<p><strong><span lang="EN-US">Background:</span></strong><span lang="EN-US"> This study aims to identify risk factors for the main complications following a bolt-connected external ventricular drain (EVD) insertion.</span></p> <p><strong><span lang="EN-US">Methods:</span></strong><span lang="EN-US"> Single-center cohort study. All patients who underwent bolt-connected EVD placement between March 2015 and February 2024 were included. The complications analyzed were infection, hemorrhage, misplacement, obstruction, and accidental pull-out. The need for EVD replacement was also registered. Univariable and Cox regression multivariable analyses were performed.</span></p>
Pregnancy-induced hypertension and its associated risk factors among pregnant garment workers attending an urban health center in Bangladesh
<p><strong><span>ABSTRACT</span></strong></p> <p><strong><span>Background: </span></strong><span>Pregnancy-induced hypertension (PIH) can lead to severe morbidity, long-term disability, and mortality among mothers and their infants<strong>. </strong>PIH is increasing worldwide, disproportionately affecting more mothers in developing countries. <span> </span><span>This study aims to determine the prevalence of PIH and its associated risk factors among pregnant garment workers attending an urban health center in Bangladesh</span>.</span></p> <p><strong><span>Methods:</span></strong><span> This cross-sectional study was conducted among pregnant garment workers at ≥20 weeks of gestation, who visited a health center of a garment in Narayanganj, Bangladesh between July and October 2023. <span>Data on socio-demographics, clinical characteristics including body weight and blood pressure, and family history of chronic diseases were gathered. </span>Logistic regression was done to calculate crude and adjusted odds ratios (OR) to identify factors associated with PIH.</span></p> <p><strong><span>Result:</span></strong><span><span> </span><span>With an average age of 27.3 years (standard deviation 3.2) and nearly two-thirds of the 150 participants having completed secondary education or higher, the mean gestational age was 21.7 weeks. One out of ten had a family history of high blood pressure and 2% had a family history of PIH. The overall prevalence of PIH was 7.3%. Higher risk for PIH was linked to family history of PIH (aOR 18.1; 95% CI 1.3, 251.3). </span></span></p> <p><strong><span>Conclusion:</span></strong><span> <span>The study identified that one out of every 14 pregnant garment workers had PIH. The findings highlight that a family history of PIH significantly increases the likelihood of developing PIH in this population. Therefore, pregnant women with this risk factor should be prioritised by health care professionals. </span></span></p>

Data from: A study on genetic variants of Fibroblast Growth Factor Receptor 2 (FGFR2) and the risk of breast cancer from North India
Genome-Wide Association Studies (GWAS) have identified Fibroblast growth factor receptor 2 (FGFR2) as a candidate gene for breast cancer with single nucleotide polymorphisms (SNPs) located in intron 2 region as the susceptibility loci strongly associated with the risk. However, replicate studies have often failed to extrapolate the association to diverse ethnic regions. This hints towards the existing heterogeneity among different populations, arising due to differential linkage disequilibrium (LD) structures and frequencies of SNPs within the associated regions of the genome. It is therefore important to revisit the previously linked candidates in varied population groups to unravel the extent of heterogeneity. In an attempt to investigate the role of FGFR2 polymorphisms in susceptibility to the risk of breast cancer among North Indian women, we genotyped rs2981582, rs1219648, rs2981578 and rs7895676 polymorphisms in 368 breast cancer patients and 484 healthy controls by Polymerase chain reaction-Restriction fragment length polymorphism (PCR-RFLP) assay. We observed a statistically significant association with breast cancer risk for all the four genetic variants (P<0.05). In per-allele model for rs2981582, rs1219648, rs7895676 and in dominant model for rs2981578, association remained significant after bonferroni correction (P<0.0125). On performing stratified analysis, significant correlations with various clinicopathological as well as environmental and lifestyle characteristics were observed. It was evident that rs1219648 and rs2981578 interacted with exogenous hormone use and advanced clinical stage III (after Bonferroni correction, P<0.000694), respectively. Furthermore, combined analysis on these four loci revealed that compared to women with 0–1 risk loci, those with 2–4 risk loci had increased risk (OR = 1.645, 95%CI = 1.152–2.347, P = 0.006). In haplotype analysis, for rs2981578, rs2981582 and rs1219648, risk haplotype (GTG) was associated with a significantly increased risk compared to the common (ACA) haplotype (OR = 1.365, 95% CI = 1.086–1.717, P = 0.008). Our results suggest that intron 2 SNPs of FGFR2 may contribute to genetic susceptibility of breast cancer in North India population.
