Moving academic conferences online: aids and barriers to delegate participation

<p>In-person academic conferences are important to disseminate research and provide networking opportunities. Whether academics attend in-person conferences is based on the cost, accessibility, and safety of the event. Therefore, in-person conferences are less accessible to academics and stakeholders that are unable to overcome some of these factors, which then act as a barrier to equal and inclusive participation. Additionally, the carbon footprint of conference travel is increasingly becoming a factor in deciding on whether to attend a conference. Online conferences may provide opportunities to mitigate these challenges. Here, we illustrate how a learned society can move their conference online. Then comparing data acquired from the virtual conference and previous in-person conferences we explore the in the aids and barriers influencing the decision of delegates to attend the meetings. Ultimately, moving meetings online aids delegate participation by removing concerns about travel, cost, and carbon emissions, but there remains a barrier to participation with online meeting being perceived as less effective for networking and social opportunities.</p>

History taking online: Opportunities and limitations in comparison to traditional bedside teaching

<p><span><span><span><span><span><span><span><span><span><span><span><b>Objective</b></span></span></span></span></span></span></span></span></span></span></span></p> <p><span><span><span><span><span><span><span><span><span><span><span>Obtaining a systematic medical history (MH) from a patient is a core competency in medical education and plays a vital role in the diagnosis of diseases. At the Faculty of Medicine at LMU Munich, students have their first course in MH taking during their second year. Due to the COVID-19 pandemic, the traditional bedside MH taking course had to be transformed into an online course (OC). Our objectives were to implement an online MH taking course, to evaluate its feasibility and to compare the evaluation results to a historic cohort that had undertaken the traditional bedside teaching course (BTC).</span></span></span></span></span></span></span></span></span></span></span></p> <p><span><span><span><span><span><span><span><span><span><span><span><b>Methods</b></span></span></span></span></span></span></span></span></span></span></span></p> <p><span><span><span><span><span><span><span><span><span><span><span>874 second-year students participated in the OC. After teaching the theoretical background via asynchronous online lectures, students participated in a practical exercise with fellow students using the video communication platform Zoom where they were able to practice taking a MH on the basis of fictitious, text-based patient cases. Students were then asked to evaluate the course through a standardized online survey with 31 questions on teaching quality and self-perceived learning success, which had also been used in previous years. The survey results were compared to the results of the historic cohort using the two-sample t-test.</span></span></span></span></span></span></span></span></span></span></span></p> <p><span><span><span><span><span><span><span><span><span><span><span><b>Results</b></span></span></span></span></span></span></span></span></span></span></span></p> <p><span><span><span><span><span><span><span><span><span><span><span>A total of n=162 students (18.5%) evaluated the OC. In the historic cohort, n=252 (30.5%) completed the survey. 85.3% of the OC respondents thought that the atmosphere during the practical exercise was productive and 83.0% greatly appreciated the flexibility in terms of time management. 27.7% of the respondents thought that traditional BTCs should be supplemented through more online activities in the future. The respondents appreciated the online resources as well as having the opportunity to undertake a MH taking course during the COVID-19 pandemic. With respect to the ability of independently taking a MH upon completion of the course, the OC was rated significantly lower relative to the BTC (mean OC: 2.4, SD: ±1.1 vs. mean BTC: 1.9, SD: ±1.1 (1=<i>strongly agree</i>; 5=<i>strongly disagree</i>); p&lt;0.0001).</span></span></span></span></span></span></span></span></span></span></span></p> <p><span><span><span><span><span><span><span><span><span><span><span><b>Conclusion</b></span></span></span></span></span></span></span></span></span></span></span></p> <p><span><span><span><span><span><span><span><span><span><span><span>OCs are a feasible format and seem to convey the theory and practical implementation in a peer-exercise format of MH taking to medical students. The theoretical background can be acquired with great flexibility. Nevertheless, the students' self-appraisal suggested that the traditional teaching format was more effective at teaching MH taking skills. Thus, we propose a blended learning concept, combining elements of both formats. In this context, we suggest prospective, randomized trials to evaluate blended learning approaches.</span></span></span></span></span></span></span></span></span></span></span></p>

