Assessment of Potential Risk Factors for COVID-19 among Health Care Workers in a Health Care Setting in Delhi, India

<p><strong><em>Executive summary</em></strong></p> <p>The novel coronavirus SARS-CoV2 (COVID-19), first detected by Wuhan Municipal Health Commission, China, in Wuhan, Hubei Province in December 2020 and eventually the disease became pandemic. It was declared as Public Health Emergency of International Concern (PHEIC) by WHO in January 2020. The COVID-19 disease primarily spreads through droplets of saliva or discharge from the nose when an infected person coughs or sneezes. People infected with the COVID-19 virus experiences mild, moderate or serious respiratory illness.&nbsp;</p> <p>Health workers play a critical role in the clinical management of patients with COVID-19 and hence are likely to be the most vulnerable for contracting the disease. Therefore,&nbsp;investigating the extent of infection in health care settings and identifying risk factors for infection among health workers along with follow-up within a facility in which a confirmed case of COVID-19 infection is receiving care can provide useful information on virus transmissibility and routes of transmission, and will bear important step in limiting amplification events in health care facilities.&nbsp;</p> <p>&nbsp;</p> <p><strong>Objectives:</strong></p> <p>1. To find out the extent of human-to-human transmission of the SARS-CoV-2 infection among health workers<br> 2. To study the clinical presentations of COVID-19 infection and the risk factors for infection among health workers.<br> 3 To evaluate the effectiveness of infection prevention and control measures among health workers in protecting against COVID-19.<br> 4. To evaluate the effectiveness of infection prevention and control programmes at health facility level&nbsp;<br> 5. To determine the serological response of health workers with symptomatic and possibly asymptomatic COVID-19 infection.</p> <p>&nbsp;</p> <p><strong>Materials and Methods:</strong></p> <p>This was a prospective cohort study conducted over a period of seven months, from December 2020 to June 2021, the period covering India&rsquo;s deadly second wave of COVID-19 pandemic. This was done among the health care workers working in HIMSR &amp; HAHC hospital, a tertiary health care setting (Dedicated COVID-19 Hospital) providing care to patients with a laboratory-confirmed COVID-19 infection. This hospital located in South East Delhi has 200 bedded COVID-19 Care Hospital and 1050 registered healthcare workers who come in contact with COVID-19-infected persons. The study population (sampling frame) included all the health personnel like doctors, nurses, paramedical staff, housekeeping staff, security staff, students of medical, nursing and paramedical sciences and other front office staff who come in contact with the patients. In this study, the first visit / interview (Baseline) was done when the staff came in contact with a confirmed COVID-19 case. The second visit / interview (Endline) was done between 22-28 days. During each of these two visits, biological sample in the form of serum was collected to check the presence of anti-COVID-19 antibodies</p> <p>&nbsp;</p> <p><strong>Results:</strong></p> <p>A total of 192 HCW were recruited in this study. All of them were interviewed and blood was collected for serology at the baseline visit as well as at endline. Out of 192 participants, 119 (61.97%) were detected with SARS-CoV2 antibodies at baseline whereas 73 (38.02%) were seronegative.&nbsp;&nbsp;Again, on22-28 days of follow-up, the seropositivity was 77.7% at the endline. We found that seropositivity was significantly and negatively associated with doctor as profession [OR:0.353, CI:0.176-0.710], COVID-19 symptoms [OR:0.210, CI:0.054-0.820], comorbidities [OR:0.139 , CI: 0.029 - 0.674], recent IPC Training [OR:0.250, CI:0.072 -0.864] , while positively associated with Partially [OR:3.303,CI: 1.256-8.685], as well as fully Vaccinated for COVID-19 [OR:2.428, CI:1.118-5.271]. We also observed seroconversion among 36.7% while 64.0% had increase in titre of antibodies during our follow-up period. The seroconversion was 63.2% in doctors, 42.9% in nurses and 13.0% in paramedics staff. Seroconversion was positively associated with doctor as profession [OR:11.43, CI:2.47 - 52.79] and with partially, as well as fully vaccinated for COVID-19 [OR: 32.63, CI: 5.11 - 208.49]. None of the HCW who were smokers and with any comorbidity did not found to have been seroconversion. We observe a negative and significant relationship of increase in titre of antibodies with recent any ILI symptoms [OR:0.17, 0.13 - 0.94], smokers[OR: 0.35, 95%CI: 0.13 - 0.94], HCW with comorbidities [OR:0.08,95CI: 0.01&nbsp;&nbsp;- 0.71],, recent full IPC Training [OR:0.07, CI:0.01 -0.63] , while positively associated with partially [OR: 7.87, 95CI: 2.18 - 28.40)], as well as fully Vaccinated for COVID-19 [OR: 3.59, 95CI: 1.46 - 8.87].&nbsp;Majority of the health care worker enrolled in our study had&nbsp;close contact exposure with COVID-19 patients while 5 had indirect exposure. It was observed that almost all (100% in both) doctors and nurses as well as almost all paramedical staff (99%) were wearing some kind of personal protective equipment (PPE) when they were exposed to a COVID-19 patient. We did not found adherences to any of the infection prevention measure adopted by the enrolled HCW during the recent contact with COVID-19 patients to be significantly associated with seroconversion.&nbsp;</p> <p><strong>Conclusion:</strong></p> <p>Majority of the health care worker (67% doctor, 80% nurses &amp; 55% paramedics) enrolled in our study had&nbsp;close contact exposure with COVID-19 patient. The results show that among&nbsp;192 HCW enrolled, 62% were seropositive at the baseline.&nbsp;&nbsp;At end line the seropositivity was increased to 77.7%. The seroconversion rate was also studied. It was found to be 36.7% in our study population (63.2% in doctors, 42.9% in nurses and 13.0% in paramedic&rsquo;s staff.). Adherence to the recommended IPC measures was reported by most participants.&nbsp;About two third (63%) of the HCW in our study were not vaccinated against COVID-19; nurses and paramedics were higher in proportion among those who were unvaccinated. Fifteen percentage were partially vaccinated and 22% were fully vaccinated against COVID-19, with doctors comprising majority among them. We also found that vaccination had the strongest association with seropositivity, seroconversion as well as serial rise of titre.</p>

