Data from: Validation of serum neurofilaments as prognostic & potential pharmacodynamic biomarkers for ALS

<p><span><span><span><span><span><span><span><span><span><span><span><u>Objective</u>. Identify preferred neurofilament assays, and clinically validate serum NfL and pNfH as prognostic and potential pharmacodynamic biomarkers relevant to ALS therapy development. </span></span></span></span></span></span></span></span></span></span></span></p> <p><span><span><span><span><span><span><span><span><span><span><span><u>Methods</u>. Prospective, multi-center, longitudinal observational study of patients with ALS (n=229), primary lateral sclerosis (PLS, n=20) and progressive muscular atrophy (PMA, n=11). Biological specimens were collected, processed and stored according to strict standard operating procedures (SOPs) <sup>1</sup>. Neurofilament assays were performed in a blinded manner by independent contract research organizations (CROs). </span></span></span></span></span></span></span></span></span></span></span></p> <p><span><span><span><span><span><span><span><span><span><span><span><u>Results</u>. For serum NfL and pNfH measured using the Simoa assay, there were no missing data (i.e. both technical replicates below the lower limit of detection were not encountered). For the Iron Horse and Euroimmun pNfH assays, such missingness was encountered in ~4% and ~10% of serum samples respectively. Mean coefficients of variation (CVs) for pNfH in serum and CSF were ~4-5% and ~2-3% respectively in all assays. Baseline NfL concentration, but not pNfH, predicted the future ALSFRS-R slope and survival. Incorporation of baseline serum NfL into mixed effects models of ALSFRS-R slopes yields an estimated sample size saving of ~8%. Depending on the method used to estimate effect size, use of serum NfL (and perhaps pNfH) as pharmacodynamic biomarkers, instead of the ALSFRS-R slope, yields significantly larger sample size savings.</span></span></span></span></span></span></span></span></span></span></span></p> <p><u>Conclusions</u><span><span><span><span><span><span><span><span><span><span><span>. Serum NfL may be considered a clinically validated prognostic biomarker for ALS. Serum NfL (and perhaps pNfH), quantified using the Simoa assay, have potential utility as pharmacodynamic biomarkers of treatment effect. </span></span></span></span></span></span></span></span></span></span></span></p>

Dataset - PONE-D-20-09507 - Usefulness of circulating miR-146a and miR-16-5p microRNAs as prognostic biomarkers in community-acquired pneumonia

<p>This dataset shows a prospective observational study performed in a cohort of 153 patients admitted to hospital with CAP.</p> <p>Clinical and analytical variables were collected, and the main outcome variable was 30-day mortality.</p> <p>Small RNA was purified from patients&acute; plasma samples by column-based protocol, and retrotranscribed to cDNA (Exiqon&#39;s miRCURY &trade; series kits), adding synthetic RNA controls (spike-in). The quality of the process was evaluated (QC control) and only 117 samples passed the test.</p> <p>FIRST STEP: Eight samples paired by age and gender were selected (4 patients who had suffered a cardiovascular event or death during follow-up and 4 who had not) and a panel of 752 human miRNAs was tested (miRCURY LNA &trade; Universal - Ready-to-Use Human Panel , Exiqon), in order to determine a preliminary pattern of differential miRNA expression between patients with different CAP evolution.</p> <p>SECOND STEP: According to the preliminary data obtained, 25 candidate miRNAs were selected: 5 intended to be used as normalizers, 5 selected by statistical criteria (univariate association with mortality) and 15 selected from an exhaustive bibliographic search on miRNAs, sepsis, inflammation and / or cardiovascular disease, prioritizing those that appeared in a greater number of publications and those related to respiratory diseases. RT-PCR was carried out by hybridization with double-stranded flurochrome (ExiLENT SYBR&reg; Green Master Mix) using the C1000 Touch CFX384 thermocycler (Bio-Rad).</p> <p>The relative amount of each miRNA was calculated with ∆Ct = CtmiRNA - CtUniSp2, and it was later normalized using the GeNorm algorithm. The final data was calculated with the formula 2<sup>-∆Ct</sup> and the values were expressed as the fold change (FC) of each miRNA with respect to UniSP2<em>.</em></p>

