Patient breast MRI images and computational breast phantom data for research in patient-derived realistic breast modelling
<p>The data is comprised of two parts: 1) patient DICOM MRI images and 2) 3D matrix of a computational breast phantom.</p> <ol> <li>The DICOM images are anonymised patient breast MRI images of a female patient diagnosed with invasive ductal carcinoma. The obtaining of the patients’ DICOM images is approved by the Ethics Committee of Medical University of Varna. The acquisition was performed with GE Signa HDxt MRI scanner. The images are from a T1-weigthed Axial multi-phase VIBRANT (3-phase) sequence and with voxel size of 0.7 mm x 0.7 mm x 0.8 mm. Contrast agent is present. The image set can be opened with any standard DICOM reader.</li> <li>The computational breast phantom is derived from the above mentioned dataset. The phantom is in the form of a 3D matrix saved as a MATLAB data file (.mat file). Each voxel has an assigned Hounsfield Unit value depending on its classification: air = 0, adipose tissue = -152, glandular tissue = 42, tumour = 64, skin = 108. The data file can be opened with MATLAB or Octave.</li> </ol>

<p>Cytomegalovirus (CMV) infection is the most significant viral infection in hosts with compromised immune systems as solid organ transplant patients. Despite significant progress being made in the prevention of CMV disease in these patients, further therapeutic strategies for CMV disease and for the CMV reactivation prevention are needed. Here, we describe the outcome of the infusion of in vitro expanded CMV-reactive T-cells, taken from a healthy CMV-seropositive donor, in a liver-transplanted recipient with a refractory recurrent CMV. In this particular case, adoptive transfer of allogenic CMV-reactive T-lymphocytes resulted in the clearance of CMV infection and resolution of the pathological manifestations of the patient. In the study we also investigated circulating miRNAs, both cellular and viral, as potential biomarkers during the course of CMV infection. The results indicate that the infusion of allogenic CMV-reactive T-cells can be an effective strategy to treat CMV infection recurrence when the generation of autologous virus specific T cell clones is not possible.</p>
Raw Data for the article: Patient-Specific Analysis of Ascending Thoracic Aortic Aneurysm with the Living Heart Human Model
<p>In ascending thoracic aortic aneurysms (ATAAs), aneurysm kinematics are driven by ventricular traction occurring every heartbeat, increasing the stress level of dilated aortic wall. Aortic elongation due to heart motion and aortic length are emerging as potential indicators of adverse events in ATAAs; however, simulation of ATAA that takes into account the cardiac mechanics is technically challenging. The objective of this study was to adapt the realistic Living Heart Human Model (LHHM) to the anatomy and physiology of a patient with ATAA to assess the role of cardiac motion on aortic wall stress distribution. Patient-specific segmentation and material parameter estimation were done using preoperative computed tomography angiography (CTA) and ex vivo biaxial testing of the harvested tissue collected during surgery. The lumped-parameter model of systemic circulation implemented in the LHHM was refined using clinical and echocardiographic data. The results showed that the longitudinal stress was highest in the major curvature of the aneurysm, with specific aortic quadrants having stress levels change from tensile to compressive in a transmural direction. This study revealed the key role of heart motion that stretches the aortic root and increases ATAA wall tension. The ATAA LHHM is a realistic cardiovascular platform where patient-specific information can be easily integrated to assess the aneurysm biomechanics and potentially support the clinical management of patients with ATAAs.</p>

<p>SARS‐CoV‐2 vaccine is considered the primary health strategy able to end the current COVID‐19 pandemic. This viral infection impacts more severely solid organ transplant recipients (SOTRs) than general population, but the effect of vaccination in this subgroup of immunosuppressed patients is not known due to their exclusion from vaccination trials. Preliminary reports suggest a lower antibody production after BNT162b2 Pfizer/BioNTech mRNA‐vaccine,<a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8222937/#ajt16702-bib-0001"> 1 </a>, <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8222937/#ajt16702-bib-0002">2 </a>, <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8222937/#ajt16702-bib-0003">3 </a>, <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8222937/#ajt16702-bib-0004">4 </a>but no data are currently available on the elicited virus‐specific T cell responses.</p>
