Extended data for behavioural change for Parkinson's disease: A randomised controlled feasibility study to promote physical activity and exercise adherence among people with Parkinson's, study protocol

<p><strong>Background: </strong>Parkinson's is a common progressive neurological condition characterized by motor and non-motor deficits. Physical activity and exercise can improve health, but many people with Parkinson's have trouble reaching the recommended dosage. Our recent literature review found improvements in exercise adherence with behavioural change interventions, but it remains unclear which are most effective. Further qualitative research and patient and public involvement has informed a novel behavioural change support intervention to be tested alongside an existing exercise program.<strong> </strong></p> <p><strong>Objective: </strong>To examine the feasibility of behavioural change support techniques delivered alongside an exercise programme to improve physical activity, function, and self-efficacy in PwP (and study procedures) to inform a future pilot RCT trial.<strong> </strong></p> <p><strong>Methods: </strong>A parallel-arm single blinded randomised feasibility study. Twenty participants with Parkinson's (Hoehn and Yahr stage 1-3) will be recruited from a physiotherapy primary-care waiting list. Following written consent, and baseline assessment, the participants will be randomly allocated to the intervention (n=10) or the control group (n=10). Both groups will receive usual care, which includes a weekly program of a multidisciplinary education, a supervised exercise class and a prescribed home exercise program. The intervention group will receive additional behavioural change techniques, targeting behaviour regulation, belief about capabilities and social influences. Class and home exercise adherence, behavioural component uptake and adherence, and negative events will be recorded. Outcomes will include enrolment and maintenance rates, physical function, falls, physical activity, and exercise self-efficacy measured pre- and post- the 12- week program (in-person). Surveys will be used to compare experiences and satisfaction between groups. Exit interviews will be completed with the intervention group only, exploring their experience of the behavioural change techniques.  </p> <p><strong>Discussion: </strong>The results will help inform a future pilot RCT, based on the intervention acceptability, consent rate, maintenance, and protocol integrity.</p>

Antibodies to Borrelia burgdorferi and Bartonella species in serum and synovial fluid from people with rheumatic diseases

<p>Vector-borne infections may underlie some rheumatic diseases, particularly in people with joint effusions. The aim of this study was to compare serum and synovial fluid antibodies to<em> B. burgdorferi </em>and <em>Bartonella </em>spp. in patients with rheumatic diseases.</p> <p>This observational, cross-sectional study examined paired synovial fluid and serum specimens collected from 110 patients with joint effusion between October 2017 and January 2022. Testing for antibodies to <em>B. burgdorferi </em>(using CDC criteria) and <em>Bartonella </em>spp. via two Indirect Fluorescent Antibody [IFA] assays, was performed as part of routine patient care.</p> <p>There were 30 participants (27%) with positive two-tier <em>B. burgdorferi</em> serology, and 26 participants (24%) with IFA seroreactivity (≥1:256) to <em>B. henselae </em>and/or <em>B. quintana. </em>Both <em>B. burgdorferi</em> IgM and IgG were detected more frequently in synovial fluid than serum: 27% of patients were either IgM or IgG positive in synovial fluid, compared to 15.5% in serum (p = 0.048). Conversely, <em>B. henselae </em>and <em>B. quintana </em>antibodies were detected more frequently in serum than synovial fluid; overall only 2% of patients had positive IFA titers in synovial fluid, compared to 24% who had positive IFA titers in serum (p &lt;0.001). There were no significant associations between <em>B. burgdorferi</em> or <em>Bartonella</em> spp. seroreactivity with any of the clinical rheumatological diagnoses.</p> <p>This study provides preliminary support for the importance of synovial fluid antibody testing for documenting exposure to <em>B. burgdorferi</em>, but not for documenting exposure to <em>Bartonella </em>spp.</p>

Meta-Organizations: a preliminary list of federations and confederations of states, tribes and indigenous peoples

Data set of a survey of people 6 months before reaching their regular retirement age

