Rates of rehospitalisation in the first two years among preterm infants discharged from the NICU of a tertiary children hospital in Vietnam – A follow-up study
<p><b>Objectives</b> To describe the characteristics of rehospitalisation in Vietnamese preterm infants, and to examine the time-to-first-readmission between two gestational age (GA) groups (extremely/very preterm, EVP, versus moderate/late preterm, MLP). Further, to compare rehospitalisation rates according to GA and corrected age (CA), and to examine the association between potential risk factors and rehospitalisation rates.</p> <p><b>Design and Setting</b> Cohort study to follow up preterm infants discharged from a neonatal intensive care unit (NICU) of a tertiary children's hospital in Vietnam.</p> <p><b>Participants</b> All preterm newborns admitted to the NICU from July 2013 to September 2014. </p> <p><b>Main outcomes</b> Rates, durations and causes of hospital admission during the first two years. </p> <p><b>Results</b> Of 294 preterm infants admitted to NICU (all out-born, GA ranged from 26 to 36 weeks), 255 were discharged alive, and 211 (83%) NICU graduates were followed up at least once during the first two years CA, of whom 56% was hospital readmitted. Median (interquartile range) of hospital stay was 7 (6 to 10) days. Respiratory diseases were the major cause (70%). Compared with MLP infants, EVP infants had a higher risk of first rehospitalisation within the first 6 months of age (p = 0.01). However, the difference in risk declined thereafter and was similar from 20 months of age. There was an interaction in rehospitalisation rates between GA and CA. Longer duration of neonatal respiratory support and having older sibling were associated with higher rehospitalisation rates. Lower rates of rehospitalisation were seen in infants with higher cognitive and motor scores (not statistically significant in cognitive scores).</p> <p><b>Conclusions</b> Hospital readmission of Vietnamese preterm infants discharged from NICU was frequent during their first two years, mainly due to respiratory diseases. Scale-up of follow-up programmes for preterm infants are needed in LMICs and attempts to prevent respiratory diseases should be considered.</p>
Data from: Post-operative follow-up for selected diffuse low-grade gliomas with WHO grade III/IV foci
Objective: Diffuse low-grade gliomas (DLGG) are defined by a continuous growth and an almost unavoidable malignant transformation. Foci of malignant glioma can be found within DLGG samples obtained from surgical resections. As the medical management of patients is classically based on the higher tumor grade, an immediate adjuvant treatment is usually proposed. To determine whether postponing the medical treatment in selected patients is feasible, we conducted a single-center retrospective study. Methods: Single-center retrospective analysis of a consecutive series of DLGG managed with this conservative strategy. Inclusion criteria were: at least one focus of malignant tumor (grade III-IV, WHO 2016), no previous chemotherapy or radiotherapy, no less than a subtotal resection of the FLAIR tumor volume, no intention of treating with immediate adjuvant therapy, minimum two years of follow-up. The time interval to the following oncological medical treatment was analyzed, as well as the functional and survival results. Results: 45 patients met the inclusion criteria (median age 36.5, median time interval from diagnosis: 7.3 months). Most tumors (86.7%) were IDH-mutant and 1p19q intact (60.0%); 10 presented with grade IV foci. With a median follow-up of 6.3 years, 75.5% of patients received a subsequent medical treatment, after a median time of 3.7 years since surgery. At the time of analysis, 9 patients (20.0%) had died (5-years and 7-years survival rates: 95.2% and 67.0%). Most surviving patients were still active professionally, without seizures. Conclusions: Postponing the medical treatment in DLGG with foci of malignant tumor following total or subtotal resection should be considered in selected patients.
Data from: Axial length elongation and myopia incidence increase in primary school-age children: 3-year follow-up study
<p>Objective: To investigate the axial length (AL) elongation in primary school-age children during 3-year follow-up period, and evaluate the associations of AL elongation with spherical equivalent (SE), AL at baseline, body height and weight.</p> <p>Design: A 3-year observational cohort study from 2014 to 2017.</p> <p>Setting: Jinshan Hospital of Fudan University in Shanghai.</p> <p>Methods: A total of 452 children successfully completed their measurements in the 3-year follow-up period. Among those children, the mean age was 6.9 ± 0.7 years, ranging from 6 years to 8 years, and 217 (42.7%) were boys. AL was measured with an ocular biometry system. Refractive error was measured using an auto-refractor without cycloplegia.</p> <p>Results: The mean changes of ALs were 0.27 ± 0.28 mm, 0.52 ± 0.40 mm, and 0.89 ± 0.51mm over 1 year, over 2 years and over 3 years, respectively. The mean changes of Spherical equivalents (SEs) were -0.27 ± 0.80 D, -0.56 ± 1.00 D, and -0.95 ± 1.41 D over 1 year, over 2 years and over 3 years, respectively. Multivariate linear regression analysis revealed that mean change of AL was associated with mean change of SE at all points (all P < 0.001). In addition, linear regression analysis revealed that AL elongation in the 3-year follow-up period was associated with AL at baseline (R2 = 0.009, P = 0.045).</p> <p>Conclusions: AL elongation is relatively high in the primary school-age children in Jinshan District, Shanghai. Effect strategies are needed to control AL elongation.</p> <p> </p>
Data from: Analysis of personal and family factors in the persistence of attention deficit hyperactivity disorder: results of a prospective follow-up study in childhood
Objectives: To study the course of ADHD during childhood and analyze possible personal and family predictor variables of the results. Method: Sixty-one children with ADHD who were between 6 and 12 years old at the baseline assessment were evaluated 30 months later (mean age at baseline: 8.70 ± 1.97; mean age at follow-up: 10.98 ± 2.19). Status of ADHD in follow-up was identified as persistent (met DSM-IV-TR criteria according to parents' and teachers' ratings), contextually persistent (met ADHD criteria according to one informant, and there was functional impairment) and remitted ADHD (with subthreshold clinical symptomatology). Associated psychological disorders of the three groups were analyzed in the follow-up with the Conners' Rating Scales. The groups were compared on ADHD characteristics (symptoms of ADHD and impairment), child psychopathology, executive functioning (EF; inhibition, working memory) and parenting characteristics (parental stress and discipline styles) at baseline. Results: At the follow-up, 55.7% of the children continued to meet the DSM-IV-TR criteria for ADHD, 29.5% showed contextual persistence, and 14.8% presented remission of the disorder. The persistent and contextually persistent ADHD groups showed more associated psychological disorders. Inattention, oppositional problems, cognitive problems and impairment at baseline distinguished the remitted ADHD children from the persistent and contextually persistent ADHD children. Moreover, the persistent groups had significantly more emotional liability and higher parental stress than the group in remission, while no differences in EF where found among the groups. Conclusions: ADHD children continue to present symptoms, as well as comorbid psychological problems, during adolescence and early adulthood. These findings confirm that persistence of ADHD is associated with child psychopathology, parental stress and impairment in childhood.
