The role of hemostatic agents following dental extractions: a systematic review and meta-analysis

Objective: This study critically evaluated the role of hemostatic agents following dental extractions, and the benefits they add over conventional hemostatic measures.

Materials and methods: a literature search was conducted, including publications that fulfilled the criteria developed based on the PICOS guidelines; Population: patients who receive simple or surgical tooth extractions, Intervention: the use of hemostatic agents following dental extraction, Control: any type of conventional hemostatic measures (i.e. suture, gauze pressure), Outcome: time to achieve hemostasis following tooth removal, postoperative pain, swelling, or bleeding events, and Study design: randomized/controlled clinical trials.

Results: Twenty-eight articles met the inclusion criteria, and 24 were meta-analyzed. Several types of hemostatic agents were used, namely tissue adhesives made from cyanoacrylate or fibrin, tranexamic acid, hemocoagulase, collagen membrane, HemCon dental dressing, epsilon aminocaproic acid, Surgicel, sponges or plugs composed of gelatin, oxidized cellulose, and fibrin. Newer hemostatic agents were also applied, such as Ankaferd blood stopper, and feracrylum. Hemostatic agents demonstrated significantly shorter time to achieve hemostasis and fewer bleeding events in patients on antithrombotic drugs or with bleeding disorders (WMD= -2.12, 95% CI -2.63 to -1.61, P < 0.00001, and RR 0.61, 95% CI 0.43 to 0.86; P= 0.005, respectively). Furthermore, the use of these agents resulted in significantly less postoperative pain following 3rd molar surgery in healthy patients (WMD= -0.33, 95% CI -0.65 to -0.02, P = 0.04). Conversely, they did not have any effect on postoperative swelling (WMD= -0.11, 95% CI -0.31 to 0.08, P = 0.26). The time needed to finish the surgical procedure when hemostatic agents were used was shorter but with no significant difference (MD= -78.93, 95% CI -187.35 to 29.49, P = 0.15).

Conclusion: Included studies differ in their methods of reporting bleeding, pain, and swelling. Thus, the results of this article should be confirmed in future research. Within these limitations, it is concluded that hemostatic agents lead to faster hemostasis and fewer postoperative bleeding incidences, compared to conventional hemostatic measures. Their use is highly recommended following dental extractions in patients on antithrombotic drugs or with bleeding disorders, who are likely to develop postoperative hemorrhage events.

uGIM: week monitorization data of a microgrid with five agents (10/04/19-16/04/19)

uGIM is a microgrid intelligent management software that can represent individual microgrid’s players using a multi-agent approach. This dataset has data regarding a week (from 10-04-2019 to 16-04-2019) of a microgrid with five players (all offices). All agents have consumption and generation data. One of the agents also has sensor data, such as temperature, movement and humidity.

In uGIM, agents are deployed in the player’s facilities using single-board computers. All the data in this dataset is read and stored in five single-board computers. Each agent integrates several resources. In this microgrid deployment, all resources use TCP/IP communication. However, uGIM supports more protocols, such as Modbus/RTU and Modbus/TCP.

uGIM related publications:
- Gomes, L., Vale, Z., & Corchado, J. M. (2020). Exosomal CD73 from serum of patients with melanoma suppresses lymphocyte functions and is associated with therapy resistance to anti-PD- 1 agents

ABSTRACT

Background

CD73 is an ectonucleotidase producing the immunosuppressor mediator adenosine. Elevated levels of circulating CD73 in patients with cancer have been associated with disease progression and poor response to immunotherapy. Immunosuppressive pathways associated with exosomes can affect T-cell function and the therapeutic efficacy of anti-programmed cell-death
protein 1 (anti-PD-1) therapy. Here, we conducted a retrospective pilot study to evaluate levels of exosomal CD73 before and
early during treatment with anti-PD-1 agents in patients with melanoma and its potential contribution to affect T-cell functions and to influence the clinical outcomes of anti-PD-1 monotherapy.

Methods

Exosomes were isolated by mini size exclusion chromatography from serum of patients with melanoma (n=41) receiving nivolumab or pembrolizumab monotherapy. Expression of CD73 and programmed death-ligand 1 (PD-L1) were evaluated on exosomes enriched
for CD63 by on-bead flow cytometry. The CD73 AMPase activity was evaluated by mass spectrometry, also in the presence of selective inhibitors of CD73. Interferon (IFN)-γ production and granzyme B expression were measured in CD3/28 activated T cells incubated with exosomes in presence of the CD73 substrate AMP. Levels of CD73 and PD-L1 on exosomes were correlated with therapy response. Exosomes isolated from healthy subjects were used as control.

Results

Isolated exosomes carried CD73 on their surface, which is enzymatically active in producing adenosine. Incubation of exosomes with CD3/28 activated T cells in the presence of AMP resulted in a significant reduction of IFN-γ release, which was reversed by the
CD73 inhibitor APCP or by the selective A2A adenosine receptor antagonist ZM241385. Expression levels of exosomal CD73 from serum of patients with melanoma were not significantly different from those in healthy subjects. Early on-treatment, expression levels of both CD73 and PD-L1 on exosomes isolated from patients receiving pembrolizumab or nivolumab monotherapy were
significantly increased compared with baseline. Early during therapy exosomal PD-L1 increased in responders, while exosomal CD73 resulted significantly increased in non-responders.

Conclusions

CD73 expressed on exosomes from serum of patients with melanoma produces adenosine and contributes to suppress T-cell functions. Early on-treatment, elevated expression levels of exosomal CD73 might affect the response to anti-PD-1 agents in patients with melanoma who failed to respond to therapy.

Funding

This work was supported by FARB 2020 University of Salerno (SM), P.O.R. Agent-Based Social Skills Training Systems: A Comprehensive Analysis of Commercial Solutions

Agent-based social skills training systems have been gaining attention for their potential to improve social skills development in various contexts. Through a rapid review methodology, data was collected from diverse sources, including company websites and research papers. This study then uses the collected data to categorize 8 commercial systems based on their agent model and feedback approaches, into two categorization tables. The findings reveal notable trends in the use of choice-based input, scenario-defined decision-making, and post-interaction feedback. Additionally, the paper discusses the limitations of these findings, highlights characteristics of commercial systems and compares them to research systems, as well as suggesting areas for future research. This study contributes to the understanding and advancement of agent-based social skills training systems, offering guidance to researchers in this field.