Response to COVID-19: Clients' perspectives on the utilization of reproductive, maternal and child health care services in Nigeria

<p>This dataset was a part of a cross-sectional descriptive study in 320 rural communities in 32 Local Government Areas (LGAs) across the Federal Capital Territory (FCT), Abuja, and 9 out of the 36 Nigerian States in 2020. The dataset contains views of women who used primary health care centres in the selected LGAs for maternal and child health care, and family planning services before, during, and after the COVID-19 pandemic lockdown in Nigeria. The sample size was determined using the Yamane sample size formula. The estimated sample size per State was 384 (3,840 for nine States and FCT), with a 10% adjustment for non-response, the total sample size was 422 per State or location (4220 for the nine States and FCT). The data are stored in SPSS format.&nbsp;<br> The study was an initiative of UNFPA Nigeria. They coordinated it under the One UN Basket fund to respond to COVID-19 and implemented it under the supervision of three national NGOs: The Women&rsquo;s Health and Action Research Centre (WHARC), Education as a Vaccine (EVA), and the Planned Parenthood Federation of Nigeria (PPFN).&nbsp;&nbsp;</p> <p>&nbsp;</p>

Brood parasites that care: alternative nesting tactics in a subsocial wasp

<div> <p>Hosts and brood parasites are a classic example of conflict. Parasites typically provide no offspring care after laying eggs, imposing costs on hosts. Female subsocial wasps, <em>Ammophila pubescens</em>, alternated between initiating their own nests and an 'intruder' tactic of replacing eggs in nests of unrelated conspecifics. Hosts could respond by substituting new eggs of their own, with up to eight reciprocal replacements. Remarkably, intruders usually provisioned offspring in host nests, often alongside hosts. We used field data to investigate why intruders provision and to understand the basis of interactions. We found that intruders could not increase their fitness payoffs by using the typical brood parasite tactic of not provisioning offspring. Intruders using the typical tactic would benefit when hosts provisioned in their stead, but their offspring would starve when hosts failed to provision. Although some hosts obtained positive payoffs when intruders mistakenly provisioned their offspring, on average utilizing a conspecific nest represents parasitism: hosts pay costs while intruders benefit. Both females used the same tactic of egg replacement, but intruders more often laid the final egg. Selection should favour better discrimination of offspring, which could lead to repeated cycles of costly egg replacement.</p> </div>

