<p>This record contains data related to article &quot;Impact of delay in follow-up due to COVID-19 pandemic on skin cancer progression: a real-life experience from an Italian hub hospital &quot;</p> <p>Abstract</p> <p><strong>Background: </strong> The COVID-19 pandemic required the setting of a national lockdown in Italy from March 9, 2020, until May 18, 2020; therefore, most of the non-COVID-19 activities were postponed. As a consequence, many follow-up visits in patients with previously excised skin cancers were delayed. The aim of this retrospective real-life case-control study is demonstrating that delay in follow-up led to an increased incidence of advanced melanoma and keratinocyte cancers on the total of surgically excised skin cancers.</p> <p><strong>Methods: </strong> The incidence of excised advanced melanoma and keratinocyte cancers in our dermato-surgery division in the period May 18 to November 18, 2020, was compared to the same time span of 2019. All data were collected from the electronic database of our department. Odds ratio with 95% confidence interval was used to assess the risk of excised advanced skin cancers in 2020 compared to 2019.</p> <p><strong>Results: </strong> From May to November 2019, we performed 265 surgical excisions, while during the pandemic in 2020, we completed 280 surgeries. The number of advanced skin cancers excised between May 18 and November 18, 2020, was significantly higher compared with the same period in 2019 (54 vs. 22, OR: 2.64; 95% CI: 1.56-4.47; P = 0.0003). Significant differences were also observed regarding the number of surgically removed advanced BCCs (OR 2.15; 95% CI 1.14-4.07; P = 0.0187) and advanced SCCs (OR 4.60; 95% CI 1.31-16.18; P = 0.0175).</p> <p><strong>Conclusion: </strong> These results confirm that delay in follow-up and, consequently, postponed surgical excisions are related to an increased incidence of advanced skin tumors, resulting in poorer prognosis lifelong. Follow-up visits should be carried on even during COVID-19 pandemic, avoiding significant delays as much as possible.&nbsp;</p>

Mortality in an Italian nursing home during COVID-19 pandemic: correlation with gender, age, ADL, vitamin D supplementation, and limitations of the diagnostic tests

<p><strong>Introduction: </strong> The COVID-19 pandemic caused an increased mortality in nursing homes due to its quick spread and the age-related high lethality.</p> <p><strong>Results: </strong> We observed a two-month mortality of 40%, compared to 6.4% in the previous year. This increase was seen in both COVID-19 positive (43%) and negative (24%) residents, but 8 patients among those testing negative on the swab, tested positive on serological tests. Increased mortality was associated with male gender, older age, no previous vitamin D supplementation and worse &quot;activities of daily living (ADL)&quot; scores, such as Barthel index, Tinetti scale and S.OS.I.A.</p> <p><strong>Conclusion: </strong> Our data confirms a higher geriatric mortality due to COVID-19. Negative residents also had higher mortality, which we suspect is secondary to preanalytical error and a low sensitivity of the swab test in poorly compliant subjects. Male gender, older age and low scores on ADL scales (probably due to immobility) are risk factors for COVID-19 related mortality. Finally, mortality was inversely associated with vitamin D supplementation.</p> <p><strong>Design: </strong> In this observational study, we described the two-month mortality among the 157 residents (age 60-100) of a nursing home after Sars-CoV-2 spreading, reporting the factors associated with the outcome. <p>Impact of respiratory protective devices on respiration: Implications for<br> panic vulnerability during the COVID-19 pandemic</p> <p>Background: The wearing of respiratory protective devices (RPDs) correctly and continually in situations where<br> people are at risk of respiratory infections is crucial for infection prevention. Certain people are poorly compliant<br> with RPDs due to RPD-related annoyance, including respiratory discomfort. We hypothesized that individuals<br> vulnerable to panic attacks are included in this group. No published studies on this topic are available. The<br> evidence for our hypothesis was reviewed in this study as a starting point for future research.<br> Methods: We selected a set of experimental studies that measured the respiratory physiological burden in RPD<br> wearers through objective and validated methods. We conducted a bibliographic search of publications in the<br> PubMed database (January 2000&ndash;May 2020) to identify representative studies that may be of interest for panic<br> respiratory pathophysiology.<br> Results: Five studies were included. Wearing RPDs exerted significant respiratory effects, including increased<br> breathing resistance, CO2 rebreathing due to CO2 accumulation in the RPD cavity, and decreased inhaled O2<br> concentration. We discussed the implications of these effects on the respiratory pathophysiology of panic.<br> Limitations: Most studies had a small sample size, with a preponderance of young participants. Different<br> methodologies were used across the studies. Furthermore, differences in physical responses between wearing<br> RPDs in experimental settings or daily life cannot be excluded.<br> Conclusions: This research supports the idea that panic-prone individuals may be at higher risk of respiratory<br> discomfort when wearing RPDs, thereby reducing their tolerance for these devices. Strategies to decrease discomfort<br> should be identified to overcome the risk of poor compliance.</p>

