A 4,302-patient cohort study of association of rare genetic alterations with endocrine disorders

<p><span>Endocrine pathologies including disorders such as diabetes and dysfunctions of endocrine glands, are frequently associated with genetic predisposition. This study investigated the association of endocrine diseases with genetic variants, copy number variations (CNVs), and mutational load of molecular pathways in 4302 patients with 409 ICD-10 diagnoses who underwent DNA testing using next-generation sequencing at the National Medical Research Center for Endocrinology (Moscow) from November 2017 </span><span>till</span><span> January 2024. We analyzed rare protein-altering genetic variants using three control cohorts (gnomAD3, RUSeq healthy, experimental).<span>&nbsp; </span>We identified 143 associated variants for diabetes mellitus and 188 genetic variants across other 18 different ICD-10 groups of diagnoses, including 25% and 30% of previously undescribed variants, respectively. In addition, we investigated the aggregation of genetic variants across individual genes and their functional ensembles (molecular pathways) and identified 105 and 101 associations with ICD-10 diagnoses, respectively. In addition, we identified 35 pathogenic and 91 likely pathogenic CNVs in 925 patients with whole exome sequencing profiles. Among them, 9 and 44 CNVs, respectively, were not previously described. Totally, we found statistically significant associations between CNVs and endocrine pathologies for 168 genes. These results expand our understanding of endocrine disease mechanisms and may indicate new potential therapeutic targets.</span></p>

Etiology, characteristics and occurrence of heart diseases in rural Lesotho (ECHO-Lesotho): A retrospective echocardiography cohort study

<p><strong>Background</strong></p> <p>In 2019, 600&rsquo;000 people in Africa died of heart failure and heart diseases will increase on the continent. It is crucial to understand the regional etiologies and risk factors for heart failure and underlying heart diseases. However, echocardiography data from rural Africa are scarce and from Lesotho non-existent. This study aims to examine the occurrence, characteristics and etiology of heart failure and heart diseases using echocardiography data from a referral hospital in rural Lesotho.</p> <p><strong>Methods</strong></p> <p>We conducted a retrospective cohort study at Seboche Mission Hospital, the only referral hospital in Butha-Buthe district (Lesotho) with an echocardiography department. We included data from all individuals referred to the department between January 2020 and May 2021. From non-hospitalized patients echocardiographic diagnosis, sex and age were available, from hospitalized patients additional sociodemographic and clinical data could be extracted.</p> <p><strong>Results</strong></p> <p>In the study period, a total of 352 echocardiograms were conducted; 213 had abnormal findings (among them 3 children). The majority of adult participants (130/210; 64%) were female and most frequent heart diseases were hypertensive (62/210, 30%), valvular (39/210, 19%) and chronic pulmonary (37/210, 18%). Heart failure represented 11% of hospitalizations in the same period. Among the 126 hospitalized heart failure patients, the most common etiology was chronic pulmonary heart disease (32/126; 25%). Former mine workers and people with a history of tuberculosis were more likely to have a chronic pulmonary heart disease.</p> <p><strong>Conclusions</strong></p> <p>The leading cause of heart disease in this setting is hypertension. However, in contrast to other African epidemiological studies, chronic pulmonary heart disease is unexpectedly common. There is an urgent need to improve awareness and knowledge about lung diseases, make diagnostic and therapeutic options available and increase prevention.</p>

Two clinically relevant features: the type of the left ventricular outlet (LVOT) shape, either convergent or divergent, as well as the angle between LVOT and the aorta can be specified. Based on a statistical shape model and appropriate filtering strategies, virtual patients featuring the specified anatomical characteristics are then generated.</p>

Data and R code for: Temperature-induced multispecies cohort effects in sympatric snakes

<p>In reptiles, reproductive maturity is often determined by size rather than age. Consequently, growth early in life may influence population dynamics through effects on generation time and survival to reproduction. Because reproductive phenology and pre- and post-natal growth are temperature-dependent, environmental conditions may induce multi-species cohort effects on body size in sympatric reptiles. I present evidence of this using ten years of neonatal size data for three sympatric viviparous snakes, Dekay's Brownsnakes (<i>Storeria dekayi</i>), Red-bellied Snakes (<i>S. occipitomaculata</i>) and Common Gartersnakes (<i>Thamnophis sirtalis</i>). End-of-season neonatal size varied in parallel across species such that snout-vent length was 36-61% greater and mass was 65-223% greater in years when gestating females could achieve higher April-May (vs. June-July or August-September) operative temperatures. Thus, temperature had a larger impact during follicular enlargement and ovulation than during gestation or post-natal growth. Multi-species cohort effects like these may affect population dynamics and increase with climate change.</p>

