Clinical and radiographic course of arrested cerebral adrenoleukodystrophy
<p><b>Objective:</b> To gain insight into the natural history of arrested cerebral adrenoleukodystrophy (CALD), we quantified the change in Neurologic Function Score (NFS) and Loes Score (LS) over time in patients whose cerebral lesions spontaneously stopped progressing.</p> <p><b>Methods: </b>We retrospectively reviewed a series of 22 patients with arrested CALD followed longitudinally over a median time of 2.4 years (0.7 – 17.0). Primary outcomes were change in radiographic disease burden (measured by LS) and clinical symptoms (measured by NFS) between patients who never developed a contrast-enhancing lesion (GdE- subgroup), and those who did (GdE+ subgroup). Secondary analyses comparing patterns of neuroanatomical involvement and lesion number, and prevalence estimates, were performed.</p> <p><b>Results: </b>Cerebral lesions were first detected at a median age of 23.3 years old (8.0 – 67.6), with an initial LS of 4 (0.5 – 9). NFS was 0.5 (0 – 6). Overall change in NFS or LS per year did not differ between subgroups. No patients who remained GdE- converted to a progressive CALD phenotype. The presence of contrast enhancement was associated with disease progression (r<sub>s</sub> = 0.559, <i>p</i> < 0.001). Four patients (18.2%) underwent step-wise progression, followed by spontaneous resolution of contrast enhancement, and re-arrest of disease. Three patients (13.6%) converted to progressive CALD. Nineteen patients (86.4%) have arrested CALD at most recent follow-up. The prevalence of arrested CALD is 12.4%.</p> <p><b>Conclusion: </b>Arrested CALD lesions can begin in childhood, and patients are often asymptomatic early in disease. The majority of patients remain stable. However, clinical and MRI surveillance is recommended as a minority of patients undergo step-wise progression, or conversion to progressive CALD.</p>
Data from: Ecological and evolutionary significance of a lack of capacity for extended developmental arrest in crocodilian eggs
Hypoxia within the oviducts maintains embryonic arrest in turtles at the pre-ovipositional stage, which expands the timeframe over which nesting can occur without compromising embryo survival. The arrest can be extended post-oviposition through incubation of eggs in hypoxia. We determined whether crocodilian embryos have this same capacity. We also tested whether increased oxygen availability during incubation alters hatching success. We incubated freshly-laid saltwater crocodile (Crocodylus porosus) eggs (N = 83) at 32°C in one of five treatments; control (normoxia; 21% O2), 3-day and 6-day hypoxia (1% O2), or 3-day and 6-day hyperoxia (42% O2). Incubation (~82 days) was then completed in normoxia. There was a significant effect of treatment on survival of embryos through to hatching (p < 0.001). The hypoxic treatments resulted in almost no hatching (6.7% and 0% survival for the 3- and 6- day treatments respectively), while the hyperoxic and control treatments resulted in normal to high hatching success (86.6%, 100% and 64.2% for the control, 3- and 6- day hyperoxic treatments respectively). Unlike turtles, hypoxic incubation of crocodile eggs failed to delay development. Our results provide the first experimental evidence that, unlike turtles, crocodiles do not exhibit embryonic arrest when incubated under hypoxic conditions immediately following oviposition. An absence of embryonic arrest is of ecological and evolutionary significance, as it implies that crocodilians lack an ability to avoid adverse environmental conditions through delayed nesting and that, unlike turtles, embryonic arrest may not be a potential explanation for the lack of viviparity in the order Crocodylia.
Data from: Acute liver dysfunction after cardiac arrest
Few data are available regarding hypoxic hepatitis (HH) and acute liver failure (ALF) in patients resuscitated from cardiac arrest (CA). The aim of this study was to describe the occurrence of these complications and their association with outcome. All adult patients admitted to the Department of Intensive Care following CA were considered for inclusion in this retrospective study. Exclusion criteria were early death (<24 hours) or missing biological data. We retrieved data concerning CA characteristics and markers of liver function. ALF was defined as a bilirubin >1.2 mg/dL and an international normalized ratio ≥1.5. HH was defined as an aminotransferase level >1000 IU/L. Neurological outcome was assessed at 3 months and an unfavourable neurological outcome was defined as a Cerebral Performance Categories (CPC) score of 3-5. A total of 374 patients (age 62 [52-74] years; 242 male) were included. ALF developed in 208 patients (56%) and HH in 27 (7%); 24 patients developed both conditions. Patients with HH had higher mortality (89% vs. 51% vs. 45%, respectively) and greater rates of unfavourable neurological outcome (93% vs. 60% vs. 59%, respectively) compared to those with ALF without HH (n=184) and those without ALF or HH (n=163; p=0.03). Unwitnessed arrest, non-shockable initial rhythm, lack of bystander cardiopulmonary resuscitation, high adrenaline doses and the development of acute kidney injury were independent predictors of unfavourable neurological outcome; HH (OR: 16.276 [95% CIs: 2.625 - 81.345; p=0.003), but not ALF, was also a significant risk-factor for unfavourable outcome. Although ALF occurs frequently after CA, HH is a rare complication. Only HH is significantly associated with poor neurological outcome in this setting.

The European Arrest Warrant Before the Irish Courts: Datasets covering 2010 - 2020
<p>The below datasets have been published as part of the following paper: <em>Imelda Maher and Rónán Riordan,</em> and Neža Šubic, ‘The European Arrest Warrant Before the Irish Courts: Judicial Dialogue, Mutual Trust, and the Limits of Interpretation’ (2022) Irish Jurist (67) 14 – 54. </p> <p> </p> <p><strong>Excel dataset entitled "IE_EAW_2010_2020_DATASET_SUPERIOR_COURTS" </strong>codes cases before the Irish Superior Courts between 2010-2020. The dataset includes only case law which concerns the European Arrest Warrant Framework Decision ("EAW FD") and cases which implement the relevant domestic legislation underpinning its application within the Irish jurisdiction. </p> <p>The excel file entitled <strong>"IE_EAW_267_PRELIM_REFS_2010_2020"</strong> complies references from Irish Courts to the Court of Justice of the European Union via the preliminary reference procedure under Article 267 of the Treaty on the Functioning of the European Union with regard to the EAW FD.
