations or trophic levels reflects patterns of energy flow through an ecosystem. The contribution of parasites to total biomass is often considered negligible; recent evidence suggests otherwise, although it comes from a narrow range of natural systems. Quantifying how local parasite biomass, whether that of a single species or an assemblage of species sharing the same host, varies across localities with host population biomass, is critical to determine what constrains parasite populations. We use an extensive dataset on all free-living and parasitic metazoan species from multiple sites in New Zealand lakes to measure parasite biomass and test how it covaries with host biomass. In all lakes, trematodes had the highest combined biomass among parasite taxa, ranging from about 0.01 to 0.25 g m−2, surpassing the biomass of minor free-living taxa. Unlike findings from other studies, the life stage contributing the most to total trematode biomass was the metacercarial stage in the second intermediate host, and not sporocysts or rediae within snail first intermediate hosts, possibly due to low prevalence and small snail sizes. For populations of single parasite species, we found no relationship between host and parasite biomass for either juvenile or adult nematodes. In contrast, all life stages of trematodes had local biomasses that correlated positively with those of their hosts. For assemblages of parasite species sharing the same host, we found strong relationships between local host population biomass and the total biomass of parasites supported. In these host–parasite biomass relationships, the scaling factor (slope in log-log space) suggests that parasites may not be making full use of available host resources. Host populations appear capable of supporting a little more parasite biomass, and may be open to expansion of existing parasites or invasion by new ones.
Data from: Spending limited resources on de-extinction could lead to net biodiversity loss
There is contentious debate surrounding the merits of de-extinction as a biodiversity conservation tool. Here, we use extant analogues to predict conservation actions for potential de-extinction candidate species from New Zealand and the Australian state of New South Wales, and use a prioritization protocol to predict the impacts of reintroducing and maintaining populations of these species on conservation of extant threatened species. Even using the optimistic assumptions that resurrection of species is externally sponsored, and that actions for resurrected species can share costs with extant analogue species, public funding for conservation of resurrected species would lead to fewer extant species that could be conserved, suggesting net biodiversity loss. If full costs of establishment and maintenance for resurrected species populations were publicly funded, there could be substantial sacrifices in extant species conservation. If conservation of resurrected species populations could be fully externally sponsored, there could be benefits to extant threatened species. However, such benefits would be outweighed by opportunity costs, assuming such discretionary money could directly fund conservation of extant species. Potential sacrifices in conservation of extant species should be a crucial consideration in deciding whether to invest in de-extinction or focus our efforts on extant species.
Data from: Lactation and resource limitation affect stress responses, thyroid hormones, immune function and antioxidant capacity of sea otters (Enhydra lutris)
1. Lactation is the most energetically demanding stage of reproduction in female mammals. Increased energetic allocation toward current reproduction may result in fitness costs, though the mechanisms underlying these trade-offs are not well understood. Trade-offs during lactation may include reduced energetic allocation to cellular maintenance, immune response and survival, and may be influenced by resource limitation. 2. As the smallest marine mammal, sea otters (Enhydra lutris) have the highest mass-specific metabolic rate necessitating substantial energetic requirements for survival. To provide the increased energy needed for lactation, female sea otters significantly increase foraging effort, especially during late-lactation. Caloric insufficiency during lactation is reflected in the high numbers of maternal deaths due to End-Lactation Syndrome in the California subpopulation. 3. We investigated the effects of lactation and resource limitation on maternal stress responses, metabolic regulation, immune function and antioxidant capacity in two subspecies of wild sea otters (northern: E. l. nereis and southern: E. l. kenyoni) within the California, Washington and Alaska subpopulations. 4. Lactation and resource limitation were associated with reduced glucocorticoid responses to acute capture stress. Corticosterone release was lower in lactating otters. Cortisol release was lower under resource limitation and suppression during lactation was only evident under resource limitation. Lactation and resource limitation were associated with alterations in thyroid hormones. Immune responses and total antioxidant capacity were not reduced by lactation or resource limitation. Southern sea otters exhibited higher concentrations of antioxidants, immunoglobulins and thyroid hormones than northern sea otters. 5. These data provide evidence for allocation trade-offs during reproduction and in response to nutrient limitation but suggest self-maintenance of immune function and antioxidant defenses despite energetic constraints. Income-breeding strategists may be especially vulnerable to the consequences of stress and modulation of thyroid function when food resources are insufficient to support successful reproduction and may come at a cost to survival, and thereby influence population trends.
