Objective To study survival and characterize long-term functional impairments as well as health-related quality of life (HRQOL) of Lambert-Eaton myasthenic syndrome (LEMS) patients. Methods In this observational study, survival of LEMS patients, separately for non-tumor (NT) and small-cell lung cancer (SCLC), was compared to the Dutch general population and to patients with SCLC. Disease course in LEMS patients was recorded retrospectively. Several scales for functional impairments and health-related quality of life were assessed. Results We included 150 LEMS patients. Survival was similar to the general population in 65 NT-LEMS patients. Tumor survival was significantly longer in 81 SCLC-LEMS patients compared to non-LEMS SCLC patients (overall median survival 17 vs. 7.0 months, p<0.0001). At diagnosis, 39 patients (62%) of 63 patients with complete follow-up data were independent for ADL activities, improving to 85% at 1-year follow-up. Physical HRQOL composite score (55.9) was significantly lower than in the general population (76.3, p<0.0001) and comparable to myasthenia gravis (60.5) Mental HRQOL composite score was 71.8 in LEMS patients, comparable to the general population (77.9, p=0.19) and myasthenia gravis (70.3). Conclusions This study shows NT-LEMS patients have normal survival. SCLC-LEMS patients have an improved tumor survival, even after correcting for tumor stage. A majority of LEMS patients report a stable disease course and remain or become independent for self-care after treatment.
Data from: Endocrine and metabolic disorders in survivors of childhood cancers and health-related quality of life and physical activity
Context: Childhood cancer survivors experience chronic health conditions that impact health related quality of life (HRQOL) and participation in optimal physical activity. Objective: The study aimed to determine independent effects of endocrine and metabolic disorders on HRQOL and physical activity. Design, Setting, and Patients: Retrospective cohort with longitudinal follow-up of survivors of childhood cancer enrolled in the North American Childhood Cancer Survivor Study. Main Outcome Measures: Medical Outcomes Short Form-36 estimated HRQOL while participation in physical activity was dichotomized as meeting or not meeting recommendations from the Center for Disease Control and Prevention. Log binomial regression evaluated the association of each endocrine/metabolic disorder with HRQOL scales and physical activity. Results: Of 7,287 survivors, median age 32 years (18-54) at their last follow-up survey, 4,884 (67%) reported one or more endocrine/metabolic disorders. Survivors with either disorder were significantly more likely to be male, older, received radiation treatment, and experience other chronic health conditions. After controlling for covariates, survivors with any endocrine/metabolic disorder were more likely to report poor physical function risk ratio ([RR] 1.25; 95% confidence interval [CI] 1.05-1.48), increased bodily pain (RR 1.27; CI 1.12-1.44), poor general health (RR 1.49; CI 1.32-1.68) and lower vitality (RR 1.21; CI 1.09-1.34) compared to survivors without. The likelihood of meeting recommended physical activity was lower among survivors with growth disorders (RR 0.90; CI=0.83-0.97), osteoporosis (RR 0.87; CI=0.76-0.99), and overweight/obesity (RR 0.92; CI 0.88-0.96). Conclusion: Endocrine and metabolic disorders are independently associated with poor HRQOL and sub-optimal physical activity among childhood cancer survivors.
Data from: Early life nutritional quality effects on adult memory retention in a parasitic wasp
Nutritional quality during early life can affect learning ability and memory retention of animals. Here we studied the effect of resource quality gained during larval development on the learning ability and memory retention of 2 sympatric strains of similar genetic background of the parasitoid Trichogramma brassicae: one uninfected and one infected by Wolbachia. Wolbachia is a common arthropod parasite/mutualistic symbiont with a range of known effects on host fitness. Here we studied, for the first time, the interaction between resource quality and Wolbachia infection on memory retention and resource acquisition. Memory retention of uninfected wasps was significantly longer when reared on high quality hosts when compared to low quality hosts. Furthermore, uninfected wasps emerging from high quality hosts showed higher values of protein and triglyceride than those emerging from low quality hosts. In contrast, the memory retention for infected wasps was the same irrespective of host quality, although retention was significantly lower than uninfected wasps. No significant effect of host quality on capital resource amount of infected wasps was observed, and infected wasps displayed a lower amount of protein and triglyceride than uninfected wasps when reared on high quality hosts. This study suggests that the nutritional quality of the embryonic period can affect memory retention of adult wasps not infected by Wolbachia. However, by manipulating the host's obtained capital resource amount, Wolbachia could enable exploitation of the maximum available resources from a range of hosts to acquire suitable performance in complex environments.
