The metabolic hormone adiponectin affects the correlation between nutritional status and pneumococcal vaccine response in vulnerable indigenous children

<p class="MsoNoSpacing"><strong><span>Background:</span></strong><span> Almost 200 million children worldwide are either undernourished or overweight</span><span>. </span><span>Only a few studies have addressed the effect of variation in nutritional status on vaccine response</span><span>. </span><span>We previously demonstrated an association between stunting and an increased post-vaccination 13-valent pneumococcal conjugate vaccine (PCV13) response. In this prospective study, we assessed to what extent metabolic hormones may be a modifier in the association between nutritional status and PCV13 response.</span></p> <p class="MsoNoSpacing"><strong><span>Methods: </span></strong><span>Venezuelan children aged 6 weeks to 59 months were vaccinated with a primary series of PCV13. Nutritional status and serum levels of leptin, adiponectin and ghrelin were measured upon vaccination and their combined effect on serum post-vaccination antibody concentrations was assessed by generalized estimating equations multivariable regression analysis.</span></p> <p class="MsoNoSpacing"><strong><span>Results:</span></strong><span> A total of 210 children were included, of whom 80 were stunted, 81 had a normal weight and 49 were overweight. Overweight children had lower post-vaccination antibody concentrations than  normal weight children </span><span>(regression coefficient -1.15, 95% CI -2.22 – -0.072)</span><span>. Additionally, there was a significant adiponectin-nutritional status interaction. In stunted children, higher adiponectin serum concentrations were associated with lower post-PCV13 antibody concentrations </span><span>(regression coefficient -0.19, 95% CI -0.24 – -0.14) </span><span>while the opposite was seen in overweight children </span><span>(regression coefficient  0.14, 95% CI 0.049 – 0.22)</span><span>. </span></p> <p><strong><span>Conclusion:</span></strong><span> Metabolic hormones, in particular adiponectin, may modify the effect of nutritional status on pneumococcal vaccine response. These findings emphasize the importance of further research to better understand the immunometabolic pathways underlying vaccine response and enable a future of optimal personalized vaccination schedules.</span></p>

Comparing the nutritional status of children and adolescents from North Korean defector families and South Korean families

<p>Objective: This study aimed to investigate the nutritional status of children and adolescents from North Korean defector (NKD) families who are currently living in South Korea (SK) and compared with the status of those from SK families.</p> <p>Design: A cross-sectional study comparing the prevalence of malnutrition, overweight, and obesity between children and adolescents from NKD families and SK families.</p> <p>Setting: Children and adolescents from NKD families were interviewed face-to-face directly, whereas the data about those from SK families acquired by using 2017 Korea National Health and Nutrition Examination Survey(KNHANES). Their nutritional status were estimated using the 2017 Korean National Growth Charts for children and adolescents</p> <p>Participants: The total number of children and adolescents was 2,136 consisting of 527 subjects from the NKD families and 1,609 subjects from the SK families.</p> <p>Results: The overall prevalence of stunting, wasting, underweight, overweight, and obesity in NKD group was 8.9%, 10.2%, 10.4%, 11.2%, and 12.2% respectively, and 1.9%, 7.1%, 5.9%, 9.2%, and 9.3%, respectively, in SK families. The NKD group showed significantly higher prevalence than SK group in stunting(p&lt;0.001), wasting(p=0.014), underweight(p&lt;0.001), obesity(p=0.041) but not in overweight.</p> <p>Conclusions: The nutritional status of children and adolescents form NKD families was worse than that of those from SK families, and also higher prevalence of obesity.</p>

Nutritional status among landslide victims in rural Uganda

<p><span>We assessed food insecurity, diet diversity, and the right to adequate food among households in communities in Eastern Uganda that were affected by major landslides in 2010 and 2018. A prospective cohort study was applied to select 422 households during May–August (the food-plenty season) of 2019. In January–March (the food-poor season) of 2020, 388 households were re-assessed. Socio-demographic, food security, diet diversity, and right to adequate food data were collected using structured questionnaires.</span><span> Four focus group discussions and key informant interviews with 10 purposively sampled duty-bearers explored</span><span> issues of food insecurity, diet, and the right to adequate food. The affected households had significantly higher mean (SE) food insecurity scores than controls, both during the food plenty season: 15.3 (0.5) vs. 10.8 (0.5), and during food-poor season: 15.9 (0.4) vs. 12.5 (0.0). The affected households had significantly lower mean (SE) diet diversity scores than controls during the food plenty season: 5.4 (0.2) vs. 7.5 (0.2) and during the food poor season: 5.2 (0.2) vs. 7.3 (0.1). Multivariate analyses showed that the disaster, education, and main source of livelihood, significantly predicted household food security and diet diversity during the food-plenty season whereas during the food-poor season, the disaster and education were predictors. </span><span>During both food seasons, the majority of affected and control households consumed unsafe food. Cash-handout was the most preferred for ensuring the right to adequate food.</span><span> Comprehension and awareness of human rights principles and state obligations were low. </span>The severity of food-insecurity and diet diversity differed significantly between the affected and control households during both food seasons. Moreover, the right to adequate food for landslide victims was not sufficiently realized. There is a need to cater to seasonal variations, disaster effects, and the right to adequate food in order to reduce landslide victims' vulnerability to food insecurity and poor diet diversity.</p>
