Time trends and associated factors of axial length in 4 and 5 year-olds children in Shanghai from 2013 to 2019

Purpose: To evaluate time trends of axial length (AL) in 4 and 5 year-olds children, and to assess factors associated with AL in Shanghai from 2013 to 2019.

Methods: This was a 7-year observational study of children of 4 and 5 year-olds in Shanghai. AL, horizontal and vertical corneal curvature, spherical equivalent (SE), body height and weight were measured. Furthermore, a questionnaire was collected including time outdoors and bad habits of using eyes.

Results: No significant differences were found in AL (P = 0.526) in 4 year-olds children, whereas significant differences were observed in SE (P = 0.001), horizontal corneal curvature (P = 0.006), vertical corneal curvature (P = 0.004), height (P < 0.001) and weight (P = 0.022) from 2013 to 2019. No significant differences were found in AL (P = 0.304), SE (P = 0.200) and weight (P = 0.292) in 5 year-olds children, whereas significant differences were observed in horizontal corneal curvature (P = 0.040), vertical corneal curvature (P = 0.015), and height (P < 0.001) from 2013 to 2019. Multivariate analyses revealed that AL was associated significantly with boys (P < 0.001) and time outdoors (P < 0.001) in all the 4 year-olds children, with boys (P < 0.001) and time outdoors (P = 0.014) in all the 5 year-olds children.

Conclusions: ALs of 4 and 5 year-olds children remain relatively stable in Shanghai from 2013 to 2019. Longitudinal studies are needed to confirm the relationship between AL elongation and environmental risk factors.

The prevalence of myopia and high myopia was lower in 2018 than in 2014 (P < 0.001 and P = 0.013, respectively). Mean AL increased with age from 7 year-oldsto18 year-oldsin 2014and 2018 (both P < 0.001), respectively. Mean SE decreased with age in 2014 and 2018 (both P < 0.001), respectively. The prevalence of myopia and high myopia increased with age in 2014 and 2018 (all P < 0.001), respectively. Multivariate regression analysis revealed that longer AL was mainly associated male gender and body height.

Conclusion This study shows normative growth values for AL and SE in Shanghai children at the age of 7 to 18 year-olds, as well as the age-specific prevalence of myopia and high myopia.

Data from: Extent and cost of inappropriate use of tumour markers in patients with pulmonary disease: a multicentre retrospective study in Shanghai, China
Objectives: The currently implemented healthcare reform in China requires substantial capital investment. Although overtreatment results in serious waste, inappropriate laboratory use is widespread, and overuse of tumor markers (TMs) has attracted increasing attention. Design: Retrospective study. Setting: The respiratory, thoracic surgery, and oncology departments of three hospitals in Shanghai, from 2014 to 2015. Participants: Patients with chronic obstructive pulmonary disease (COPD) and primary bronchogenic lung cancer (PLC). Based on clinical guidelines and physician experience, the criteria of suitability of TM examinations were determined, and the number, cost, and proportion of inappropriate TM requests were analyzed. Results: The area under the receiver operating characteristic (ROC) curve for CEA+CYFRA211+SCC+NSE in COPD and PLC patients was 0.813, in accordance with the cost-effectiveness principle, indicating good clinical and health economics values. In the 2706 patients, 12,496–16,956 (58.27–79.06%) of TM requests were inappropriate. Furthermore, the involved expense was 650,200–1,014,156 yuan, accounting for 7.69–12.00% of examination expenses, and 1.35–2.11% of hospitalization costs. Conclusions: We found that the inappropriate use of TMs was widespread for patients with pulmonary disease. Clinicians should use TMs strictly according to the guidelines to effectively manage laboratory resources and control costs.