Raw Data for the article: Apneic Tracheostomy in COVID-19 Patients on Veno-Venous Extracorporeal Membrane Oxygenation
<p>COVID-19 creates an impressive burden for intensive care units in terms of need for advanced respiratory care, with a huge number of acute respiratory distress syndromes (ARDS) requiring prolonged mechanical ventilation. In some cases, this proves to be insufficient, with a refractory respiratory failure calling for an extracorporeal approach (veno-venous ECMO). In this scenario, most of these patients need an early tracheostomy procedure to be carried out, which creates the risk of distribution of aerosol particles, possibly leading to personnel infection. The use of apneic tracheostomy has been proposed for COVID-19 patients, but in case of ECMO it may produce lung derecruitment, severe hypoxemia, and sudden worsening of respiratory mechanics. We developed an apneic tracheostomy technique and applied it in over 32 patients supported by veno-venous ECMO. We present data showing the safety and feasibility of this technique in terms of patient care and personnel protection. Gas exchange and pH did not show statistically significant changes after the tracheostomy, nor did respiratory mechanics data or the need for inspiratory pressure and FiO<sub>2</sub>. The use of apneic tracheostomy was a safe option for patient care during ECMO and reduced the possibility of virus spreading.</p>
Raw Data for the article: Functional Gastrointestinal Disorders in Patients With Epilepsy: Reciprocal Influence and Impact on Seizure Occurrence
<p><strong>Introduction:</strong> The complex relationship between the microbiota-gut-brain axis (MGBA) and epilepsy has been increasingly investigated in preclinical studies. Conversely, evidence from clinical studies is still scarce. In recent years, the pivotal role of MGBA dysregulation in the pathophysiology of functional gastrointestinal disorders (FGID) has been recognized. With this background, we aimed to investigate the prevalence of FGID in patients with epilepsy (PWE) and the possible impact of bowel movement abnormalities on seizure recurrence. <strong>Methods:</strong> A total of 120 PWE and 113 age-, sex-, and BMI-matched healthy subjects (HS) were consecutively enrolled. A questionnaire to evaluate the presence of FGID (according to Rome III diagnostic criteria) was administrated to all participants. In a subgroup of drug-resistant patients, we administered an <em>ad-hoc</em> questionnaire combining Bristol stool charts and seizure diaries to evaluate seizure trends and bowel movement changes. <strong>Results:</strong> A higher prevalence of FGID in PWE (62.5%) than in HS (39.8%) was found (<em>p</em> < 0.001). The most frequently observed disorder was constipation, which was significantly higher in PWE than in HS (43.3 vs. 21.2%, <em>p</em> < 0.001), and was not associated with anti-seizure medication intake according to multivariable analysis. In drug-resistant patients, most seizures occurred during periods of altered bowel movements, especially constipation. A significant weak negative correlation between the number of days with seizures and the number of days with normal bowel movements was observed (<em>p</em> = 0.04). According to multivariable logistic regression analysis, FGID was significantly associated with temporal lobe epilepsy as compared with other lobar localization (<em>p</em> = 0.03). <strong>Conclusions:</strong> Our clinical findings shed new light on the complex relationship between epilepsy and the MGBA, suggesting a bidirectional link between bowel movement abnormalities and seizure occurrence. However, larger studies are required to better address this important topic.</p>
Raw Data for the article: Usefulness of longitudinal systolic strain and delayed enhancement cardiac magnetic resonance in depicting risk of supraventricular arrythmias in patients with acute myocarditis and preserved left ventricular function
<p><strong>Background: </strong>Myocarditis have variable clinical presentation, evolution and prognosis. Aim of our study was to evaluate the value of speckle tracking echocardiography and cardiac magnetic resonance (CMR) in the short-term prediction of supraventricular arrhythmias (SVA) in patients with acute myocarditis.