TCIA/TGCA-LIHC portal venous phase CT-scans for cirrhosis classification

These are the scans provided by The Cancer Genome Atlas Liver Hepatocellular Carcinoma (TCGA-LIHC) data collection, published by The Cancer Imaging Archive here and used in our MICCAI 2023 paper. You can find the GitHub of the paper here.

All the data are submitted to TCIA data usage policy and must be strictly respected by future users.

The selected scans are in portal venous phase, which is the phase that is clinically used to identify cirrhosis. In this list, you will find all the patients available in the TCGA-LIHC database, regardless of the availability of the annotations or the duplicated patients at different dates. In the dataframe provided in the Git repo, you will find the dataframe with one scan per patient (only at the first date), and with the Ishak score for each patient provided in the TCIA/TGCA database, which are the exact ones that we used in the paper. The names of the scans in the dataframe match exactly the ones below, to ease reproducibility.

The scans are already pre-processed and are the exact ones that we use in evaluation phase in the published paper.

Raw Data for the article: Human Amnion-Derived Mesenchymal Stromal Cells: A New Potential Treatment for Carbapenem-Resistant Enterobacterales in Decompensated Cirrhosis

Background: Spontaneous bacterial peritonitis (SBP) is a severe and often fatal infection in patients with decompensated cirrhosis and ascites. The only cure for SBP is antibiotic therapy, but the emerging problem of bacterial resistance requires novel therapeutic strategies. Human amniotic mesenchymal stromal cells (hA-MSCs) possess immunomodulatory and anti-inflammatory properties that can be harnessed as a therapy in such a context.

Methods: An in vitro applications of hA-MSCs in ascitic fluid (AF) of cirrhotic patients, subsequently infected with carbapenem-resistant Enterobacterales, was performed. We evaluated the effects of hA-MSCs on bacterial load, innate immunity factors, and macrophage phenotypic expression.

Results: hA-MSCs added to AF significantly reduce the proliferation of both bacterial strains at 24 h and diversely affect M1 and M2 polarization, C3a complement protein, and ficolin 3 concentrations during the course of infection, in a bacterial strain-dependent fashion.

Conclusion: This study shows the potential usefulness of hA-MSC in treating ascites infected with carbapenem-resistant bacteria and lays the foundation to further investigate antibacterial and anti-inflammatory roles of hA-MSC in in vivo models.

Raw Data for the article: Mortality after transjugular intrahepatic portosystemic shunt in older adult patients with cirrhosis: A validated prediction model

Background and aims: Implantation of a transjugular intrahepatic portosystemic shunt (TIPS) improves survival in patients with cirrhosis with refractory ascites and portal hypertensive bleeding. However, the indication for TIPS in older adult patients (greater than or equal to 70 years) is debated, and a specific prediction model developed in this particular setting is lacking. The aim of this study was to develop and validate a multivariable model for an accurate prediction of mortality in older adults.

Approach and results: We prospectively enrolled 411 consecutive patients observed at four referral centers with de novo TIPS implantation for refractory ascites or secondary prophylaxis of variceal bleeding (derivation cohort) and an external cohort of 415 patients with similar indications for TIPS (validation cohort). Older adult patients in the two cohorts were 99 and 76, respectively. A cause-specific Cox competing risks model was used to predict liver-related mortality, with orthotopic liver transplant and death for extrahepatic causes as competing events. Age, alcoholic etiology, creatinine levels, and international normalized ratio in the overall cohort, and creatinine and sodium levels in older adults were independent risk factors for liver-related death by multivariable analysis.

Conclusions: After TIPS implantation, mortality is increased by aging, but TIPS placement should not be precluded in patients older than 70 years. In older adults, creatinine and sodium levels are useful predictors for decision making. Further efforts to update the prediction model with larger sample size are warranted.

DECISION Masterclass 2022 - Overview of the effects of LMWH in cirrhosis (Agustín Albillos Martínez)

https://decision-for-liver.eu/

The objective of the DECISION project is to enhance the understanding of the pathophysiology of decompensation of #cirrhosis leading to acute-on-chronic liver failure (#ACLF) or death. This consortium will take advantage of already existing large and clinically well-characterized cohorts to ultimately develop prognostic and response tests and combinatorial therapies tailored to the needs of individual patients to decrease the risk of short-term death. In this masterclass, DECISION consortium member Prof. Dr. Agustín Albillos Martínez (Department Head Gastroenterology and Hepatology at "Ramón y Cajal" Hospital & Professor of Medicine at the University of Alcalá), provided an overview of the effects of low molecular weight #heparin (#LMWH) in #cirrhosis. In his lecture, he gave a brief refresher on the mode of action of anti-coagulants, followed by the pathophysiological basis for the long-term use of #anticoagulation in cirrhosis as well as the experience with anticoagulants in the clinic. This project has received funding from the European Union’s Horizon 2020 research and innovation programme under grant agreement No 847949.