Data from: Development of a simple, practice-based tool to assess quality of paediatric emergency care delivery in resource-limited settings: identifying critical actions via a Delphi study
Objective: Provision of timely, high-quality care for the initial management of critically ill children in African hospitals remains a challenge. Monitoring the completion of critical actions during resuscitations can inform efforts to reduce variability and improve outcomes. We sought to develop a practice-based tool based on contextually relevant actions identified via a Delphi process. Our goal was to develop a tool that could identify gaps in care, facilitate identification of training and standardized assessment to support quality improvement efforts. Design: Six sentinel conditions were selected based on disease epidemiology and mortality at rural and urban African emergency departments. Potential critical actions were identified through focused literature review. These actions were evaluated within a three-round modified Delphi process. A set of logistical filters was applied to the candidate list to derive a practice-based tool. Setting and participants: Attendees at an international emergency medicine conference comprised an expert panel of 25 participants, with 84% working primarily in African settings. Consensus rounds allowing novel responses were conducted via online and in-person surveys. Results: The expert panel generated 199 actions that apply to six conditions in emergently ill children. Application of appropriateness criteria refined this to 92 candidate actions across seven categories: core skills, active seizure, altered mental status, diarrheal illness, febrile illness, respiratory distress, polytrauma. From these, we identified 28 actions for inclusion in a practice-based tool contextually relevant to the initial management of critically ill children in Africa. Conclusions: A group consensus process identified critical actions for severely ill children with select sentinel conditions in emergency paediatric care in an African setting. Absence of these actions during resuscitation might reflect modifiable gaps in quality of care. The resulting practice-based tool is context-relevant and can serve as a foundation for training and quality improvement efforts in African hospitals and emergency departments.
Data from: Teasing apart plant community responses to N enrichment: the roles of resource limitation, competition and soil microbes
Although ecologists have documented the effects of nitrogen enrichment on productivity, diversity and species composition, we know little about the relative importance of the mechanisms driving these effects. We propose that distinct aspects of environmental change associated with N enrichment (resource limitation, asymmetric competition, and interactions with soil microbes) drive different aspects of plant response. We test this in greenhouse mesocosms, experimentally manipulating each factor across three ecosystems: tallgrass prairie, alpine tundra and desert grassland. We found that resource limitation controlled productivity responses to N enrichment in all systems. Asymmetric competition was responsible for diversity declines in two systems. Plant community composition was impacted by both asymmetric competition and altered soil microbes, with some contributions from resource limitation. Results suggest there may be generality in the mechanisms of plant community change with N enrichment. Understanding these links can help us better predict N response across a wide range of ecosystems.
Ecological significance of marcescence in Himalayan plants: Why is standing dead phytomass more important in demanding, resource-limited environments?