Uncertain crisis time affects psychosocial dimensions in beta-thalassemia patients during Covid-19 pandemic: A cross-sectional study

<p>Submitted materials consist of study dataset (Dataset_Thalassemia.xsls, EXCEL format), Syntax of analyses performed (Syntax_Thalassemia.sps, SPSS format), Output of results (Logfile_Thalassemia.spv, SPSS format).</p> <p>Study dataset includes socio-demographic data of 43 beta-thalassemia patients and 86 controls and the clinical and psychosocial variables used in the study (i.e., distress, anxiety, depression, loneliness, coping strategies) as well as scores of perceived impact of Covid-19 outbreak and changes in life habits. Statistical analysis included group differences (one-way ANOVA) between patients and control subjects on: 1) psycho-socio-emotional measures including distress, depression and anxiety, loneliness and coping styles; 2) perceived impact of Covid-19 outbreak on health and economy; and 3) life habit changes, and in particular on the Social Habits, the Outdoor Habits and the Indoor Habits Scales. Study results no significant group differences (one-way ANOVA) in loneliness sub-scores, distress and depression variables. A significant group effect was found on anxiety scale and transcendent coping scale, with patients showing higher scores than control subjects in both measures. After multiple comparison correction, only the difference on transcendent coping scores remained significant. Significant group differences in the perceived impact of Covid-19 for economy was also found, showing that patients perceived the impact of Covid-19 for economy as more serious than control subjects. Finally, a significant group difference in the Outdoor Habits Scale emerged, with patients reporting greater changes in outdoor habits than controls. Outdoor Habits Scale correlated with DASS anxiety sub-scale.</p>

PMID: 33660030; PMCID: PMC7928172.</p> <p>Abstract</p> <p><strong>Purpose:&nbsp;</strong>The purpose of the present cross-sectional study is to investigate the role of perceived COVID-19-related organizational demands and threats in predicting emotional exhaustion, and the role of organizational support in reducing the negative influence of perceived COVID-19 work-related stressors on burnout. Moreover, the present study aims to add to the understanding of the role of personal resources in the Job Demands-Resources model (JD-R) by examining whether personal resources-such as the professionals&#39; orientation towards patient engagement-may also strengthen the impact of job resources and mitigate the impact of job demands.</p> <p><strong>Methods:&nbsp;</strong>This cross-sectional study involved 532 healthcare professionals working during the COVID-19 pandemic in Italy. It adopted the Job-Demands-Resource Model to study the determinants of professional&#39;s burnout. An integrative model describing how increasing job demands experienced by this specific population are related to burnout and in particular to emotional exhaustion symptoms was developed.</p> <p><strong>Results:&nbsp;</strong>The results of the logistic regression models provided strong support for the proposed model, as both Job Demands and Resources are significant predictors (OR = 2.359 and 0.563 respectively, with p &lt; 0.001). Moreover, healthcare professionals&#39; orientation towards patient engagement appears as a significant moderator of this relationship, as it reduces Demands&#39; effect (OR = 1.188) and increases Resources&#39; effect (OR = 0.501).</p> <p><strong>Conclusions:&nbsp;</strong>These findings integrate previous findings on the JD-R Model and suggest the relevance of personal resources and of relational factors in affecting professionals&#39; experience of burnout.</p>
