Data from: 8-way randomized controlled trial of doxylamine, pyridoxine and dicyclomine for nausea and vomiting during pregnancy: restoration of unpublished information

Objectives: We report information about an unpublished 1970s study ("8-way" Bendectin Study) that aimed to evaluate the relative therapeutic efficacy of doxylamine, pyridoxine, and dicyclomine in the management of nausea and vomiting during pregnancy. We are publishing the trial's findings according to the restoring invisible and abandoned trials (RIAT) initiative because the trial was never published. Design: Double blinded, multi-centred, randomized placebo-controlled study. Setting: 14 clinics in the United States. Participants: 2308 patients in the first 12 weeks of pregnancy with complaints of nausea or vomiting were enrolled. Interventions: Each patient was randomized to one of eight arms: placebo, doxylamine/pyridoxine/dicylcomine, doxylamine/pyridoxine, dicylomine/pyridoxine, doxylamine, dicyclomine/pyridoxine, pyridoxine and dicyclomine. Each patient was instructed to take 2 tablets at bedtime and 1 additional tablet in the afternoon or morning if needed, for 7 nights. Outcomes: Reported outcomes included the number of hours of nausea reported by patients, the frequency of vomiting reported by patients and the overall efficacy of medication as judged by physicians. Results: Data from 1599 (69% of those randomized) participants were analyzed. Based on the available summary data of physician evaluation of symptoms and ignoring missing data and data integrity issues, the proportion of participants who were "evaluated moderate or excellent" was greater in each of the seven active treatment groups when compared with placebo (57%): doxylamine/pyridoxine/dicylcomine (14% absolute difference versus placebo; 95% CI: 4 to 24), doxylamine/pyridoxine (21; 95% CI 11 to 30), dicylomine/pyridoxine (21; 95% CI 11 to 30), doxylamine (20; 95% CI 10 to 29), dicyclomine/pyridoxine (4; 95% CI -6 to 14), pyridoxine (9; 95% CI -1 to 19) and dicyclomine (4; 95% CI -6 to 14). Based on incomplete information, the most common adverse events were apparently drowsiness and fatigue. There is a high risk of bias in these previously unpublished results given the high attrition rate in a 7 day trial, the lack of prespecified outcomes or analyses, and the exclusion of some data because of questionable data integrity. Conclusion: The available information about this "8-way Bendectin" trial indicates it should not be used to support the efficacy of doxylamine, pyridoxine or dicyclomine for the treatment of nausea and vomiting during pregnancy because of a high risk of bias.

Data from: Investigating the impact of feedback update interval on the efficacy of restorative brain–computer interfaces

Restorative brain–computer interfaces (BCIs) have been proposed to enhance stroke rehabilitation. Restorative BCIs are able to close the sensorimotor loop by rewarding motor imagery (MI) with sensory feedback. Despite the promising results from early studies, reaching clinically significant outcomes in a timely fashion is yet to be achieved. This lack of efficacy may be due to suboptimal feedback provision. To the best of our knowledge, the optimal feedback update interval (FUI) during MI remains unexplored. There is evidence that sensory feedback disinhibits the motor cortex. Thus, in this study, we explore how shorter than usual FUIs affect behavioural and neurophysiological measures following BCI training for stroke patients using a single-case proof-of-principle study design. The action research arm test was used as the primary behavioural measure and showed a clinically significant increase (36%) over the course of training. The neurophysiological measures including motor evoked potentials and maximum voluntary contraction showed distinctive changes in early and late phases of BCI training. Thus, this preliminary study may pave the way for running larger studies to further investigate the effect of FUI magnitude on the efficacy of restorative BCIs. It may also elucidate the role of early and late phases of motor learning along the course of BCI training.