<ol> <li>Understanding mechanisms allowing plants to thrive in challenging conditions is critical for predicting their responses to global environmental change. An often overlooked ecological adaptation is marcescence, where leaves and stems are retained beyond their typical shedding time, with implications for nutrient recycling and carbon sequestration. Marcescence may be common in plants with conservative resource-use strategies, especially in environments with limited resources and marked seasonality, such as deserts and alpine regions. However, the extent to which marcescence occurs in different taxa across different habitats and its relationship to seasonality and plant functioning as reflected in ecophysiological traits remains understudied.</li> <li>We studied 600 individuals across 40 Himalayan herb species found in desert, steppe, alpine, and subnival habitats, spanning 3000 to 5400 m elevation. Marcescence was observed in 37 out of 40 species evaluated, with 57% of the sampled individuals displaying dead phytomass, indicating the widespread nature of this phenomenon in Himalayan ecosystems.</li> <li>Environment and plant traits emerged as significant factors influencing the amount of dead-standing biomass. These findings remained robust after accounting for intraspecific variation and phylogenetic inertia. Desert and steppe species exhibited greater marcescent biomass, characterized by tenfold more dead stem biomass than alpine and subnival plants. In contrast, the proportion of dead phytomass increased with elevation due to increased dead leaf fraction.</li> <li>Tall desert species with heavily lignified stems and limited nitrogen and phosphorus content had high levels of stem marcescence, which may aid litter photodegradation and nutrient recycling but also deter herbivory and promote seed dispersal (tumbleweed). Conversely, slow-growing alpine and subnival plants with higher water-use efficiency, foliar C:N ratios, and parenchymatic storage tissue rich in mobile carbohydrates had a high proportion of marcescent leaves, which may aid survival in these resource-limited environments by providing delayed nutrients through moisture-conserving mulch that also provides insulation and protection from frost.</li> <li>This study provides new insights into the complex interplay of habitat, seasonality, and plant traits in dead phytomass retention in Himalayan plants. Marcescence is prevalent in resource-conservative species in highly seasonal, resource-limited habitats and may significantly shape the functioning and biodiversity of Himalayan ecosystems.</li> </ol>
Data from: Loss of fungal symbionts at the arid limit of the distribution range in a native Patagonian grass – resource ecophysiological relations
<p>1. Crucial to our understanding of plant ecology is the consideration of the eco-physiological responses and constraints of plant-fungal symbioses throughout the native distribution range of their host.</p> <p>2. We examined key eco-physiological roles of two co-occurring fungal symbionts [Epichloë endophytes and arbuscular mycorrhizal fungi (AMF)] in the endemic grass Hordeum comosum across a wide bioclimatic gradient and contrasting grazing severity. We sampled H. comosum plants along four humid-to-arid transects in Patagonia, Argentina, covering its entire distribution range and determined Epichloë presence, AMF root colonization, nitrogen and phosphorus concentration, intrinsic water use-efficiency (iWUE, the ratio of photosynthesis to stomatal conductance) and 18O-enrichment of cellulose in shoots.</p> <p>3. Root colonization by AMF increased with Epichloë-presence. All plants hosted Epichloë in the humid range of the gradient, but symbioses occurrence decreased towards arid sites which also displayed severe grazing symptoms at site level.</p> <p>4. Symbiosis with Epichloë correlated positively with shoot nitrogen concentration in the centre of the distribution range, and with shoot phosphorus concentration across the entire distribution range.</p> <p>5. The site-level relationship of AMF colonization with 18O-enrichment and iWUE suggested that mycorrhiza boosted stomatal conductance in humid environments but curbed it in arid environments.</p> <p>6. While the interpretation of interactions and potential causalities from observational studies should be done with caution, this study demonstrates distinct correlations between plant-fungal symbiont associations and key resource parameters (phosphorus, nitrogen, and iWUE vs 18O-enrichment). Such correlations may suggest particular functional roles for these symbionts in the ecology of their host plant.</p>

Complementary and Alternative Medicine Use and its Impact on Quality of Life among Cancer Patients in Freetown, Sierra Leone: Considerations for a Resource-Limited Setting