</p> <p><strong>Methods: </strong>Seventy patients (mean age 31±14 years old) with acute myocarditis and preserved left ventricular ejection fraction (LVEF) were enrolled. Longitudinal systolic strain (LS) of the left ventricle (LV), mechanical dispersion (MD) and CMR with quantitative measurement of delayed enhancement (DE) were performed in a subset of 43 patients. Logistic regression and ROC analysis were used to identify predictors of SVA RESULTS: Only LS measured at sup-epicardial, mid-wall and sub-endocardial level of the apical 4-chamber view was significantly lower in patients with SVA, while MD was marginally prolonged in this setting. A value of LS > - 16.1% measured at LV mid-wall in the apical 4-chamber view (ROC-AUC .75, Sensitivity 63%, Specificity 85%) was the most accurate measure to identify patients with SVA. DE mass was also helpful with a ROC-AUC .76; a DE-Mass > 18.9 gr. had a Sensitivity 63% and a Specificity 77%, to identify patients at risk of SVA.</p> <p><strong>Conclusions: </strong>Both DE mass and LS were associated with higher risk of SVA in patients with acute myocarditis and preserved LVEF. However, LS measured at the mid-wall level and limited to LV segments included in the apical 4-chamber view was the most accurate measure and did not show interaction with DE mass.</p>

Impact of a pulmonary rehabilitation program on social disadvantage and physical activity data of postCOVID19 patients: A North-African pilot study
<p><strong>WHAT IS ALREADY KNOWN</strong></p> <ul> <li>In post-acute COVID19, many patients still manifest symptoms for weeks and months.</li> <li>Psychological manifestations are prevalent and persisting in recovered COVID19 patients.</li> <li>Indeed, COVID19 was reported to possibly lead to symptoms of depression and anxiety, and to alter health-related quality of life (HRQoL).</li> <li>Physical activity has been proven to be effective in improving both mental and physical health.</li> <li>Thus, COVID19 patients could benefit from pulmonary rehabilitation program (PRP).</li> <li>Many studies related the effects of a standard PRP on physical activity and HRQoL in post COVID19 patients. However, to the best of the authors’ knowledge, no study evaluated the physical activity and social disadvantage of patients who were hospitalized for COVID19 in a low income country.</li> <li>This paper is the second part of a cross sectional study that aimed to evaluate the impact of a PRP on social disadvantage (physical activity, hospital anxiety and depression (HAD), HRQoL) data of Tunisian patients with postCOVID19.</li> </ul> <p><strong>WHAT THIS PAPER ADDS</strong></p> <ul> <li>The present North African study demonstrated that a PRP improved HRQoL, HAD, and physical activity data of Tunisian COVID19 patients.</li> <li>For instance, the means of the functional, psychological and relation dimensions, and the VQ11 total score (ie; main outcome) decreased significantly by 1.79, 2.00, 1.57, and 5.36 points. In addition, the means of the HAD anxiety and depression scores decreased significantly by 2.07, and 2.57 points, respectively, and the means of the physical activity and total scores improved significantly by 1.75 and 1.78 points, respectively.</li> <li>To the best of the authors’ knowledge, this is the first African and Arab study investigating the impact of a PRP on postcovid19 patients in terms of social disadvantages and physical activity.</li> <li> </li> </ul> <ul> <li>This study confirms that an outpatient PRP program improves physical activity, psychological status and HRQoL in the management of postCOVID19 patients.</li> </ul>
Patient-reported outcomes via electronic health record portal vs. telephone: process and retention data in a pilot trial of anxiety or depression symptoms in epilepsy
<p>Objective: To close gaps between research and clinical practice, tools are needed for efficient pragmatic trial recruitment and patient-reported outcome(PROM) collection. The objective was to assess feasibility and process measures for patient-reported outcome collection in a randomized trial comparing electronic health record(EHR) patient portal questionnaires to telephone interview among adults with epilepsy and anxiety or depression symptoms.</p> <p>Results: Participants were 60% women, 77% White/non-Hispanic, with mean age 42.5 years. Among 15 individuals randomized to EHR portal, 10(67%, CI 41.7-84.8%) met the 6-month retention endpoint, versus 100%(CI 79.6-100%) in the telephone group(p=0.04). EHR outcome collection at 6 months required 11.8 minutes less research staff time per participant than telephone (5.9, CI 3.3-7.7 vs. 17.7, CI 14.1-20.2). Subsequent telephone contact after unsuccessful EHR attempts enabled near complete data collection and still saved staff time.</p> <p>Discussion: Data from this randomized pilot study of pragmatic outcome collection methods for patients with anxiety or depression symptoms in epilepsy includes baseline participant characteristics, recruitment flow resulting from a novel EHR-based, care-embedded recruitment process, and data on retention along with various process measures at 6-months.</p>