Data from: Is saltmarsh restoration success constrained by matching natural environments or altered succession? a test using niche models

1.Restored habitats, such as saltmarsh created through managed realignment, sometimes fail to meet targets for biological equivalence with natural reference sites. Understanding why this happens is important in order to improve restoration outcomes. 2.Elevation in the tidal frame and sediment redox potential are major controls on the distribution of saltmarsh plants. We use niche models to characterize ten species' responses to these, and test whether differences in species occurrence between restored and natural saltmarshes in the UK result from failure to recreate adequate environmental conditions. 3.Six species occurred less frequently in recently restored marshes than natural marshes. Failure of restored marshes to achieve the elevation and redox conditions of natural marshes partially explained the underrepresentation of five of these species, but did not explain patterns of occurrence on older (&gt; 50 years) restored marshes. 4.For all species, an effect of marsh age remained after controlling for differences in environmental conditions. This could be due to differences in successional mechanism between restored and natural marshes. In recently restored marshes, high-marsh species occurred lower in the tidal frame and low-marsh species occurred at higher elevations than in natural marshes. This supports the hypothesis that competition is initially weaker in restored marshes, because of the availability of bare sediment across the whole tidal frame. Species that establish outside their normal realized niche, such as Atriplex portulacoides, may inhibit subsequent colonization of other species that occurred less frequently than expected on older restored marshes. 5.Synthesis and applications. Niche models can be used to test whether abiotic differences between restored sites and their natural counterparts are responsible for discrepancies in species occurrence. In saltmarshes, simply replicating environmental conditions will not result in equivalent species occurrence.

Barriers to restoration: Pollution alters nurse effects for an ecosystem engineer

<p>Nurse plants modify the environment to facilitate the recruitment of propagules, and are potentially valuable tools for ecological restoration. Yet empirical tests, particularly in polluted environments, remain rare. The few studies that have examined nurse-effects in polluted environments report exclusively positive effects, but most tests have focused on pollution-tolerant species in metal contaminated environments. Biotic interactions are highly context-dependent, however, such that extrapolations to other suites of species and pollutant types appear premature. We examined changes in intraspecific nurse-effects across pollution regimes for a pollution-sensitive, macroalgal ecosystem engineer that is a target for intertidal restoration. In a manipulative field experiment, we out-planted propagules in the presence and absence of conspecific canopies at (i) unpolluted control shores and (ii) shores that received a partially-remediated (low toxicity) sewage effluent, and monitored the performance (survival and growth) of these propagules over-time. Algal canopies facilitated the survival and early growth of propagules at control sites but not at partially-remediated sites, where propagules performed poorly irrespective of canopy presence. <i>Synthesis and applications: </i> Using a pollution-sensitive ecosystem engineer, we show for the first time that nurse-plant effects can be contingent on pollution regime, and that exposure to pollution (even at low levels) can erode nurse effects. We caution that nurse-plants are unlikely to be a universal tool for ecological restoration, even in systems where facilitative nurse-effects naturally occur (in our case, algal canopies on unpolluted rocky reefs). Management practitioners should carefully consider the pollution tolerance of target species, and test whether nurse-plant effects are maintained under current pollution regimes, before large-scale translocations are attempted.</p>

Tallgrass prairie plant communities across twenty years of restoration

<p>Ecosystem restoration projects need to measure progress toward project goals and deliver desired outcomes. This study examines longitudinal plant community data collected from permanent transects at the Nachusa Grasslands preserve in northern Illinois, USA. Managers established permanent transects for repeated plant community monitoring beginning in the mid-1990s. Native plant communities, including rare species, have persisted, or improved with management over two decades. Planted prairies have lower proportions of native species than native prairies but have generally maintained native-dominated communities and in some cases, increased presence of native species. Savannas have shown a distinct transition from shrub-dense communities to herbaceous understories dominated with native species. Restoration efforts at Nachusa Grasslands have been successful at sustaining unique native plant communities through management practices like prescribed fire, brush removal, and aggressive invasive species control. As a disturbance dependent ecosystem that has developed with human management over millennia, tallgrass prairie and savanna can thrive through restoration and active management.</p>

Plant health and habitat restoration, creation questionnaire