<p> </p> <p><span>Cancer patients are increasingly resorting to complementary and alternative medicine (CAM) to alleviate the effects of the disease and enhance their quality of life (QoL). However, to our knowledge, no study has investigated CAM use and its impact on QoL in patients with cancer in Sierra Leone. Therefore, this study aimed to assess the prevalence of and associated factors with CAM use, and compare the quality of life between CAM users and non-users.</span></p> <p><span>This cross-sectional descriptive study was conducted at three health facilities in Freetown viz Connaught Hospital Ola During Children's Hospital, and the Water and Stone Clinic. Data were collected through face-to-face interviews, using an adapted questionnaire. The QoL was assessed using the European Organization for Research and Treatment of Cancer tool. Descriptive analysis was employed, and binary logistic regression tests were used to explore the associated factors of CAM use, with statistical significance set at p<0.05.</span></p> <p><span>In total, 204 participants completed the study. The prevalence of CAM use was 48.5%. Among the various CAMs examined in this study, herbal medicines (48.8%) followed by special foods (19.4%) were the most commonly used forms of CAM. The primary reason for CAM use was the management of cancer complications (43.4%). Over half of the CAM users did not disclose their CAM use to their doctors (53.5%). There was a statistically significant association between CAM use and tertiary education (AOR=2.68, 95%CI=1.31-5.49, p=0.007) and chemotherapy treatment (AOR=1.91, 95%CI= 1.06-3.46, p=0.032). There was no considerable impact of CAM on QoL, although financial difficulty was statistically significant among CAM users compared with non-CAM users.</span></p> <p><span>This study revealed that CAM use is prevalent among cancer patients, with herbal medicines being the most commonly used form, followed by special foods and prayers. <span> </span>Patients primarily obtain information about CAM from family, friends, and relatives, and most do not discuss CAM use with their doctors. Tertiary education and chemotherapy were found to be predictors of CAM use. Regarding the impact of CAM on QoL, only financial difficulty was statistically significant.</span></p>

High resource overlap and small dietary differences are widespread in food-limited warbler communities
<p class="CxSpFirst">Although both interspecific competition and coexistence mechanisms are central to ecological and evolutionary theory, past empirical studies have generally focused on simple (two-species) communities over short time periods. Experimental tests of these species interactions are challenging in complex study systems. Moreover, a number of studies of 'imperfect generalists', consistent with Liem's Paradox, raise questions about the ability of evolved species differences to effectively partition niche space when resources vary considerably across the annual cycle. Here we used a recently developed theoretical framework to combine past research on population-level processes with observational data on resource use to test for ongoing interspecific competition and understand the nature of resource overlap. We compared species diet overlaps and differences in several distinctive communities centred on a focal species, the American Redstart <i>Setophaga ruticilla</i> replicated both spatially and seasonally, in combination with documentation of population regulation to assess the ability of similar species to partition dietary niche space and limit interspecific competition. Our results document high dietary overlap in most of the communities studied, with only subtle differentiation consistent with known species-differences in foraging behaviour and morphology. These findings are largely consistent with species foraging as imperfect generalists. However, in contrast to past studies, the high diet overlaps observed here during times of inferred resource scarcity were driven by low value prey taxa (e.g. small ants) and did not involve truly 'private' resources. All of these factors increase the potential negative impacts of interspecific competition and limit the ability of these birds to avoid competition if conditions further deteriorate, either seasonally or at longer intervals.</p>
Dataset: Resource limitation has a limited impact on the outcome of virus-fungus co-infection in an insect host
<p>Infection by pathogens is strongly affected by the diet or condition of the prospective host. Studies that examine the impact of diet have mainly focussed on single pathogens; however, co-infections within a single host are thought to be common. Different pathogen groups might respond differently to resource availability and diverse infections could increase the costs of host defence, meaning the outcome of mixed infections under varying dietary regimes is likely to be hard to predict. We used the generalist cabbage looper, <em>Trichoplusia ni</em> and two of its pathogens, the DNA virus <em>T. ni</em> nucleopolyhedrovirus (TniSNPV) and the entomopathogenic fungus, <em>Beauveria bassiana</em> to examine how nutrient reduction affected the outcome of mixed pathogen infection. We challenged insects with a low or high effective dose of virus, alone or combined with a single dose of fungus. We manipulated food availability after pathogen challenge by diluting artificial diet with cellulose, a non-nutritious bulking agent, and examined its impact on host and pathogen fitness. Reducing diet quantity did not alter overall or pathogen-specific mortality. In all cases, TniSNPV-induced mortality was negatively affected by fungus challenge. Similarly, <em>B. bassiana</em>-induced mortality was negatively affected by TniSNPV challenge, but only at the higher virus dose. Dietary dilution mainly affected <em>B. bassiana </em>speed of kill when mixed with a high dose of TniSNPV, with an increase in the duration of fungal infection when cellulose was low (high quantity). One pathogen dominated the production of transmission stages in the cadavers and co-infection did not affect the yield of either pathogen. There was no evidence that co-infections were more costly to the survivors of pathogen challenge. In conclusion, dietary dilution did not determine the outcome of mixed pathogen infection, but it had more subtle effects, that differed between the two pathogens and could potentially alter pathogen recycling and host-pathogen dynamics.</p>