A Double-Edged Sword: Parental care increases risk of offspring infection by a maternally-vectored parasite

<p>Parental care can protect offspring from predators but can also create opportunities for parents to vector parasites to their offspring. We hypothesized that the risk of infection by maternally-vectored parasites would increase with the frequency of mother-offspring contact. Ammophila spp. wasps (Hymenoptera: Sphecidae) build nests in which they rear single offspring. Ammophila species exhibit varied offspring provisioning behaviors: some species enter the nest once to provision a single, large caterpillar, whereas others enter the nest repeatedly to provision with many smaller caterpillars. We hypothesized that each nest visit increases the risk of offspring parasitism by Paraxenos lugubris (Strepsiptera: Xenidae), whose infectious stages ride on the mother wasp (phoresy) to reach the vulnerable Ammophila offspring. We quantified parasitism risk by external examination of museum-curated Ammophila specimens—the anterior portion of P. lugubris protrudes between the adult host's abdominal sclerites and reflects infection during the larval stage. As predicted, Ammophila species that receive larger numbers of provisions incur greater risks of parasitism, with nest provisioning behavior explaining ca. 90% of the interspecific variation in mean parasitism. These findings demonstrate that parental care can augment, rather than reduce, risk of parasite transmission to offspring.</p>

Coordination of care by breeders and helpers in the cooperatively breeding long-tailed tit, Aegithalos caudatus

<p><span>In species with biparental and cooperative brood care, multiple carers cooperate by contributing costly investment to raise a shared brood. However, shared benefits and individual costs also give rise to conflict among carers over investment. Coordination of provisioning visits has been hypothesized to facilitate the resolution of this conflict, preventing exploitation, and ensuring collective investment in the shared brood. We used a 26-year study of long-tailed tits, <em>Aegithalos caudatus</em>, a facultative cooperative breeder, to investigate whether care by parents and helpers is coordinated, whether there are consistent differences in coordination between individuals and reproductive roles, and whether coordination varies with helper relatedness to breeders. Coordination takes the form of turn-taking (alternation) or feeding within a short time interval of another carer (synchrony), and both behaviors were observed to occur more than expected by chance, i.e. 'active' coordination. First, we found that active alternation decreased with group size while active synchrony occurred at all group sizes. Secondly, we show that alternation was repeatable between observations at the same nest, while synchrony was repeatable between observations of the same individual. Active synchrony varied with reproductive status, with helpers synchronizing visits more than breeders, although active alternation did not vary with reproductive status. Finally, we found no significant effect of relatedness on either alternation or synchrony exhibited by helpers. In conclusion, we demonstrate active coordination of provisioning by carers and conclude that coordination is a socially plastic behavior depending on reproductive status and the number of carers raising the brood.</span></p>

Effects of season length and uniparental care efficiency on the evolution of parental care

<p>Parental care patterns differ enormously among and even within species. In Chinese penduline tits (<em>Remiz pendulinus</em>), for example, biparental care, female-only care, male-only care, and biparental desertion all occur in the same population; moreover, the distribution of care patterns differs systematically between populations. By means of an individual-based model, we show that such diversity can readily evolve. We report five main findings. First, under a broad range of parameters, different care patterns (e.g. male care and biparental care) coexist at equilibrium. Second, for many parameters, alternative evolutionary outcomes are possible; this can explain differences in care patterns across populations. Third, rapid evolutionary transitions can occur between alternative equilibria; this can explain the often-reported evolutionary lability of parental care patterns. Fourth, season length has a strong but non-monotonic effect on the evolved care patterns. Fifth, when uniparental care efficiency is low, biparental care tends to evolve; however, in many scenarios uniparental care is still common at equilibrium. Our study sheds new light on Triver's hypothesis that the sex with the highest pre-zygotic investment is predestined to invest a lot post-zygotically as well. We also discuss the implications of climate change, which simultaneously affects season length and efficiency of parental care.</p>