Data from: Risk factors associated to a high Mycobacterium tuberculosis complex seroprevalence in wild boar (Sus scrofa) from a low bovine tuberculosis prevalence area
<p>Animal tuberculosis is a worldwide zoonotic disease caused principally by <i>Mycobacterium bovis,</i> a member of the <i>Mycobacterium tuberculosis complex </i>(MTC). In southern Iberian peninsula, wild reservoirs such as the wild boar, among other factors, have prevented the eradication of bovine tuberculosis. However, most of the studies have been focused on south-central Spain, where the prevalence of tuberculosis is high among wild ungulates and cattle herds. In northern regions, where wild boar density and bovine tuberculosis prevalence are lower, fewer studies have been carried out and the role of this species is still under debate. The aim of this study was to describe the temporal and spatial distribution of antibodies against MTC in wild boar from the Basque Country, northern Spain. Sera from 1902 animals were collected between 2010 and 2016. The seroprevalence was determined with an in house enzyme-linked immunosorbent assay and the search of risk factors was assessed by Generalized Linear Models. Overall, 17% of wild boars (326/1902; 95%CI, [15.5%–18.9%]) showed antibodies against MTC. Risk factors associated with seropositivity were the year and location of sampling, the number of MTC positive cattle, the distance to positive farms and the percentage of shrub cover. Age was associated with increased antibody titres among seropositive individuals. The seroprevalence detected was higher than those previously reported in neighbouring regions. Hence, further studies are needed to better understand the role of wild boar in the epidemiology of tuberculosis in low tuberculosis prevalence areas and consequently, its relevance when developing control strategies.</p>
Supporting data for: Risk factors common to leading eye health conditions and major non-communicable diseases: A rapid review and commentary
<p><strong><span>Background</span></strong><span>: </span><span>To gain an understanding of the intersection of risk factors between the most prevalent eye health conditions that are associated with vision impairment and non-communicable diseases (NCDs).</span></p> <p><span><strong>Methods</strong>:</span><span> A</span><span> series of rapid reviews of reviews reporting on non-modifiable risk factors, age and sex, and modifiable risk factors, including social determinants, were conducted for five common eye health conditions that are the leading causes of vision impairment globally (refractive error including uncorrected refractive error, cataract, age-related macular degeneration (AMD), glaucoma, and diabetic retinopathy) and five prevalent NCDs (cancer, cardiovascular disease, chronic respiratory disease, dementia, and depressive disorders). </span></p> <p><span><strong>Results</strong>: </span><span>Eye health conditions and NCDs share many risk factors. Increased age was found to be the most common shared risk factor, associated with increased risks of AMD, cataract, diabetic retinopathy, glaucoma, refractive error, cancer, cardiovascular disease, chronic obstructive pulmonary disease, and dementia. Other shared risk factors included smoking, obesity, alcohol consumption (mixed results), and physical activity was protective, though limited evidence was found for eye conditions. Social determinants are well documented as risk factors for NCDs.</span></p> <p><span><strong>Conclusion</strong>:</span><span> There is substantial overlap in common established risk factors for the most frequent vision-impairing eye conditions and leading NCDs. Increasing efforts should be made to integrate preventative and risk reduction interventions to improve health, with the greatest shared benefits for initiatives that aim to reduce smoking, improve diet, and promote physical activity.</span></p>
Identifying neprilysin-inhibiting FDA approved drugs as possible risk factors for Alzheimer's disease
