Awareness of fifth metatarsal stress fractures among soccer coaches in Japan: A cross-sectional study

Although a fifth metatarsal stress fracture is the most frequent stress fracture in soccer players, awareness of fifth metatarsal stress fractures among soccer coaches is unclear. Therefore, we performed an online survey of soccer coaches affiliated with the Japan Football Association to assess their awareness of fifth metatarsal stress fractures. A total of 150 soccer coaches were invited for an original online survey. Data on participants' age, sex, types of coaching licence, coaching category, types of training surface, awareness of fifth metatarsal stress fractures, and measures employed to prevent fifth metatarsal stress fractures were collected using the survey. Data from 117 coaches were analysed. Eighty-seven of the 117 coaches were aware of fifth metatarsal stress fractures; however, only 30% reported awareness of preventive and treatment measures for fifth metatarsal stress fractures. Licensed coaches (i.e., licensed higher than level C) were also more likely to be aware of fifth metatarsal stress fractures than unlicensed coaches were. Furthermore, although playing on artificial turf is an established risk factor for numerous sports injuries, soccer coaches who usually trained on artificial turf were more likely to be unaware of the risks associated with fifth metatarsal stress fractures than coaches who trained on other surfaces were (e.g., clay fields). Soccer coaches in the study population were generally aware of fifth metatarsal stress fractures; however, most were unaware of specific treatment or preventive training strategies for fifth metatarsal stress fractures. Additionally, coaches who practised on artificial turf were not well educated on fifth metatarsal stress fractures. Our findings suggest the need for increased awareness of fifth metatarsal stress fractures and improved education of soccer coaches regarding injury prevention strategies.

Relating hydro-mechanical and elastodynamic properties of dynamically-stressed tensile-fractured rock in relation to fracture aperture and contact area

We exploit nonlinear elastodynamic properties of fractured rock to probe the micro-scale mechanics of fractures and understand the relation between fluid transport and fracture aperture and area, stiffness proxy, under dynamic stressing. Experiments are conducted on rough, tensile-fractured Westerly granite specimen subject to triaxial stresses. Fracture permeability is measured from steady-state fluid flow with deionized water. Pore pressure oscillations are applied at amplitudes ranging from 0.2 to 1~MPa at 1~Hz frequency. During dynamic stressing we transmit acoustic signals through the fracture using an array of piezoelectric transducers (PZTs) to monitor the evolution of fracture interface properties. We examine the influence of fracture aperture and contact area by conducting measurements at effective normal stresses of 10, 12.5, 15, 17.5, and 20~MPa. Additionally, the evolution of contact area with stress is characterized using pressure sensitive film. These experiments are conducted separately with the same fracture and they map contact area at stresses from 9 to 21~MPa. The resulting `true' area of contact measurements made for the entire fracture surface and within the calculated PZT sensor footprints, numerical modeling of Fresnel zone. We compare the elastodynamic response of the the fracture using the stress-induced changes ultrasonic wave velocities for a range of transmitter-receiver pairs to image spatial variations in contact properties, which is informed by fracture contact area measurements. These measurements of the nonlinear elasticity are related to the fluid-flow, permeability, in response to dynamic stressing and similar comparisons are made for the slow-dynamics, recovery, of the fracture interface following the stress perturbations.

The stress- and fluid-pressure-driven frictional slip data for variably roughened fractures in Gonghe granite

We explore the impacts of stress- and fluid-pressure-driven frictional slip on variably roughened fractures in Gonghe granite (Qinghai province, China). Slip is on an inclined fracture under simple triaxial stresses with concurrent fluid throughflow allowing fault permeability to be measured both pre- and post-reactivation. Under stress-drive, smooth faults are first slip-weakening and transition to slip-strengthening with rough faults slip-strengthening, alone. A friction criterion accommodating a change in friction coefficient and fault angle is able to fit the data of stable-slip and stick-slip. Under fluid-pressure-drive, excess pore pressures must be significantly larger than average pore pressures suggested by the stress-drive-derived failure criterion. This overpressure is conditioned by the heterogeneity of the pore pressure distribution in radial flow on the fault and related to the change in permeability. Fault roughness impacts both the coefficient of friction and the permeability and therefore exerts important controls in fluid-injection-induced earthquakes. The results potentially improve our ability to assess and mitigate the risk of injection-induced earthquakes in EGS.

Imaging elastodynamic and hydraulic properties of in-situ fractured rock: An experimental investigation exploring effects of dynamic stressing and shearing

We describe laboratory experiments to elucidate the relationship between nonlinear elasticity and permeability evolution in fractured media subjected to local stress perturbations. This study is part of an effort to image fluid pathways and fracture properties using active-source acoustic monitoring during fluid injection and shear of rough fractures. Experiments were conducted with L-shaped samples of Westerly granite fractured in-situ under tri-axial conditions with deionized water subsequently circulated through the resulting fractures. After in-situ fracturing, we separately imposed oscillations of the applied normal stress and pore pressure with amplitudes ranging from 0.2 to 1 MPa and frequencies from 0.1 to 40 Hz. During these dynamic perturbations an array of piezoelectric transducers continuously transmitted ultrasonic pulses across the fracture to monitor the evolving elastic response. We interpret the resulting evolution of elastic wave properties in the context of elastic nonlinearity and relate the estimated nonlinearity parameters to the relative change in permeability of the fractured media. Fracture roughness is then altered in-situ by shearing, with the generation of breccia and wear products. We document the evolution of permeability and fracture contact stiffness as a function of dynamic stressing and shear offset and discuss our findings in relation to fractures in Earth's crust.

Comparison of planar digital radiography and helical standing computed tomography for assessment of condylar stress fracture risk in Thoroughbred racehorses

Background: Catastrophic injury has a low incidence but leads to the death of many Thoroughbred racehorses.

Objectives: To determine sensitivity, specificity, and reliability for condylar stress fracture risk assessment from fetlock digital radiographs (DR) and standing computed tomography (sCT).

Study design: Controlled ex vivo experiment.

Methods: A blinded set of thoracic limb fetlock DR and sCT images were prepared from 31 Thoroughbreds. Four observers evaluated the condyles and parasagittal grooves (PSG) of the third metacarpal bone for the extent of dense bone and lucency/fissure and assigned a risk assessment grade for condylar stress fracture. Sensitivity and specificity for detection of subchondral structural changes in the condyles and PSG and for risk assessment for condylar stress fracture were determined by comparison with a reference. Agreement between observers and the reference assessment and reliability between observers were determined. Intra-observer repeatability was also assessed.

Results: Sensitivity for detection of structural change was lower than specificity for both imaging methods and all observers. For horses with normal risk, observer assessment often agreed with the reference. Sensitivity for risk assessment was lower than specificity for all observers. For horses with a high risk of injury, observers generally underestimated risk. Diagnostic sensitivity of risk assessment was improved with sCT imaging, particularly for horses with elevated risk of injury. Assessment repeatability and reliability was better with sCT than DR.

Main limitations: The ex vivo study design influenced DR image sets.

Conclusions: Risk assessment through screening with diagnostic imaging is a promising approach to improve injury prevention in racing Thoroughbreds. Knowledge of sensitivity and specificity of fetlock lesion detection provides the critical guidance needed to improve screening programs for racehorses. We found improved detection of MC3 subchondral structural change and risk assessment for condylar stress fracture with sCT ex vivo.