Data from: Change in quality of life for patients with Irritable Bowel Syndrome following referral to a gastroenterologist: a cohort study
Background: Irritable bowel syndrome (IBS), a chronic functional condition, considerably reduces quality of life (QoL) and referral to gastroenterology is common. Until now, however, the impact of seeing a gastroenterologist for IBS on patients' QoL and utility has not been assessed. Methods: Patients referred with "probable IBS" to the Nottingham Treatment Centre between October 2012 and March 2014 were invited to complete a QoL questionnaire (EuroQol–5 Dimension) before their first appointment. Patients with confirmed IBS who completed this baseline assessment were sent follow-up questionnaires three and twelve months later. Global QoL and utility were measured at each time point and change from baseline calculated. Paired t-tests analysed the significance of any change. Results: Of 205 invited patients, 69 were eligible and recruited. Response at three and twelve months was 45% and 17% respectively. Median global QoL at baseline was 67.5 (Interquartile range [IQR] 50.0 to 80.0), with a mean increase of 3.25 (95% confidence interval [CI] -5.38 to 11.88) three months later and a mean decrease of -1.82 (95% CI -16.01 to 12.38) after one year. The median utility at baseline was 0.76 (IQR 0.69 to 0.80), with a mean increase of 0.06 (95%CI -0.01 to 0.14) at three months and no change, 0.00 (-0.16 to 0.16), after one year. Conclusion: Patients experienced a small but not statistically significant increase in QoL and utility three months after seeing a gastroenterologist for IBS, which was not maintained. Gastroenterology referral does not appear to appreciably improve Qol for most people with IBS.
Data from: Assessing the relationship between quality of life and behavioral activation using the Japanese behavioral activation for depression scale-short form
Quality of life (QOL) is an important health-related concept. Identifying factors that affect QOL can help develop and improve health-promotion interventions. Previous studies suggest that behavioral activation fosters subjective QOL, including well-being. However, the mechanism by which behavioral activation improves QOL is not clear. Considering that QOL improves when depressive symptoms improve post-treatment and that behavioral activation is an effective treatment for depression, it is possible that behavioral activation affects QOL indirectly rather than directly. To clarify the mechanism of the influence of behavioral activation on QOL, it is necessary to examine the relationships between factors related to behavioral activation, depressive symptoms, and QOL. Therefore, we attempted to examine the relationship between these factors. Participants comprised 221 Japanese undergraduate students who completed questionnaires on behavioral activation, QOL, and depressive symptoms: the Japanese versions of the Behavioral Activation for Depression Scale-Short Form (BADS-SF), WHO Quality of Life-BREF (WHOQOL-26), and Center for Epidemiologic Studies Depression Scale (CES-D). The BADS-SF comprises two subscales, Activation and Avoidance, and the WHOQOL-26 measures overall QOL and four domains, Physical Health, Psychological Health, Social Relationships, and Environment. Mediation analyses were conducted with BADS-SF activation and avoidance as independent variables, CES-D as a mediator variable, and each WHO-QOL as an outcome variable. Results indicated that depression completely mediated the relationship between Avoidance and QOL, and partially mediated the relationship between Activation and QOL. In addition, analyses of each domain of QOL showed that Activation positively affected all aspects of QOL directly and indirectly, but Avoidance had a negative influence on only part of QOL mainly through depression. The present study provides behavioral activation strategies aimed at QOL enhancement.
