Data from: Education Research: An experiential outpatient teleneurology curriculum for residents

Background: Telemedicine is rapidly becoming a major vehicle of delivering neurologic care to patients who have limited access to subspecialists and exaggerated travel hardship. However, neurology residents receive little-to-no training in telemedicine in outpatient clinics. Methods: We piloted, to our knowledge, the first formalized, experiential outpatient teleneurology curriculum. Neurology residents in their third and fourth postgraduate years (PGY3 and PGY4) at the University of California San Francisco (UCSF) completed an interactive lecture and 4 weeks of teleneurology clinics at the San Francisco Veterans Affairs Medical Center (SFVAMC). Change in residents' telemedicine knowledge and perspectives on the utility, challenges, benefits, and future practice implementation of teleneurology were evaluated in 11 residents using precurriculum and postcurriculum quizzes and surveys after 2 of 4 weeks on the rotation. Results: Residents' performance on quizzes improved from 53% to 88% (p = 0.002). Residents' impression of video visits compared to in-person visits changed, with more individuals indicating video visits to be the "same" if not "somewhat superior" with regards to obtaining a focused history, formulating a focused assessment and plan, communicating recommendations, and the overall care provided (p ≤ 0.04). All residents felt more competent using telemedicine for patient care in their eventual career. Conclusion: Our formal didactic and clinic-based teleneurology curriculum for neurology residents, which shared core themes suggested by the 2017 American Academy of Neurology (AAN) Telemedicine Work Group's published recommendations, showed a statistically significant improvement in knowledge and perspectives about the promise and limitations of teleneurology practice, as well as increased comfort levels in future implementation.

Data from: Mortality risk and social network position in resident killer whales: sex differences and the importance of resource abundance

An individual's ecological environment affects their mortality risk, which in turn has fundamental consequences for life history evolution. In many species social relationships are likely to be an important component of an individual's environment, and therefore their mortality risk. Here we examine the relationship between social position and mortality risk in resident killer whales (Orcinus orca) using over three decades of social and demographic data. We find that the social position of male, but not female, killer whales in their social unit predicts their mortality risk. More socially integrated males have a significantly lower risk of mortality than socially peripheral males, particularly in years of low prey abundance, suggesting that social position mediates access to resources. Male killer whales are larger and require more resources than females, increasing their vulnerability to starvation in years of low salmon abundance. More socially integrated males are likely to have better access to social information and food sharing opportunities which may enhance their survival in years of low salmon. Our results show that observable variation in the social environment is linked to variation in mortality risk, and highlight how sex differences in social effects on survival may be linked to sex differences in life-history evolution.

Data from: Spatial and temporal genetic structure of a river-resident Atlantic salmon (Salmo salar) after millennia of isolation

The river-resident Salmo salar ("småblank") has been isolated from other Atlantic salmon populations for 9,500 years in upper River Namsen, Norway. This is the only European Atlantic salmon population accomplishing its entire life cycle in a river. Hydropower development during the last six decades has introduced movement barriers and changed more than 50% of the river habitat to lentic conditions. Based on microsatellites and SNPs, genetic variation within småblank was only about 50% of that in the anadromous Atlantic salmon within the same river. The genetic differentiation (FST) between småblank and the anadromous population was 0.24. This is similar to the differentiation between anadromous Atlantic salmon in Europe and North America. Microsatellite analyses identified three genetic subpopulations within småblank, each with an effective population size Ne of a few hundred individuals. There was no evidence of reduced heterozygosity and allelic richness in contemporary samples (2005–2008) compared with historical samples (1955–56 and 1978–79). However, there was a reduction in genetic differentiation between sampling localities over time. SNP data supported the differentiation of småblank into subpopulations and revealed downstream asymmetric gene flow between subpopulations. In spite of this, genetic variation was not higher in the lower than in the upper areas. The meta-population structure of småblank probably maintains genetic variation better than one panmictic population would do, as long as gene flow among subpopulations is maintained. Småblank is a unique endemic island population of Atlantic salmon. It is in a precarious situation due to a variety of anthropogenic impacts on its restricted habitat area. Thus, maintaining population size and avoiding further habitat fragmentation are important.
