Data from: Incidence of frontotemporal lobar degeneration in Italy: the Salento-Brescia Registry study

Objective The goal of the present work, based on a collaborative research registry in Italy (the Salento-Brescia Registry), was to assess the incidence of frontotemporal lobar degeneration (FTLD) and to define the frequencies of different FTLD phenotypes in the general population. Methods The study was conducted from January 1, 2017, to December 31, 2017, in 2 Italian provinces: Lecce (in Puglia) in the south (area 2,799.07 km2, inhabitants 802,082) and Brescia (in Lombardy) in the north (area 4,785.62 km2, inhabitants 1,262,678). During the study period, all new cases of FTLD (incident FTLD) were counted, and all patients' records were reviewed. The incidence was standardized to the Italian general population in 2017. Results In the 2 provinces, 63 patients with FTLD were diagnosed. The incidence rate for FTLD was 3.05 (95% confidence interval [CI] 2.34–3.90) per 100,000 person-years (py), while the age-sex standardized incidence rate was 3.09 (95% CI 2.95–3.23) per 100,000 py. In the Italian population, the lifetime risk was 1:400. There was a progressive increase in FTLD incidence across age groups, reaching its peak in the 75- to 79-year-old group, with an incidence rate of 15.97 (95% CI 8.94–26.33) per 100,000 py. The behavioral variant of frontotemporal dementia was the most common phenotype (37%). No difference in crude incidence rate between the 2 provinces was observed. Conclusion FTLD is a more common form of dementia than previously recognized, with a risk spanning in a wide age range and with maximum incidence in the mid-70s. Improved knowledge of FTLD epidemiology will help to provide appropriate public health service policies.

Data from: Axial length elongation and myopia incidence increase in primary school-age children: 3-year follow-up study

<p>Objective: To investigate the axial length (AL) elongation in primary school-age children during 3-year follow-up period, and evaluate the associations of AL elongation with spherical equivalent (SE), AL at baseline, body height and weight.</p> <p>Design: A 3-year observational cohort study from 2014 to 2017.</p> <p>Setting: Jinshan Hospital of Fudan University in Shanghai.</p> <p>Methods: A total of 452 children successfully completed their measurements in the 3-year follow-up period. Among those children, the mean age was 6.9 ± 0.7 years, ranging from 6 years to 8 years, and 217 (42.7%) were boys. AL was measured with an ocular biometry system. Refractive error was measured using an auto-refractor without cycloplegia.</p> <p>Results: The mean changes of ALs were 0.27 ± 0.28 mm, 0.52 ± 0.40 mm, and 0.89 ± 0.51mm over 1 year, over 2 years and over 3 years, respectively. The mean changes of Spherical equivalents (SEs) were -0.27 ± 0.80 D, -0.56 ± 1.00 D, and -0.95 ± 1.41 D over 1 year, over 2 years and over 3 years, respectively. Multivariate linear regression analysis revealed that mean change of AL was associated with mean change of SE at all points (all P &lt; 0.001). In addition, linear regression analysis revealed that AL elongation in the 3-year follow-up period was associated with AL at baseline (R2 = 0.009, P = 0.045).</p> <p>Conclusions: AL elongation is relatively high in the primary school-age children in Jinshan District, Shanghai. Effect strategies are needed to control AL elongation.</p> <p> </p>

Data from: Incidence rate of community-acquired pneumonia in adults: a population-based prospective active surveillance study in three cities in South America

OBJECTIVE: To determine the incidence rate and mortality of community-acquired pneumonia (CAP) in adults in three cities in Latin America during a three-year period. DESIGN: Prospective population based study. SETTING: Healthcare facilities (outpatient centers and hospitals) in the cities of General Roca (Argentina), Rivera (Uruguay) and Concepcion (Paraguay). PARTICIPANTS: 2302 adults aged 18 years and older with CAP were prospectively enrolled between January 2012 and March 2015. MAIN OUTCOME MEASURES: Incidence rate of CAP in adults, predisposing conditions for disease, mortality at 14 days and at one year were estimated. Incidence rate of CAP, within each age group, was calculated by dividing the number of cases by the person-years of disease-free exposure time based on the last census; incidence rates were expressed per 1000 person-years. RESULTS: Median age of participants was 66 years, 46.44 % were men, 68% were hospitalized. Annual incidence rate was 7.03 (CI95% 6.64-7.44) per 1000 person-years in General Roca, 6.33 (CI95% 5.92-6.78) per 1000 person-years in Rivera and 1.76 (CI95% 1.55-2.00) per 1000 person-years in Concepcion. Incidence rates were highest in participants aged over 65 years. 82.4% had at least one predisposing condition and 48% had 2 or more (multi-morbidity). Chronic heart disease (43.6%) and smoking (37.3%) were the most common risk factors. 14-day mortality rate was 12.1% and one-year mortality was 24.9%. Multi-morbidity was associated with an increased risk of death at 14 days (OR 2.91; 2.23 to 3.80) and at 1 year (OR 3.00; 2.44 to 3.70). CONCLUSIONS: We found a high incidence rate of CAP in adults, ranging from 1.76 to 7.03 per 1000 person-years, in three cities in South America, disclosing the high burden of disease in the region. Efforts to improve prevention strategies are needed.

Data from: Incidence and risk factors of first-line antiretroviral treatment failure among human immunodeficiency virus-infected children in Amhara regional state, Ethiopia: a retrospective follow-up study

Objective: This study aimed to assess the incidence and risk factors of treatment failure among Human Immunodeficiency Virus (HIV)/Acquired Immunodeficiency Syndrome infected (AIDS) children who were on Antiretroviral Therapy (ART) in Amhara National Regional State, Ethiopia. Methods: A retrospective follow-up study was conducted from January 2010 to March 2016. A total of 824 children under the age of 15 years who had started ART were included in the study. Data were collected from children's medical chart and ART registration logbook using a standard checklist. A Weibull regression model was used to identify the risk factors for treatment failure. Adjusted Hazard Ratios with 95% confidence intervals were used to declare statistical significance. Results: The mean (±SD) age of the children was 6.4 ± 3.6 years with a median (IQR) follow up of 30.5 (IQR: 14.6, 51.4) months. Sixty-three children (7.7%, 95%CI:5.8, 9.5) developed treatment failure. Seventeen (27.0%) of whom were immunological and 46 (73.0 %) clinical failures. The incidence rate of treatment failure was 22.1/10000 person-months. The cumulative probability of failure was 0.4 with 28562.5 person-month observations. Lack of disclosure [AHR=4. 4, 95%CI (1.8, 11.3)], opportunistic infections during initiation of ART [AHR= 2.3, 95% CI (1.3, 4.1)] and prolonged follow up [AHR = 0.06, 95% CI (0.02, 0.18)] were the main predictors of treatment failure. Conclusion: This study revealed that the incidence of treatment failure remains a significant public health concern in Ethiopia. Undisclosed HIV status to children, the presence of opportunistic infections during initiation of ART and prolonged follow up was found to be the main predictors of treatment failure. Hence, early detection of the treatment failures and further studies on viral monitor need to consider.