Data for: Evaluation of antibody kinetics and durability in health individuals vaccinated with inactivated COVID-19 vaccine (CoronaVac): a cross-sectional and cohort study in Zhejiang, China

<p><strong>Background</strong>: Although inactivated COVID-19 vaccines are proven to be safe and effective in the general population, the dynamic response and duration of antibodies after vaccination in the real world should be further assessed.</p> <p><strong>Methods</strong>: We enrolled 1067 volunteers who had been vaccinated with one or two doses of CoronaVac in Zhejiang Province, China. Another 90 healthy adults without previous vaccinations were recruited and vaccinated with three doses of CoronaVac, 28 days and 6 months apart. Serum samples were collected from multiple timepoints and analyzed for specific IgM/IgG and neutralizing antibodies (NAbs) for immunogenicity evaluation. Antibody responses to the Delta and Omicron variants were measured by pseudovirus-based neutralization tests.</p> <p><strong>Results</strong>: Our results revealed that binding antibody IgM peaked 14–28 days after one dose of CoronaVac, while IgG and NAbs peaked approximately 1 month after the second dose and then declined slightly over time. Antibody responses had waned by month 6 after vaccination and became undetectable in the majority of individuals at 12 months. Levels of NAbs to live SARS-CoV-2 were correlated with anti-SARS-CoV-2 IgG and NAbs to pseudovirus, but not IgM. Homologous booster around 6 months after primary vaccination activated anamnestic immunity and raised NAbs 25.5-fold. The neutralized fraction subsequently rose to 36.0% for Delta (p=0.03) and 4.3% for Omicron (p=0.004), and the response rate for Omicron rose from 7.9% (7/89) to 17.8% (16/90).</p> <p><strong>Conclusions</strong>: Two doses of CoronaVac vaccine resulted in limited protection over a short duration. The inactivated vaccine booster can reverse the decrease of antibody levels to prime strain, but it does not elicit potent neutralization against Omicron; therefore, the optimization of booster procedures is vital.</p>

Monocentric retrospective cohort of 30,423 COVID-19 patients

<p><span>Population and design: </span><span>Monocentric cohort comparing COVID-19 patients exposed or not exposed to the treatment hydroxychloroquine (HCQ)+azithromycin (AZ) used as a standard of care in our center (Institut Hospitalo-Universitaire (IHU) Méditerranée Infection, Marseille, France). The inclusion period was from 2 March 2020 to 31 December 2021, with a follow-up period of 6 weeks.</span></p> <p><span>Inclusion and exclusion criteria: </span><span>Data included were those of patients ≥ 18 years of age with PCR-proven COVID-19 disease regardless of symptoms (asymptomatic or symptomatic) and treated in our centre, i.e. having had a medical examination by one of the doctors in our centre (Institut Hospitalo-Universitaire (IHU) Méditerranée Infection, Marseille, France) either as outpatients or as inpatients, i.e. hospitalized on the day of the visit in our outpatient unit following evaluation or directly transferred from another medical ward except intensive care unit. The reasons for exclusion were an erroneous patient identification (identity surveillance and duplicates), lack of available medical data, lack of COVID-19 after checking the medical record (including patients without COVID-19 consulting for a post-COVID-19 syndrome), expression of opposition to the use of their medical data for research purposes (in accordance with European General Data Protection Regulation), and data from patients hospitalized in our center after intensive care. Data from COVID-19 outpatients left without medical advice were excluded.</span></p>

Associations of serum uric acid with cardiovascular disease risk factors: a retrospective cohort study in Southeastern China