Point-of-care monitoring of head and neck cancer treatment response and recurrence development using nanopore-based ctDNA consensus sequencing

<p>Circulating tumor DNA (ctDNA) in blood may become a generic biomarker for non-invasive cancer diagnosis and monitoring. However, detection of ctDNA is challenged by the presence of many circulating DNA molecules from healthy cells. We found that single ctDNA molecules can be sequenced with high accuracy by a three-step process consisting of capturing, copying and concatenation of the original double-stranded ctDNA molecules. This innovative approach - called CyclomicsSeq -&nbsp; is unparalleled by any other method in terms of cost-efficiency and speed, allowing point-of-care cancer diagnostics.</p> <p>Within this CPOC, subsidized by the Oncode institute, we have applied our CyclomicsSeq ctDNA test in patients with advanced head and neck cancer squamous cell carcinoma (HNSCC). Head and neck cancer (HNSCC) accounts for 380,000 cancer-related deaths worldwide. For these patients, determining whether a patient responds to the primary chemoradiation treatment is challenging, and non-responders are sometimes identified when other treatment options are no longer possible. By measuring the ctDNA levels in the blood of these patients prior to and during treatment, we aim to identify non-responders at an earlier stage.</p> <p>This dataset contains base calls of TP53 of 47&nbsp;nanopore sequencing runs. We included 10 patients and 7 controls. For the patients, we have samples of multiple time points (0 = prior to treatment, 1 = 1 week after treatment initiation, etc).</p>

Do Carefully-timed Email Messages Increase Accuracy and Precision in Citizen Scientists' Reports of Events?

<p>Periodic messages are a commonly used tactic for reminding citizen science program participants to take a desired action such as collecting observations. In this study, we evaluate the impact of such messages on the accuracy and precision of observations contributed to <em>Nature&rsquo;s Notebook</em>, a citizen science phenology observing program.</p> <p>To encourage participants in <em>Nature&rsquo;s Notebook</em> to log the timing of leaf-out and flowering with maximum accuracy and precision, we email observers three days prior to when the events were expected to occur based on forecast models. Unplanned interruptions to the scripts driving these email prompts allowed us to evaluate whether the messages had the intended impacts.</p>

Use of health care services in community-dwelling older adults in two regions of Spain

<p>This dataset contains data on use of health care resources of community-dwelling older adults aged 70 or over, who were functionally independent. Data of health resources use included contacts along two consecutive years&nbsp;with: the general practitioner, primary care nurse, the specialists, visits to emergency rooms,&nbsp;and hospital admissions and length of stay. The data included also information about sex, region, polipharmacy, age-adjusted Charlson Comorbidity Index and funcionality, measured by Timed Up and Go test. The data collection was performed in two Spanish regions. Baseline assessment was done between 2015 and 2016, and patients were followed for 2 years. There were in total 1488 registries considering both years.</p>

A New Approach for an Old Idea - Professor Kelvin Tsoi (The Chinese University of Hong Kong, School of Public Health and Primary Care)</p> <p>Bio: Professor Kelvin Tsoi is an Epidemiologist specialized in Digital Health. His research interests focus on digital innovation in chronic disease management, including mobile and telecare application for hypertension management, technological implementation and social engagement for cognitive screening, artificial intelligent application on electronic health records. He also works as the traditional epidemiologist on evidence-based medicine and population cohort studies. He obtained his Bachler Degree from Department of Statistics and Doctor of Philosophy from School of Public Health in the Chinese University of Hong Kong. He further received post-doctoral training in the Division of Gastroenterology and Hepatology, Department of Medicine and Therapeutics. He was also appointed as a Director of CUHK JC Bowel Cancer Education Centre to promote colorectal cancer screening. In 2011, he worked as a research scientist in Hospital Authority. He led projects covering a wide range of service areas on chronic diseases, such as service demand projection for schizophrenia and dementia. The experience of database management enhanced his understanding of the HA database structures. In 2013, he was invited to join the interdisciplinary team for Big Data research and worked closely with a team of engineers and data scientists. Genotypic sex shapes maternal care in the African Pygmy mouse, Mus minutoides