<p>This is an&nbsp;anonymized&nbsp;data set of a standardized survey of people about six months (+/- 3 months) before reaching their regular retirement age (n = 400). The survey is representative for&nbsp;the German-speaking part of Switzerland. Data was gained by&nbsp;telephone interviews, conducted by the market research institute DemoSCOPE in September 2019. The survey was part of the project &ldquo;Identity Constructions for Retirement&rdquo;, funded by the Swiss National Science Foundation (SNSF). &nbsp;Personal details that could lead back to the identity of the participants (among others postal code, profession, responses to open-ended questions) were removed from the data set.</p>

<p><strong>Supplementary materials </strong>(Appendix E) for the article:</p> <p><strong><em>Ambient Intelligence System Enabling People With Blindness to Develop Electrotechnical Components and Their Drivers</em></strong></p> <p><em>Abstract: </em> Along with the development of new assistive technologies, including ambient intelligence (AmI) environments, new job opportunities are created for people with blindness or visual impairment. Whereas research on software development for people with blindness has been conducted since the 1960s, the design and development of electrotechnical equipment still lacks any way to compensate for the disability which would enable people with blindness to perform even the activities pertaining to this field. This article aims to introduce these new technological procedures enabling the compensation of the disability of people with blindness when designing and developing electrotechnical components (or prototypes) and their drivers by using the AmI system RUDO modules and other tools. This includes the modules enabling measuring electrotechnical quantities, tracing of oscilloscope curves, and ensuring a unified user interface for programming and operation of the devices connected. Other approaches are introduced herein that focused on the production of mechanical components for device construction. This article also contains an illustrative video of practical use of these technological procedures by a person with blindness. To assess the usability of the designed technological procedures to the work of a designer with blindness and one without visual impairment, the cognitive walkthrough method was used. The main contribution of this article is to broaden the knowledge base by the principles of involving people with blindness in the development and construction of electrotechnical components (or prototypes). DC/DC converter is connected into miniature screw terminal block, commonly available in electrotechnical equipment stores. The DC/DC converter has six contacts, which means that in two cases two contacts are connected at the same time to one socket. After connecting the DC/DC converter, the laboratory power supply is switched on to keep the device energized. Person with blindness measures the voltage at the input and also at the output of the DC/DC converter. This will ensure that it works correctly in the given connection.</p> <p>A power switcher is connected to the constructed device. In our case, the power switcher should switch the power supply of the appliances to either photovoltaics or the mains. The device with DC / DC converter is connected to the LAN and via the second wire is connected to the power switcher, i.e.:</p> <ul> <li>the software on the home server will control this device with a DC/DC converter via LAN,</li> <li>a device with a DC/DC converter will command the power switcher to switch the power current.</li> </ul> <p>The power switcher is turned on and the person with blindness goes to the computer, where he runs a test program to test the entire connection. The test program taps the relay three times and waits a while. The test program repeats this sequence until it is interrupted. In the end, the contacts of the power relay were measured with an audible short-circuit meter, which always started to emit a whistling tone when the state was switched on. This debugged the functionality of the hardware and its data connection to the home server.</p> <p><em><strong>Part 2: Preparation of mechanical components for device construction.</strong></em></p> <p>The example includes cutting a board made of pertinax material on miter saw and its further modification. This modification involves grinding the edge of the cut board and drilling holes for further assembly of the board using a steel stencil and a meter marked in Braille. Finally, two miniature screw terminal blocks can be glued to the board prepared in this way.</p> <p>&nbsp;</p>

Patient-reported outcomes among people living with HIV on single- versus multi-tablet regimens