Critical PO2 as a diagnostic biomarker for the effects of low-oxygen modified and controlled atmospheres on phytosanitary irradiation treatments in the Cabbage Looper Trichoplusia ni (Hübner)

<p>BACKGROUND: Phytosanitary irradiation is a sustainable alternative to chemical fumigants for disinfesting fresh commodities from insect pests. However, irradiating insects in modified atmospheres with very low oxygen (&lt;1 kPa O2) has repeatedly been shown to increase radioprotective response. Thus, there is a concern that modified atmosphere packaging could reduce the efficacy of phytosanitary irradiation. One hurdle slowing the widespread application of phytosanitary irradiation is a lack of knowledge about how moderate levels of hypoxia relevant to modified atmosphere packaging of most fresh commodities (3-10 kPa O2) may affect phytosanitary irradiation treatments. Therefore, we hypothesize that critical PO2 (Pcrit), the level of oxygen at which an insect's metabolism becomes impaired, can be used as a diagnostic biomarker to predict the induction of a radioprotective response. RESULTS: Using the cabbage looper Trichoplusia ni (Hübner), we show that there is a substantial increase in radiation resistance when larvae are irradiated in atmospheres more hypoxic than their Pcrit (3.3 kPa O2). These data are consistent with our hypothesis that Pcrit could be used as a diagnostic biomarker for what levels of hypoxia may induce radioprotective effects that could impact phytosanitary irradiation treatments. CONCLUSION: We propose that the relationship between Pcrit and radioprotective effects could allow us to build a framework for predicting the effects of low-oxygen atmospheres on the efficacy of phytosanitary irradiation. However, more widespread studies across pest species are still needed to test the generality of this idea.</p>

Sepsis biomarker study 2011-2012 Sweden

<p>Data collected during the prospective "Sepsis Skaraborg study" performed 2011-2012 in the western region of Sweden. Adult patients admitted to the emergency department for suspicion of a community-onset sepsis were asked to participate. Only those patients who gave their written informed consent were enrolled. The study was approved by the Regional Ethical Review Board of Gothenburg (376-11). The file includes data for patient characteristics, biomarker measurements, and patient classifications using Sepsis-2 and Sepsis-3 criteria.</p>

Identification of immune-related candidate biomarkers in plasma of patients with sporadic vestibular schwannoma

<p>Vestibular schwannoma (VS) is an intracranial tumor arising from neoplastic Schwann cells, and typically presenting with hearing loss. The traditional belief that hearing deficit is caused by physical expansion of the VS, compressing the auditory nerve, does not explain the common clinical finding that patients with small tumors can have profound hearing loss, suggesting that tumor-secreted factors could influence hearing ability in VS patients. We conducted profiling of patients' plasma for 66 immune-related factors in patients with sporadic VS (N&gt;170) and identified and validated candidate biomarkers associated with tumor size (S100B) and hearing (MCP-3). We further identified a 9-biomarker panel (TNR-R2, MIF, CD30, MCP-3, IL-2R, BLC, TWEAK, eotaxin, S100B) with outstanding discriminatory ability for VS. These findings revealed possible therapeutic targets for VS, providing a unique diagnostic tool that may predict hearing change and tumor growth in VS patients, and may inform the timing of tumor resection to preserve hearing.</p>

Host biomarkers and combinatorial scores for the detection of serious and invasive bacterial infection in pediatric patients with fever without source

