Data from: Vegetarian diet and incidence of total, ischemic and hemorrhagic stroke in two cohorts in Taiwan

<p><span><b>Objectives <i>–</i></b> To determine how a vegetarian diet affects stroke incidence in two prospective cohorts, and to explore whether the association is modified by dietary vitamin B12 intakes. </span></p> <p><b>Methods</b><i> –</i>Participants without stroke in the Tzu Chi Health study (Cohort1, n=5,050, recruited in 2007-2009) and the Tzu Chi Vegetarian Study (Cohort2, n=8,302, recruited in 2005) were followed until end of 2014. Diet was assessed through food frequency questionnaires in both cohorts at baseline. Stroke events and baseline comorbidities were identified through the National Health Insurance Database. A subgroup of 1528 participants in Cohort1 were assessed for serum homocysteine, vitamin B12, and folate. Associations between vegetarian diet and stroke incidences were estimated by Cox regression with age as time scale, adjusted for sex, education, smoking, alcohol, physical activities, BMI (only in Cohort1), hypertension, diabetes, dyslipidemia, and ischemic heart diseases.</p> <p><b>Results <i>–</i></b> Vegetarians had lower serum vitamin B12 and higher folate and homocysteine than nonvegetarians. In Cohort1, 54 events occurred in 30,797 person-years follow-up. Vegetarians (vs. nonvegetarians) experienced lower risk of ischemic stroke (HR: 0.26, 95% CI: 0.08-0.88). In Cohort2, 121 events occurred in 76,797 person-years follow-up. Vegetarians (vs. nonvegetarians) experienced lower risk of overall stroke (HR: 0.52, 95% CI: 0.33-0.82), ischemic stroke (HR: 0.41, 0.19-0.88), and hemorrhagic stroke (HR: 034, 95%CI: 0.12-1.00). Our explorative analysis showed that vitamin B12 intake may modify the association between vegetarian diet and overall stroke (<i>p</i>-interaction = 0.046).</p> <p><b><i>Conclusion</i></b> –Taiwanese vegetarian diet is associated with a lower risk of ischemic and hemorrhagic strokes.</p>

Metadata from database covering longitudinal data from patients with subarachnoid hemorrhage

<p>Data from patient history, lab values (low-resolution data) and from multiple biosensors (high-resolution data) were prospectively collected from patients with aneurysmal subarachnoid hemorrhage (aSAH) who were admitted to the neurosurgical intensive care unit at the University Hospital of Zurich since October 2016. The cohort covers data from about 300 patients.&nbsp;The data collection was carried out via a CNS data collector (Moberg ICU Solutions, Ambler, PA) and the processing and storage of the high-resolution data was realized on the dedicated information technology infrastructure "ICU Cockpit" [doi.org/10.1093/jamia/ocac064].</p> <p><span>With the present project, highly sensitive patient data in need of protection is collected. Data cannot be made publicly available. However, for specific research questions, data may be made available upon request with the appropriate ethics committee approval and after completion of a DTA.</span></p>

Rare missense functional variants at COL4A1 and COL4A2 in sporadic intracerebral hemorrhage

<p><b>Objective</b><br> To test the genetic contribution of rare missense variants in <i>COL4A1</i> and <i>COL4A2</i> in which common variants are genetically associated with sporadic intracerebral hemorrhage (ICH), we performed rare variant analysis in multiple sequencing data for the risk for sporadic ICH.</p> <p><b>Methods</b><br> We performed sequencing across 559Kbp at 13q34 including <i>COL4A1</i> and <i>COL4A2</i> among 2,133 individuals (1,055 ICH cases; 1,078 controls) in US-based and 1,492 individuals (192 ICH cases; 1,300 controls) from Scotland-based cohorts, followed by sequence annotation, functional impact prediction, genetic association testing, and <i>in silico</i> thermodynamic modeling.</p> <p><b>Results</b><br> We identified 107 rare nonsynonymous variants in sporadic ICH, of which 2 two missense variants, rs138269346 (COL4A1<sup>I110T</sup>) and rs201716258 (COL4A2<sup>H203L</sup>), were predicted to be highly functional and occurred in multiple ICH cases but not in controls from the US-based cohort. The minor allele of rs201716258 was also present in Scottish ICH patients, and rs138269346 was observed in two ICH-free controls with a history of hypertension and myocardial infarction. Rs138269346 was nominally associated with non-lobar ICH risk (P=0.05), but not with lobar ICH (P=0.08), while associations between rs201716258 and ICH subtypes were non-significant (P&gt;0.12). Both variants were considered pathogenic based on minor allele frequency (&lt;0.00035 in EUR), predicted functional impact (<i>deleterious</i> or <i>probably</i> <i>damaging</i>), and <i>in silico</i> modeling studies (substantially altered physical length and thermal stability of collagen).</p> <p><b>Conclusions</b><br> We identified rare missense variants in <i>COL4A1</i>/<i>A2</i> in association with sporadic ICH. Our annotation and simulation studies suggest that these variants are highly functional and may represent targets for translational follow-up.</p>

Weakly-Supervised Learning Significantly Reduces the Number of Labels Required for Intracranial Hemorrhage Detection on Head CT

<p>Modern machine learning pipelines, in particular those based on deep learning (DL) models, require large amounts of labeled data. For classification problems, the most common learning paradigm consists of presenting labeled examples during training, thus providing \emph{strong supervision} by directly presenting examples from the different classes, e.g. positive and negative samples. As a result, the adequate training of these models demands the curation of large datasets with high-quality labels. This constitutes a major obstacle for the development of DL models in radiology---in particular for cross-sectional imaging (e.g., computed tomography [CT] scans)---where labels must come from manual annotations by expert radiologists at the image or slice-level (as opposed to the examination level, such as could be obtained using natural language processing of radiology reports).&nbsp;<br> This work studies the question of what kind of labels&nbsp;should be collected for the problem of intracranial hemorrhage detection in brain CT. We investigate whether image-level annotations should be preferred to examination-level ones. By framing this task as a Multiple Instance Learning (MIL) problem, and employing modern attention-based DL architectures, we analyze the degree to which different levels of supervision improve the detection performance. We find that strong supervision (learning with local image-level annotations) and weak supervision (learning with only global examination-level labels) achieve comparable performance in both examination- and image-level hemorrhage detection, as well as in hemorrhage localization at the pixel-level (explainability). Furthermore, we study this behavior as a function of the number of labels available during training. Our results suggest that local labels may not be necessary at all, drastically reducing the time and cost involved in collecting and curating datasets.</p>