<p class="MsoNormal"><span>Objective: To evaluate the associations between serum uric acid (SUA) levels and cardiovascular disease (CVDs) risk factors, focusing on potential sex-specific differences.</span></p> <p class="MsoNormal"><span>Design: A retrospective cohort study.</span></p> <p class="MsoNormal"><span>Setting: A large community-based survey was conducted every two years from 2010 to 2018 in Hangzhou, Zhejiang Province, Southeastern China.</span></p> <p class="MsoNormal"><span>Participants: 6119 participants aged 40 years and above who underwent at least three times of physical examinations were enrolled. </span></p> <p class="MsoNormal"><span>Methods: Participants were categorized into four groups (Q1-Q4) based on baseline SUA quartiles within the normal range, with hyperuricemia (HUA) as the fifth group. The Q1 was the reference. By stratifying participants by gender, the relationships between SUA levels and systolic blood pressure (SBP), diastolic blood pressure (DBP), fasting blood glucose (FBG), and total cholesterol (TC) were investigated using linear regression models in the generalized estimating equation (GEE). Additionally, the associations of elevated SUA levels and HUA with hypertension, hyperglycemia, and dyslipidemia were correspondingly examined using multivariate logistic regression models.</span></p> <p class="MsoNormal"><span>Results: After adjusting for confounding variables, we found positive associations between SUA levels and SBP, DBP, FBG, and TC in women, and with TC in men (P &lt; 0.01). Likewise, Elevated SUA quartiles and HUA were linked to increased dyslipidemia risk in both sexes, and increased hyperglycemia risk only in women, with HRs (95%CI) of 1.64 (1.05-2.55) and 2.37 (1.47-3.81) in the Q4 and HUA group, respectively. Women with HUA had higher hypertension risk (HR=1.45, 95% CI 1.21-1.73), while no such association was observed in men. Stratified analyses revealed significant associations between elevated SUA levels and CVDs risk factors in postmenopausal and non-obese women.</span></p> <p class="MsoNormal"><span>Conclusions: Elevated SUA levels increase the risk of dyslipidemia in both sexes. SUA levels within normal-range and HUA are positively associated with hyperglycemia and hypertension in postmenopausal women, but not in men.</span></p>

Data from: Novel methods to define invasive procedures at the end-of-life were developed to improve quality of end of life care research: A population-based cohort study in colorectal cancer

<p><strong>Background</strong></p> <p>Understanding the use of invasive procedures (IPs) at the end-of-life (EoL) is important to avoid under- and overtreatment, but epidemiologic analysis is hampered by limited methods to define treatment intent and EoL phase. This study applied novel methods to report IPs at the EoL using a colorectal cancer (CRC) case study.</p> <p><strong>Methods</strong></p> <p>An English population-based cohort of adult patients diagnosed between 2013 and 2015 was used with follow-up to 2018. Procedure intent (curative, non-curative, diagnostic) by cancer site and stage at diagnosis was classified by two surgeons independently. Joinpoint regression modelled weekly rates of IPs for 36 sub-cohorts of patients with incremental survival of 0-36 months. EoL phase was defined by a significant IP rate change before death. Zero-inflated Poisson regression explored associations between IP rates and clinical/sociodemographic variables.</p> <p><strong>Results</strong></p> <p>Of 87,731 patients included, 41,972 (48%) died. 9,492 procedures were classified by intent (interrater agreement 99.8%). Patients received 502,895 IPs (1.39 and 3.36 per person year for survivors and decedents). Joinpoint regression identified significant increases in IPs four weeks before death in those living 3-6 months, and eight weeks before death in those living 7–36 months from diagnosis. 7,908 (18.8%) patients underwent IPs at the EoL, with stoma formation the most common major procedure. Younger age, early-stage disease, men, lower comorbidity, those receiving chemotherapy and living longer from diagnosis were associated with IPs.</p> <p><strong>Conclusions</strong></p> <p>Methods to identify and classify IPs at the EoL were developed and tested within a CRC population. This approach can be now extended and validated to identify potential under- and overtreatment. </p>