<p><strong>Background: </strong>Single-tablet regimens (STR) have become the golden standard in antiretroviral therapy. However, there are calls to reintroduce (generic) components as multi-tablet regimens (MTR) because of cost-savings. Patient-reported outcome measures (PROs) are an important touchstone to support or to renounce this proposal. As such, our aim was to compare PROs of people living with HIV taking an STR versus a MTR in a real world setting.</p> <p><strong>Materials and methods: </strong>188 people living with HIV participated in a longitudinal study. 132 remained on their MTR and 56 switched to an STR. At baseline, months 1-3-6-12-18 and 24, participants filled in questionnaires on health-related quality of life (HRQoL), depressive symptoms, HIV symptoms, neurocognitive complaints (NCC) treatment satisfaction and adherence. Generalized linear mixed models and generalized estimation equations mixed models were built.</p> <p><strong>Results: </strong>At baseline, the two groups did not differ in clinical parameters and PROs. The mixed models revealed differences among the groups regarding neurocognitive complaints and treatment satisfaction. The odds of having NCC increased by 4,1% per month in the STR-group as compared to the MTR-group (p=0.035). Nonetheless, treatment satisfaction also increased in the STR-group: at month 6, median change score was highly positive: 24 (IQR 7,5-29). Treatment satisfaction showed a significantly different evolution in the groups: the estimated state score increased by 3,3 in the STR-group while it decreased by 0,2 in the MTR-group (p=0.003). HRQoL measures, HIV symptoms, depressive symptoms and adherence did not differ between the groups over time.</p> <p><strong>Conclusions: </strong>Neurocognitive complaints and treatment satisfaction differed between the two groups. More people on an STR reported NCC over time compared to the MTR-group. Contrarily, treatment satisfaction in the STR-group increased significantly over time. It should be further investigated to which extent treatment satisfaction is a predictor for long-term treatment persistence and health outcomes.</p>

People and machines: co creating with heritage collections

<p>Machine-learning technologies such as Handwritten Text Recognition (HTR) are beginning to be used to open and transform cultural heritage material, enabling search, discovery and the creation of linked data. Whereas in 2014, Ridge described cultural heritage crowdsourcing projects as asking the public to undertake tasks that cannot be done automatically,1 today, some projects are asking volunteers to undertake tasks which could be completed by a machine with a high level of speed and accuracy. A deeper exploration of the opportunities and challenges presented by increased incorporation of machine-learning technologies into crowdsourcing projects has yet to be undertaken. This workshop was designed to start these discussions.<br> The &lsquo;People and Machines&rsquo; interdisciplinary workshop explored the best routes to fuse digital innovations with the dedication and enthusiasm of volunteers through discussion of what makes an effective crowdsourcing task, how to maintain volunteer motivation, and methods of supporting volunteers to produce useful data, both as part of traditional crowdsourcing, and as part of workflows incorporating machine learning.<br> This report uses the term &lsquo;machine learning&rsquo; to refer to a range of technical approaches which use statistical models and algorithms to analyse and draw inferences from patterns in data. Machine learning models and algorithms &lsquo;learn&rsquo; from the data they are given, and autonomously adapt and improve their accuracy in response. Machine learning, which includes supervised learning, unsupervised learning, and deep learning, is a subfield of Artificial Intelligence.</p>

Measurement of Social Strain in People with Dementia: A Pre-liminary Study of the Reliability and Validity of the Negative Relationship Quality Questionnaire in Indonesia

<p>People<strong>&nbsp;</strong>with dementia (PWD) may exhibit symptoms that negatively affect their relationships with their families or friends which could cause social strain. The Negative Relationship Quality (NRQ) questionnaire can be used to measure social strain in PWD. There has never been an Indonesian adaptation of the NRQ. This preliminary study aimed to measure the validity and reliability of the NRQ among PWD in Indonesia (NRQ-INA). This study used a cross-sectional design. Forward&ndash;backward translation methods were conducted first. Pearson&rsquo;s correlation and factor analysis were employed for the validity test. Cronbach&rsquo;s alpha and test&ndash;retest were used to determine reliability. The NRQ-INA has four parallel items related to social strain that are divided into three subscales and asked to spouse/partner, family members, and friends, leading to a total of 12 questions. The results of validity testing from 60 respondents showed that all items in the NRQ-INA were strongly valid with correlation coefficients (<em>r</em>) of &gt;0.8 (<em>p</em>&lt;0.01). Factor analysis showed a convergence with&nbsp;the&nbsp;variance explained&nbsp;of more than 50% for all items in each subscale, which&nbsp;also indicated that NRQ-INA had acceptable construct validity to measure social strain. Cronbach&rsquo;s alpha values (&alpha;) were 0.926, 0.942, and 0.938 for the subscales of spouse, friends, and family members, respectively. The correlations of test&ndash;retest reliability for all items were &gt;0.7 (<em>p</em>&lt;0.01), demonstrating a reliable NRQ-INA measurement. In conclusion, NRQ-INA had a good validity and reliability to measure social strain in PWD. Further study of the concurrent validity among PWD is still needed.</p>