<p>Although Alzheimer’s disease is the most common type of dementia, unfortunately, safe and effective treatments that could slow the progress of this progressive neurodegenerative disease are still lacking. This amplifies the importance of preventing this disease and, thus, identifying its risk factors. It has already been known that various drugs may increase the risk for Alzheimer’s disease. Here, we searched for approved drugs that may increase this risk through inhibiting neprilysin. Neprilysin is the rate-limiting enzyme in the amyloid beta clearance pathway and is even able to catalyze unphysiologically large amount of amyloid beta. Inhibitors of neprilysin may increase the risk of Alzheimer’s disease and exacerbate its progress via increasing the amounts of the accumulated amyloid beta and its secondary products.</p> <p>In this study, after predicting the blood-brain barrier (BBB) penetration of approved drugs and their metabolites, we used various computational tools to identify potential inhibitors of neprilysin among them. These tools included various structure-based approaches like docking with AutoDockZn, PLANTS, FlexX, Fitted, Molegro, and Flare and rescoring with these scoring functions: Convex-PL, GNINA, JamdaScorer, KORP-PL, RF-Score-VS, Rank Score, dG, VScore. The reliability of these methods was assessed by investigating the correlations of their predictions with experimental measurements. We also used these ligand-based approaches: 2D-QSAR modelling and similarity comparison based on feature trees.</p> <p>These <em>in silico</em> tools suggested that several drugs deserve to be further investigated beacuase of their considerable binding affinity to neprilysin and BBB penetration: aripiprazole, etoricoxib, nabumetone, fentiazac, clorotepine, pitolisant, and chloroquine. Other drugs whose BBB penetration were not predicted to be much high yet their chronic or common use may lead to consequential inhibition of neprilysin can be added to this list, like glyburide, lisinopril, cilazapril, fosinopril, prucalopride, ibersartan, valsartan, and montelukast. Further investigations, including <em>in vitro</em>, animal, and human investigations, can help mitigate the huge and rising societal and economic burden of Alzheimer’s disease globally.</p>

Clinical characteristics, risk factors and complications of COVID-19 among critically ill older adults – A case control study
<p><strong>Background</strong>: The older population is often disproportionately and adversely affected during humanitarian emergencies, as has also been seen during the COVID-19 pandemic. Data regarding COVID-19 in older adults is usually over-generalised and does not delve into details of the clinical characteristics in them. This study was conducted to analyse clinical and laboratory characteristics, risk factors, and complications of COVID-19 between older adults who survived and those who did not.</p> <p><strong>Methods</strong>: We conducted a case-control study among older adults(age > 60 years) admitted to the Intensive Care Unit(ICU) during the COVID-19 pandemic. The non-survivors (cases) were matched with age and sex-matched survivors (control) in a ratio of 1: 3. The data regarding socio-demographics, clinical characteristics, complications, treatment, laboratory data, and outcomes were analysed.</p> <p><strong>Results</strong>: The most common signs and symptoms observed were fever (cases vs controls) (68.92 vs. 68.8%), followed by shortness of breath (62.2% Vs. 52.2%), and cough (47.3% Vs. 60.2%). Our analysis found no association between the presence of any of the comorbidities and mortality. At admission, laboratory markers such as LDH(Lactate Dehydrogenase), WBC(White Blood Count), creatinine, CRP(C-Reactive Protein), D-dimer, ferritin, and IL-6(Interleukin-6) were found to be significantly higher among the cases than among the controls. Complications such as development of seizure, bacteremia, acute renal injury, respiratory failure, and septic shock were seen to have a significant association with non-survivors.</p> <p><strong>Conclusions</strong>: Hypoxia, tachycardia, and tachypnoea at presentation were associated with higher mortality. The older adults in this study mostly presented with the typical clinical features of COVID-19 pneumonia. The presence of comorbid illnesses among them did not affect mortality. Higher death was seen among those with higher levels of CRP, LDH, D-dimer, and ferritin; and with lower lymphocyte counts.</p>