<p><span>Sexually dimorphic behaviours, such as parental care, have long been thought to be </span><span>mainly</span><span> driven by gonadal hormones. In the past two decades, a few studies have challenged this view, highlighting the direct influence of the sex chromosome complement (XX vs XY or ZZ vs ZW). The African pygmy mouse, </span><span>Mus minutoides</span><span>, is a wild mouse species with naturally occurring XY sex reversal induced by a third, feminizing X* chromosome, leading to three female genotypes: XX, XX* and X*Y. Here, we show that sex reversal in X*Y females shapes a divergent maternal care strategy (maternal aggression, pup retrieval and nesting behaviours) from both XX and XX* females. Although neuroanatomical investigations were inconclusive, we show that the dopaminergic system in the anteroventral periventricular nucleus of the hypothalamus is worth investigating further as it may support differences in pup retrieval behaviour between females. Combining </span><span>behaviours</span><span> and neurobiology in a rodent subject to natural selection, we evaluate potential candidates for the neural basis of maternal behaviours and strengthen the underestimated role of the sex chromosomes in shaping sex differences in brain and behaviours. All things considered, we further highlight the emergence of a third sexual phenotype, challenging the binary view of phenotypic sexes.</span></p>

Data for: Selection on the joint action of pairs leads to divergent adaptation and coadaptation of care-giving parents during pre-hatching care

<p>The joint actions of animals in partnerships or social groups evolve under both natural selection, from the wider environment, and social selection, imposed by other members of the pair or group. We used experimental evolution to investigate how jointly expressed actions evolve upon exposure to a new environmental challenge. Our work focused on the evolution of carrion nest preparation by pairs of burying beetles <em>Nicrophorus vespilloides</em>, a joint activity undertaken by the pair but typically led by the male. In previous work, we found that carrion nest preparation evolved to be faster in experimental populations without post-hatching care (No Care lines) than with post-hatching care (Full Care lines). Here we investigate how this joint activity evolved. After 15 generations of experimental evolution, we created heterotypic pairs (No Care females with Full Care males, and No Care males with Full Care females) and compared their carrion nest making with homotypic No Care and Full Care pairs. We found that pairs with No Care males prepared the nest more rapidly than pairs with Full Care males, regardless of the female's line of origin. We discuss how social coadaptations within pairs or groups could act as a post-mating barrier to gene flow.</p>

Data and code for: a behavioural and microbiological study of wound care in Camponotus floridanus

<p><span>Open wounds pose a major infection and mortality risk in animals. To reduce these risks, many animal species apply antimicrobial compounds on their wounds.<sup> </sup>Ant societies use antimicrobial secretions from the metapleural gland to combat pathogens<sup> </sup>but this gland has been lost over evolutionary time in several genera including <em>Camponotus</em>. Using behavioral and microbiological experiments, we studied how <em>Camponotus floridanus</em> handles infected wounds without the use of antimicrobial secretions. When we experimentally injured a worker's leg at the femur, nestmates amputated the injured limb by biting the base (trochanter) of the leg until it was severed, thereby significantly increasing survival compared to ants that did not receive amputations. However, when the experimental injury was more distal (at the tibia), nestmates did not amputate the leg and instead directed more wound care to the injury site. Experimental amputations also failed to improve survival in ants with infected tibia injuries unless the leg was amputated immediately after pathogen exposure. Micro CT-scans revealed that the muscles likely responsible for leg hemolymph circulation are predominantly in the femur. Thus, it is likely that femur injuries, by attenuating hemolymph flow, provide sufficient time for workers to perform amputations before pathogen spread. Overall, this study provides the first example of the use of amputations to treat infected individuals in a non-human animal and demonstrates that ants can adapt their type of treatment depending on the location of wounds.</span></p>
