Deterministic evolution and stringent selection during pre-neoplasia
<p>The earliest events during human tumor initiation, while poorly characterized, may hold clues to malignancy detection and prevention. Here we model occult pre-neoplasia by bi-allelically inactivating <em>TP53</em>, a common early event in gastric cancer, in human gastric organoids. Causal relationships between this initiating genetic lesion and resulting phenotypes were established using experimental evolution in multiple clonally derived cultures over two years. <em>TP53</em> loss elicited progressive aneuploidy, including copy number alterations and structural variants prevalent in gastric cancers, with evident preferred orders. Longitudinal single cell sequencing of <em>TP53</em> deficient gastric organoids similarly indicates progression towards malignant transcriptional programs. Moreover, high-throughput lineage tracing with expressed cellular barcodes demonstrates reproducible dynamics whereby initially rare subclones with shared transcriptional programs repeatedly attain clonal dominance. This powerful platform for experimental evolution exposes stringent selection, clonal interference, and a striking degree of phenotypic convergence in pre-malignant epithelial organoids. These data imply predictability in the earliest stages of tumorigenesis and reveal evolutionary constraints and barriers to malignant transformation with implications for earlier detection and interception of aggressive, genome instable tumors.</p>

MORBIMORTALIDADE POR NEOPLASIA DE ESÔFAGO NO ESTADO DO PIAUÍ: ANÁLISE DOS ANOS DE 2018 – 2022
<p>INTRODUÇÃO: O câncer de esôfago é uma condição maligna do revestimento interno do esôfago e tem como principais fatores de risco: idade, tabagismo, etilismo, ingesta de vegetais em conserva e de bebidas aquecidas. Destaca-se também os dois principais tipos histológicos Carcinoma Escamoso (CEC) e o Adenocarcinoma. METODOLOGIA: Trata-se de um estudo epidemiológico retrospectivo, de caráter descritivo e natureza quantitativa. Para essa finalidade, foram utilizados os dados disponibilizados pelo Sistema de Informações sobre Morbidade Hospitalar do SUS (SIH/SUS) pertencentes aos casos de Mortalidade Hospitalar por neoplasia de esôfago no estado do Piauí entre os anos de 2018 e 2022. RESULTADOS: Entre os anos de 2018 a 2022, foram internados 939 pacientes, dos quais 464 (49,41%) foram internações eletivas e 475 (50,58%) como urgência. Em 2019, observa-se o maior número de internações. No entanto, em 2018 os valores de serviços hospitalares associados a essas internações foi o mais alto (271.672,21). A média de permanência dos pacientes nos hospitais variou ao longo dos anos. Os óbitos totalizaram 78 ocorrências. A taxa de mortalidade variou consideravelmente em 2018, foi de 4,94%, aumentando progressivamente para 18,33% em 2022.Quanto à faixa etária, a distribuição variou, com a maioria das internações ocorrendo em pacientes com idades entre 50 e 79 anos. DISCUSSÃO: Nesse estudo foi possível discutir as principais descobertas sobre neoplasia de esôfago no Piauí dos últimos cinco anos. Aborda variações em internações, custos hospitalares, mortalidade e características dos pacientes, destacando a necessidade de uma gestão hospitalar adaptável e eficaz. A crescente taxa de mortalidade ao longo dos anos ressalta a importância de avaliar e aprimorar os protocolos de atendimento. Esses resultados são cruciais para orientar políticas de saúde, alocação de recursos e melhorias na qualidade do atendimento, buscando uma prestação de serviços mais eficaz e equitativa. CONCLUSÃO: Este estudo oferece uma análise detalhada das internações hospitalares de 2018 a 2022, destacando variações significativas nos tipos de internações, custos hospitalares, média de permanência, óbitos e características demográficas dos pacientes.</p>