Readiness and determinants of Vietnam's general public to receive the COVID-19 vaccine: A national online cross-sectional study

<p><span><strong>Background</strong>: </span><span>Despite vaccinations' efficacy in combating disease, people's readiness to get the COVID-19 vaccine is substantially varied. This study aimed to assess Vietnamese people's readiness, attitudes, and determinants for COVID-19 vaccination.</span></p> <p><strong><span>Methods</span></strong><span>: </span><span>A web-based cross-sectional online survey was conducted using a convenience sample approach. The Vietnamese population's readiness to receive COVID-19 vaccinations was assessed using the 7C (7Cs) of vaccination readiness scale. The scale was posted on Facebook and Zalo platforms. Descriptive statistics were calculated, and inferential analysis was applied to identify determinants predicting respondents' vaccine readiness.</span></p> <p><span><strong>Results</strong>: </span>Of the 1086 respondents invited to the study, 1026 completed the questionnaire. The Vietnamese population demonstrated a moderate level of readiness for COVID-19 vaccination uptake, with an average 7Cs score of 103.25<span>±</span>15.13. <span>A high level was underscored in <em>complacency, constraints, collective responsibility</em>, and <em>compliance</em> components, and a low level was reported in the<em> calculation</em> component. </span>The Vietnamese population emphasized that the awareness of the significant adverse effects of COVID-19 vaccination was the primary factor influencing their <span>readiness</span> to get the vaccine (<em>p</em> &lt; 0.001). Worrying about the vaccine manufacturer and its origin was the second most crucial factor influencing their readiness to get the vaccine (<em>p</em> &lt; 0.001).</p> <p><span><strong>Conclusions</strong>: </span>Building confidence between people and the Vietnamese authorities is a high priority to enhance people's readiness to get the COVID-19 vaccine. The authorities should focus on dispelling disinformation posted on social media and promoting the usefulness of COVID-19 vaccines.</p>

Diagnostic importance of pulmonary interleukin-1 beta and interleukin-8 in ventilator-associated pneumonia online first

ABSTRACT Background Ventilator-associated pneumonia (VAP) is the most commonly fatal nosocomial infection. Clinical diagnosis of VAP remains notoriously inaccurate. We tested the hypothesis that significantly augmented inflammatory markers distinguish VAP from conditions closely mimicking VAP. Methods A prospective, observational cohort study in two university hospital intensive care units recruiting 73 patients with clinically suspected VAP, and a semi-urban primary care practice recruiting a reference group of 21 age- and sex-matched volunteers. Growth of pathogens at &gt;104 colony forming units/ml bronchoalveolar lavage fluid (BALF) distinguished VAP from 'non-VAP'. Inflammatory mediators were quantified in BALF and serum. Mediators showing significant differences between patients with and without VAP were analysed for diagnostic utility by receiver operator characteristic (ROC) curves. Results. Seventy-two patients had recoverable lavage - 24% had VAP. BALF interleukin (IL)-1β, IL-8, granulocyte-colony stimulating factor and macrophage inflammatory protein 1α were significantly higher in the VAP group (all P&lt;0·005). Using a cut off of 10 pg/ml, BALF IL-1β generated negative likelihood ratios for VAP of 0·09. In patients with BALF IL-1β &lt;10 pg/ml the post-test probability of VAP was 2·8%. Using a cut off value for IL-8 of 2 ng/ml, positive likelihood ratio was 5.03. There was no difference in cytokine levels between patients with sterile BALF and those with growth of &lt;104 CFU/ml. Conclusions BALF IL-1β and IL-8 are amongst the strongest markers yet identified for accurately demarcating VAP within the larger population of patients with suspected VAP. These findings have potential implications for reduction in unnecessary antibiotic use but require further validation in larger populations.