<h3><span>Background </span></h3> <p><span>Improved tools are required to detect bacterial infection in children with fever without source (FWS), especially when younger than 3 years old. The aim of the present study was to investigate the diagnostic accuracy of a host signature combining for the first time two viral-induced biomarkers, tumor necrosis factor-related apoptosis-inducing ligand (TRAIL) and interferon </span><span>γ</span><span>-induced protein-10 (IP-10), with a bacterial-induced one, C-reactive protein (CRP), to reliably predict bacterial infection in children with fever without source (FWS) and to compare its performance to routine individual biomarkers (CRP, procalcitonin (PCT), white blood cell and absolute neutrophil counts, TRAIL, and IP-10) and to the Labscore.</span></p> <h3><span>Methods</span></h3> <p><span>This was a prospective diagnostic accuracy study conducted in a single tertiary center in children aged less than 3 years old presenting with FWS. Reference standard etiology (bacterial or viral) was assigned by a panel of three independent experts. Diagnostic accuracy (AUC, sensitivity, specificity) of host individual biomarkers and combinatorial scores was evaluated in comparison to reference standard outcomes (expert panel adjudication and microbiological diagnosis). </span></p> <h3><span>Results </span></h3> <p><span>241 patients were included. 68 of them (28%) were diagnosed with a bacterial infection and 5 (2%) with invasive bacterial infection (IBI). Labscore, ImmunoXpert, and CRP attained the highest AUC values for the detection of bacterial infection, respectively 0.854 (0.804–0.905), 0.827 (0.764–0.890), and 0.807 (0.744–0.869). Labscore and ImmunoXpert outperformed the other single biomarkers with higher sensitivity and/or specificity and showed comparable performance to one another although slightly reduced sensitivity in children &lt; 90 days of age.</span></p> <h3><span>Conclusion </span></h3> <p><span>Labscore and ImmunoXpert demonstrate high diagnostic accuracy for safely discriminating bacterial infection in children with FWS aged under and over 90 days, </span><span>supporting their adoption in the assessment of febrile patients. </span></p>

Identifying biomarkers for chronic obstructive pulmonary disease in the salivary metabolome.

<p><strong><span>Background:</span></strong><span> The lack of specificity in spirometry and accuracy in &lsquo;pre-disease states&rsquo;, and elderly indicates the need for other accurate, inexpensive and non-invasive tests for diagnosis, triage severity and particularly for choosing and monitoring response to treatments in chronic obstructive pulmonary disease (COPD). We compared salivary metabolomic signatures from patients with COPD and controls across a range of severity of airflow obstruction. </span></p> <p><a name="_61k4y1plsv5"></a><strong><span>Methods</span></strong></p> <p><span>45 people with COPD (mean age 66.1 &plusmn; 23 years, forced expiratory volume [FEV<sub>1</sub>] 47<u>+</u>15% predicted) and 48 healthy controls (mean age 56.0 <u>+</u>16.2 years, FEV<sub>1</sub> 90<u>+</u>23 % predicted) provided saliva that was assessed by flow infusion electrospray mass spectrometry (FIE-MS). Spectra were interrogated using an online library package. Data (patient data and metabolomic outputs) are available from this site.&nbsp;</span></p> <p><a name="_j99pcjszp0an"></a><strong><span>Results</span></strong></p> <p><span>Four potential biomarkers identified the presence of COPD with a sensitivity of 73%, specificity of 72%.<span>&nbsp; </span>Six metabolites predicted FEV1 % in the COPD cohort (<em>P </em>&lt; 0.001, R<sup>2</sup> &gt; 0.3, AUC &gt; 0.7) whilst a range of multivariate approaches targeted six metabolites linked to GOLD stage (<em>P </em>&lt; 0.001, AUC &gt; 0.7). Identification of the metabolites suggested changes in pterin biosynthesis, lipid processing, nucleotide metabolism and melatonin in COPD patients. </span></p> <p><a name="_2n4ucvxs4l1n"></a><strong><span>Conclusions</span></strong></p> <p><span>Metabolic fingerprinting of saliva samples could differentiate COPD from age matched controls and inform COPD severity of airflow obstruction.
