Quantitative Cervical Cord MRI in Spinal Muscular Atrophy: a sensitive imaging biomarker of Disease Evolution and Treatment
<p>Background Nusinersen is a promising therapy for spinal muscular atrophy (SMA), a rare disease characterized by the degeneration of alpha motor neurons in the spinal cord (SC), leading to progressive muscle atrophy and paralysis. This study aimed to validate cervical SC MRI as an imaging biomarker for assessing SMA severity, progression, and response to nusinersen treatment.</p> <p>Methods Five pediatric patients and three adult patients with SMA underwent clinical assessment and SC MRI at multiple time points during treatment with nusinersen. Total cord area (TCA), grey matter area (GMarea), and magnetization transfer saturation (MTsat) were measured at cervical SC levels.</p> <p>Results In pediatric patients, TCA increased significantly at all cervical levels during the observation period (C4-C5, p<0.001), whereas GMarea showed a significant increase only at the C3-C4 level (p=0.043). TCA was found to correlate with motor scales, particularly with measures of upper limb motor function.<span> </span>Pediatric patients demonstrated higher TCA values compared to the SC growth rates reported in healthy controls in the literature, especially during the early part of the observation period. GM MTsat values correlated with clinical scores. In adults, TCA and GMarea strongly correlated with clinical measures.</p> <p>Conclusion This study demonstrates that nusinersen has a positive effect on both adult and pediatric patients, with more pronounced effects in the latter, improving motor function and SC growth. SC MRI metrics reflect disease severity and correlate strongly with clinical scales, providing a valuable imaging marker for monitoring treatment response in SMA and evaluating emerging therapies.</p>
Quantitative MRI assessment of peripheral nerve injury in ATTRv amyloidosis patients
<p>Background and Objectives: Early detection of peripheral nerve damage in patients with hereditary transthyretin amyloidosis (ATTRv) has become essential for the prompt initiation of effective therapies that have been recently approved. In our study, we propose a new variable echo time (vTE) MRI sequence for aiding in the quantitative assessment of nerve damage. The approach allows the detection of signals from short T2 components of the nerve, such as the epineurium, myelin, and possibly pre-fibrillar amyloid material deposition within the nerve.</p> <p>Methods: In this cohort study, twenty patients with clinically confirmed ATTRv polyneuropathy (PNP) and twenty-one healthy controls (HC) underwent 3T MRI. vTE was performed on the right thigh with a 6 cm longitudinal coverage of the sciatic nerve between the gluteal region and the popliteal fossa. From the vTE acquisition, cross-sectional area (CSA) of epineurium and fascicular regions were calculated manually and the corresponding pseudoT2* (pT2*), a parameter related to the intrinsic T2 relaxation time of a given tissue, was extrapolated from the two acquired echoes of the vTE.</p> <p>Results: Significantly higher CSA, fascicular area, and pT2* were found in the ATTRv group compared to healthy controls. pT2* also correlated with previously validated clinical outcome measures (i.e., Polyneuropathy Disability Scoring System, Neuropathy Impairment Score (NIS), and NIS items related to lower limbs), and with nerve conduction parameters, demonstrating the ability to discriminate ATTRv patients with different degrees of PNP from HC.</p> <p>Conclusion: In conclusion, the vTE sequence provides novel and reliable imaging markers capable of detecting early nerve microstructural changes that relate to disease onset and severity.</p>

Texture analysis and machine learning to predict water T2 and fat fraction from non-quantitative MRI of thigh muscles in Facioscapulohumeral muscular dystrophy
<p><strong>Introduction</strong>. This database includes the radiomic features used as covariates to train machine learning algorithms in the paper “ Texture analysis and machine learning to predict water T2 and fat fraction from non-quantitative MRI of thigh muscles in Facioscapulohumeral muscular dystrophy”.</p> <p><strong>Purpose</strong>. Quantitative MRI (qMRI) plays a crucial role for assessing disease progression and treatment response in neuromuscular disorders, but the required MRI sequences are not routinely available in every center. The aim of this study was to predict qMRI values of water T2 (wT2) and fat fraction (FF) from conventional MRI, using texture analysis and machine learning.</p> <p><strong>Method</strong>. Fourteen patients affected by Facioscapulohumeral muscular dystrophy were imaged at both thighs using conventional and quantitative MR sequences. Muscle FF and wT2 were calculated for each muscle of the thighs. Forty-seven texture features were extracted for each muscle on the images obtained with conventional MRI. Multiple machine learning regressors were trained to predict qMRI values from the texture analysis dataset.</p> <p><strong>Results</strong>. Eight machine learning methods (linear, ridge and lasso regression, tree, random forest (RF), generalized additive model (GAM), k-nearest-neighbor (kNN) and support vector machine (SVM) provided mean absolute errors ranging from 0.110 to 0.133 for FF and 0.068 to 0.115 for wT2. The most accurate methods were RF, SVM and kNN to predict FF, and tree, RF and kNN to predict wT2.</p> <p><strong>Conclusion</strong>. This study demonstrates that it is possible to estimate with good accuracy qMRI parameters starting from texture analysis of conventional MRI.</p>
