Efficacy and safety of subcutaneous vs. sublingual immunotherapy in allergic rhinitis: a systematic review and meta-analysis

<p>Allergic rhinitis significantly impacts patients' quality of life, and allergen immunotherapy (AIT) offers an alternative to conventional treatments. This study compares the efficacy and safety of subcutaneous immunotherapy (SCIT) and sublingual immunotherapy (SLIT) for allergic rhinitis. A comprehensive search of PubMed, Embase, and ClinicalTrials.gov identified nine randomized controlled trials involving 780 patients (427 SCIT, 353 SLIT). The primary outcome was symptom score; secondary outcomes included medication score, symptom medication score, and local and systemic reactions. Results showed SCIT significantly reduced symptom scores compared to SLIT (Pooled SMD: -0.52, 95% CI: -0.60, -0.03, I2 =83%, P&lt;0.05). However, SCIT patients experienced more severe systemic reactions (grade 3&amp;4) than SLIT patients (Pooled SMD: 6.27, 95% CI: 1.47, 26.73, I2 =0%, P=0.01). Other outcomes were comparable between both groups. In conclusion, SCIT is slightly more effective than SLIT but is associated with a higher frequency of severe systemic reactions, guiding clinicians to tailor treatments to individual patient needs.</p>

Supporting DNN Safety Analysis and Retraining through Heatmap-based Unsupervised Learning

<p>This repository provides the data used for the experiments of the paper&nbsp; &quot;Supporting DNN Safety Analysis and Retraining through Heatmap-based Unsupervised Learning&quot; by Hazem Fahmy, Fabrizio Pastore, Mojtaba Bagherzadeh, and Lionel Briand appearing in IEEE Transactions on Reliability (doi: 10.1109/TR.2021.3074750)</p> <p>Deep neural networks (DNNs) are increasingly important in safety-critical systems, for example in their perception layer to analyze images. Unfortunately, there is a lack of methods to ensure the functional safety of DNN-based components.</p> <p>We observe three major challenges with existing practices regarding DNNs in safety-critical systems: (1) scenarios that are underrepresented in the test set may lead to serious safety violation risks, but may, however, remain unnoticed; (2) char- acterizing such high-risk scenarios is critical for safety analysis; (3) retraining DNNs to address these risks is poorly supported when causes of violations are difficult to determine.</p> <p>To address these problems in the context of DNNs analyzing images, we propose HUDD, an approach that automatically supports the identification of root causes for DNN errors. HUDD identifies root causes by applying a clustering algorithm to heatmaps capturing the relevance of every DNN neuron on the DNN outcome. Also, HUDD retrains DNNs with images that are automatically selected based on their relatedness to the identified image clusters.</p> <p>We evaluated HUDD with DNNs from the automotive domain. HUDD was able to identify all the distinct root causes of DNN errors, thus supporting safety analysis. Also, our retraining approach has shown to be more effective at improving DNN accuracy than existing approaches.</p> <p>&nbsp;</p>

Data set and scripts - Influence of Festive Periods on Road Safety: Multidimensional Analysis (Road Accidents in Colombia 2017-2021)

<p>This dataset comprises historical information about road accidents in Colombia from 2017 to 2021, titled 'Road Accidents 2017-2021', containing 18,600 records of accident events on roads managed by the National Roads Institute (INV&Iacute;AS, 2021). The dataset includes 41 descriptors and was last updated on July 15, 2022. It has been published under the Open Data initiative (Law 1712 of 2014 on Transparency and Access to National Public Information).</p> <p>In addition to accident information, the dataset integrates a database with holiday dates and road identifiers, ensuring data coherence and quality for data analysis purposes. Statistical analysis is conducted through exploratory data analysis focusing on the years 2017 to 2021, utilizing Python (version 3.10) within the Jupyter Notebooks execution environment and specialized libraries (Pandas, NumPy, Matplotlib, and Seaborn), due to their ease of application for this dataset. After data normalization, the dataset comprises 18,554 records, with 46 excluded due to inconsistent data formats.</p>

Data from: Evaluation of a pharmacist-led actionable audit and feedback intervention for improving medication safety in primary care: an interrupted time series analysis