<p><span>T</span>his dataset corresponds to the paper "Size-selective predation effects on juvenile Chinook salmon cohort survival off Central California evaluated with an individual-based model" which is in press at Fisheries Oceanography. The abstract for this paper is as follows: </p> <p>Variation in the recruitment of salmon is often found to be correlated with marine climate indices, but mechanisms behind environment-recruitment relationships remain unclear and correlations often break down over time. We used an ecosystem modeling approach to explore bottom-up and top-down mechanisms linking a variable environment to salmon recruitment variations. Our ecosystem model incorporates a regional ocean circulation sub-model for hydrodynamics, a nutrient-phytoplankton-zooplankton sub-model for producing planktonic prey fields, and an individual-based model (IBM) representing juvenile Chinook salmon (<em>Oncorhynchus</em> <em>tshawytscha</em>), combined with observations of foraging distributions and diet of a seabird predator. The salmon IBM consists of modules, including a juvenile salmon growth module based on temperature and salmon-prey availability, a behavior-based movement module, and a juvenile salmon predation mortality module based on juvenile salmon size distribution and predator-prey interaction probability. Seabird-salmon interactions depend on spatial overlap and juvenile salmon size, whereby salmon that grow past the size range of the prey distribution of the predator will escape predation. We used a 21-year historical simulation to explore interannual variability in juvenile Chinook salmon growth and predation-mediated survival under a range of ocean conditions for sized-based mortality scenarios. We based a series of increasingly complex predation scenarios on seabird observational data to explore variability in predation mortality on juvenile Chinook salmon. We initially included information about the predator spatial distribution, then added population size, and finally, the predator's diet percentage made up of juvenile salmon. Model agreement improves with added predator complexity, especially during periods when predator abundance is high. Overall, our model found that when the fraction of juvenile salmon in seabird diet increased relative to alternate prey (e.g., Northern anchovy <em>Engraulis</em> <em>mordax</em>, and juvenile rockfish <em>Sebastes</em> spp.), there was a concomitant decrease in salmon cohort survival during their first year at sea.</p>

Prospective cohort study of a community-based primary care program's effects on pharmacotherapy quality in low-income Peruvians with type 2 diabetes and hypertension

<p>A door-to-door survey was conducted to enumerate all household members by age and sex in a low-income community in Peru. 856 adults 35 years and older were eligible to participate in screening for type 2 diabetes and hypertension. 709 (83%) participated in screening. 130 (18.3%) were diagnosed with hypertension and/or type 2 diabetes of which 109 (84%) participated at program onset and 22 were added later from earlier non-participants in screening or program onset to form the cohort of 131 patients with diabetes and/or hypertension. The primary care program had components of the Chronic Care Model, community health workers, and freely accessible visits and medications. The program operated between September 2011 and May 2014, and consisted of two care periods (separated by a six-month hiatus), first a 10-month home-care period, then a 17-month clinic-care period. The dataset is two files corresponding to two exposures: the 27-month program overall (post- versus pre-) (N=262 observations, 131 pairs with patients as self-controls) and care period (clinic versus home), N=211 (109 home and 102 clinic observations, &gt;131 because 80 patients participated in both care periods). Exposures were evaluated for their effects on guidelines-based pharmacotherapy standards: hypoglycemic and antihypertensive medications, low-dose aspirin, and first-line angiotensin converting enzyme inhibitor (ACEi) treatment of diabetes with elevated blood pressure.</p>

HLA class I Sanger sequences data of Honduras HIV cohort

<p>HLA polymorphisms represent the strongest genetic modifier of HIV disease progression. Diverse HLA distribution can lead to distinct HIV control landscapes at the population level. We aimed to describe HLA allele and haplotype frequencies (linkage disequilibrium, LD), CCR5-Δ32 frequency and the impact of these variants on HIV disease outcome. HLA class I (cI) loci were typed at 4-digit resolution, and CCR5 variants were determined in 402 HIV clade B-infected, ART-naïve individuals from Honduras. HLA LD were assessed using Fisher's exact test. Using univariable and multivariable analyses we evaluated HLA associations with HIV pVL and CD4 counts. We did not find any effect on HIV control between CCR5 genotypes. Previously defined HLA associations were found: <em>B*57:01/03</em>, <em>B*42:01</em>, <em>A*25:01</em> and <em>C*12:03</em> (protective), and <em>B*53:01</em> and <em>A*68:01</em> (risk). Being consistent with our previous research in a Mesoamerican HIV cohort, Amerindian <em>B*35:12</em> was associated to poor HIV control. Other HLA-HIV associations not previously described were <em>C*03:04</em> and <em>B*08:01</em> that were associated with higher pVL. Overall, this first report highlights the immunogenetic uniqueness admixture of the Honduras population that express Amerindian, Caucasian and African HLA subtypes. These findings not only support this cohort as ideal for identifying HLA correlates of HIV control but also may improve future research regarding allotransplantation and disease association.</p>
