Data from: Longitudinal associations between diurnal cortisol variation and later life cognitive impairment
Objective: To determine whether HPAA dysfunction is prospectively associated with global cognitive impairment in later life Methods: This cross-cohort study integrates two large longitudinal datasets: Whitehall II and the National Survey for Health and Development (NSHD), on data collected in the Whitehall II study in 2002/2004, 2007/2009 and 2012/2013; and for NSHD in 2006/2010 and 2015. Serial salivary cortisol samples were collected multiple times within a 24-hour period at mean ages 61.2 and 65.9 years in Whitehall II and at age 60-64 from NSHD participants. Cortisol profile is defined using cortisol awakening response (CAR) and am:pm ratio. Cognitive function was measured using the MMSE in Whitehall II and ACE-III in NSHD, harmonised into a thirty-point score. Models were adjusted for age, sex, diagnoses of hypertension, diabetes, BMI, educational attainment and interval between HPAA and cognitive assessments. Results: In fully adjusted models, increased am:pm cortisol ratio was prospectively associated with better later-life cognitive function years later (0.02 fewer errors per SD increase in am:pm cortisol ratio, p<0.01) and verbal fluency (0.03 SD increase in verbal fluency per SD increase in am:pm ratio, p<0.01). Increasing age, lower educational attainment, diagnosis of hypertension, diagnosis of diabetes and increased BMI were associated with worse cognitive function and poorer verbal fluency. There were no associations between depression and later-life cognition or reverse associations between cognition and later-life cortisol profiles. Conclusions: Loss of diurnal HPAA variation is evident in individuals subsequently experiencing more cognitive impairment. It may serve as an early pre-clinical marker of cognitive decline.
Data from: Vastus lateralis architecture changes during pregnancy - a longitudinal study
While the incidence of falls has been described to increase with pregnancy, the mechanism behind this is unclear. Pregnancy associated changes in lower extremity muscle strength could be a possible factor influencing injury risk. Thus, the aim of this longitudinal study was to investigate muscle strength and architectural properties of the lower limbs in different stages of pregnancy and postpartum. In nineteen pregnant women (30±4years) and fifteen non-pregnant controls (28±4years) muscle strength and architectural properties of the vastus lateralis muscle were assessed combining dynamometry, ultrasound, kinematic and electromyographic measurements. Body mass and body composition were determined using bioimpedance analysis. In the pregnant women, the measurements were conducted in the 16±4th (EP) and 29±4th week of pregnancy (LP) as well as in the 32±9th week postpartum (PP). Muscle thickness and pennation angle of the fascicles significantly increased at LP, while muscle strength remained constant during and after pregnancy. Body mass, skeletal muscle mass, fat mass, intracellular and extracellular water also peaked at LP. Postpartum values did not differ from the controls. Changes in the muscle properties were not related to changes in body mass and body composition. Conditions during pregnancy promote changes in the vastus lateralis architecture indicating muscle hypertrophy. However, pregnancy did not increase muscle strength while body mass progressively increases. Therefore, in the event of balance perturbations pregnant women may not be able to meet the requirements for the increased physical demand.
Data from: Ectopic fat obesity presents the greatest risk for incident type 2 diabetes: a population-based longitudinal study
Objectives: Obesity is a risk factor for type 2 diabetes mellitus. Among obesity, visceral fat obesity, and ectopic fat obesity, it has been unclear which has the greatest effect on incident diabetes. Methods: In this historical cohort study of 8430 men and 7034 women, we investigated the effect of obesity phenotypes on incident diabetes. Obesity, visceral fat obesity, and ectopic fat obesity were defined as body mass index ≥25 kg/m2, waist circumference ≥90 cm in men or ≥80 cm in women, and having fatty liver diagnosed by abdominal ultrasonography, respectively. We divided the participants into eight groups according to the presence or absence of the three obesity phenotypes. Results: During the median 5.8 years follow-up for men and 5.1 years follow-up for women, 286 men and 87 women developed diabetes. Compared to the non-obese group, the hazard ratios (HRs) of incident diabetes in the only-obesity, only-visceral fat obesity, only-ectopic fat obesity groups, and with all-three types of obesity group were 1.85 (95%CI 1.06–3.26, p = 0.05) in men and 1.79 (0.24–13.21, p = 0.60) in women, 3.41 (2.51–4.64, p < 0.001) in men and 2.30 (0.87–6.05, p = 0.12) in women, 4.74 (1.91–11.70, p < 0.001) in men and 13.99 (7.23–27.09, p < 0.001) in women and 10.5 (8.02–13.8, p < 0.001) in men and 30.0 (18.0–50.0, p < 0.001) in women. Moreover, the risk of incident diabetes of the groups with ectopic fat obesity were almost higher than that of the four groups without ectopic fat obesity. Conclusion: Ectopic fat obesity presented the greatest risk of incident type 2 diabetes.