<p><strong>Background</strong>. We evaluated the impact of a pharmacist-led Safety Medication dASHboard (SMASH) intervention on medication safety in primary care.<br> <strong>Methods and findings</strong>. SMASH comprised: (1) training of clinical pharmacists to deliver the intervention; (2) a web-based dashboard providing actionable, patient-level feedback; and (3) pharmacists reviewing individual at-risk patients, and initiating remedial actions or advising general practitioners on doing so. It was implemented in forty-three general practices covering a population of 235,595 people in Salford (Greater Manchester), UK. All practices started receiving the intervention between 18 April 2016 and 26 September 2017. We used an interrupted time series analysis of rates of potentially hazardous prescribing and inadequate blood-test monitoring, comparing observed rates post-intervention to extrapolations from a 24-month pre-intervention trend. The number of people registered to participating practices and having one or more risk factors for being exposed to hazardous prescribing or inadequate blood-test monitoring at the start of the intervention was 47,413 (males: 23,073 [48.7%]; mean age: 60 [standard deviation: 21]). At baseline, 95% of practices had rates of potentially hazardous prescribing (composite of 10 indicators) between 0.88% and 6.19%. The prevalence of potentially hazardous prescribing reduced by 27.9% (95% confidence interval [CI], 20.3% to 36.8%) at 24 weeks and by 40.7% (95% CI, 29.1% to 54.2%) at twelve months after introduction of SMASH. The rate of inadequate blood-test monitoring (composite of 2 indicators) reduced by 22.0% (95% CI, 0.2% to 50.7%) at 24 weeks and by 23.5% (95% CI, -4.5% to 61.6%) at 12 months. After 12 months, 95% of practices had rates of potentially hazardous prescribing between 0.74% and 3.02%. We did not randomise practices but enrolled them in a naturalistic fashion. All our measurements were based on routinely kept electronic health records.<br> <strong>Conclusions</strong>. The SMASH intervention was associated with reduced rates of potentially hazardous prescribing and inadequate blood-test monitoring in general practices. This reduction was sustained over 12 months after start of the intervention for prescribing but not for monitoring of medication. There was a marked reduction in the variation in rates of high-risk prescribing between practices.</p>

Such analysis is performed after a system requirements specification is available. Therefore, it is then worthwhile to investigate specification techniques to detect their strengths and weaknesses with respect to discovering hazards early in the development process. <strong>Objective</strong>: In this paper, we investigate similarities and differences in the results of a preliminary safety analysis from requirements specified using models in Business Process Modeling Notation (BPMN) and Textual Use Cases (TUC). <strong>Method</strong>: We adopted a controlled experiment as research method using computer engineering students as subjects. <strong>Results</strong>: The subjects of BPMN group found more accidents, hazards as well as more causes of hazards. Moreover, they have a higher preference for the template used for safety analysis documentation. <strong>Conclusions</strong>: The use of BPMN to represent the interactions among actors in a system probably lead to the discovery of more accidents and hazards, but more experiments are necessary to test this hypothesis since the results are not statistically significant.</p>

Efficacy and safety of semaglutide for type 2 diabetes by race and ethnicity: a post hoc analysis of the SUSTAIN trials

<p class="CxSpFirst"><b>Context:</b> Variations in the prevalence and aetiology of type 2 diabetes (T2D) across race and ethnicity may affect treatment responses. Semaglutide is a glucagon-like peptide-1 (GLP-1) analog approved for once–weekly, subcutaneous (s.c.) treatment of T2D.</p> <p class="CxSpFirst"><b>Objective:</b> To compare semaglutide efficacy and safety in race and ethnicity subgroups across the SUSTAIN trials.</p> <p class="CxSpMiddle"><b>Design:</b> <i>Post hoc </i>analysis of data from phase 3 randomized SUSTAIN 1–5 and 7 (pooled) and SUSTAIN 6 trials.</p> <p class="CxSpMiddle"><b>Participants:</b> 3,074 subjects (SUSTAIN 1–5 and 7); 1,648 subjects (SUSTAIN 6).</p> <p class="CxSpMiddle"><b>Interventions:</b> Semaglutide 0.5 or 1.0 mg, placebo or active comparator (sitagliptin 100 mg, exenatide extended release 2.0 mg, insulin glargine 100IU/ml and dulaglutide 0.75 or 1.5 mg).</p> <p class="CxSpFirst"><b>Main Outcome Measures:</b> Change in HbA<sub>1c</sub> and body weight from baseline to week 30/40/104, other efficacy and safety endpoints.</p> <p class="CxSpMiddle"><b>Results:</b> HbA<sub>1c</sub> was reduced from baseline by 1.0–1.5%-points and 1.3–2.0%-points, and body weight by 2.3–4.7 kg and 3.6–6.1 kg with semaglutide 0.5 and 1.0 mg, respectively, across race and ethnicity subgroups. Minor changes in blood pressure and lipid profiles were observed. Adverse events (AEs) were reported in similar proportions of subjects across trials. More Asian vs other race subgroups discontinued treatment prematurely due to AEs. The most commonly reported AEs were gastrointestinal disorders.</p> <p class="CxSpMiddle"><b>Conclusion(s):</b> In this SUSTAIN trials <i>post hoc</i> analysis, semaglutide was associated with consistent and clinically relevant reductions in HbA<sub>1c</sub> and body weight in subjects with T2D, with minor variations in efficacy and safety outcomes associated with race or ethnicity.</p>

<p>This repository includes the outcomes of the GEOLAB Transnational Access project SAFETY, carried out at Deltares' geotechnical centrifuge facilities and coordinated by University of Porto. The main goal of SAFETY (Stability Analysis For Enhanced Tailings storage facilities) is to evaluate and enhance the stability of tailings storage facilities (TSF) under adverse conditions. These facilities, used to store the fine waste material (tailings) from mining processes, are prone to instability issues such as liquefaction, which can lead to catastrophic failures. The project focuses specifically on analyzing the stability of dry stack embankments&mdash;a more sustainable and potentially safer method of tailings disposal. The project has attempted to simulate the behavior of tailings dams through a series of centrifuge tests.</p> <p>&nbsp;</p> <p>&nbsp;</p>
