Development of a prognostic model for early breast cancer integrating neutrophil to lymphocyte ratio and clinical-pathological characteristics

<p><strong>Abstract</strong></p> <p>Breast cancer-related inflammation is critical in tumorigenesis, cancer progression, and patient prognosis. Several inflammatory markers derived from peripheral blood cells count, such as the neutrophil-lymphocyte ratio (NLR), derived neutrophil-lymphocyte ratio (dNLR), platelet-lymphocyte ratio (PLR), monocyte-lymphocyte ratio (MLR) and systemic immune-inflammation index (SII) are considered as prognostic markers in several types of malignancy. Here, we investigate and validate a prognostic model in early breast cancer (eBC) patients to predict disease-free survival (DFS) based on readily available baseline clinicopathological prognostic factors and preoperative peripheral blood-derived indexes. We analyzed a training cohort of 710 BC patients and two external validation cohorts of 980 and 157 eBC patients, respectively, with different demographic origins. An elevated preoperative NLR is a better DFS predictor than PLR, MLR, and SII in patients with eBC. The prognostic model generated in this study was able to classify patients into three groups with different risks of relapse based on ECOG-PS, presence of comorbidities, T and N stage, PgR status, and NLR. Prognostic models derived from the combination of clinicopathological features and peripheral blood indices, such as NLR, represent attractive markers mainly because they are easily detectable and applicable in daily clinical practice. More comprehensive prospective studies are needed to unveil their actual effectiveness.</p>

Data from: Outcome of breast cancer in Moroccan young women correlated to clinic-pathological features, risk factors and treatment: a comparative study of 716 cases in a single institution

Background: Breast cancer in young women is quite uncommon and shows more aggressive characteristics with major disparities between worldwide populations. Prognosis and outcome of breast cancer in young patients are widely studied, but still no consensus is available. Methods: We retrospectively included 716 cases of breast cancer women diagnosed in 2009 at the National Institute of Oncology of Rabat. Patients were divided into two groups according to their age: women aged ≤40 years (Group 1) and women aged &gt;40 years (Group 2). Data were recorded from patients' medical files and analyzed using SPSS 13.0 software (IBM). Results: Young patients represent 24.9% of all patients with breast cancer. The comparison between the two groups displayed significant differences regarding nulliparity (p = 0.001) and progesterone receptor negativity (p = 0.01). Moreover, more progression (Metastases/Relapse) was registered in young women as compared to older women with breast cancer (p = 0.03). The estimated median follow-up period was 31 months. The 5-years Event-Free Survival (EFS) of patients with local disease was 64.6% in young women and 71.5% in older women with breast cancer (p = 0.04). Multivariate analysis in young women showed that nulliparity (HR: 7.2; 95%CI: 1.16–44.54; p = 0.03), T3 tumors (HR: 17.39; 95%CI: 1.74–173.34; p = 0.01) and negative PgR status (HR: 19.85; 95%CI: 1.07–366.54; p = 0.04) can be considered as risk factors for poorer event free survival while hormone therapy was associated with better EFS (HR: 0.11; 95%CI: 0.00–0.75; p = 0.03). In Group 2, multivariate analysis showed that patients with inflammatory breast cancer, N+ status, absence of radiotherapy, absence of chemotherapy, and absence of hormone therapy are at increased risk of recurrence. Conclusions: In Morocco, breast cancer is more frequent in young women as compared to western countries. Breast cancer in young women is more aggressive and is diagnosed late, leading to an intensive treatment. Moreover, the main factors associated with breast cancer development in young women would be hormonal and reproductive status. Analysis of other genetic biomarkers is needed to explain the high prevalence of breast cancer in young women to improve breast cancer management in Morocco

Disease trajectories in hospitalized COVID-19 patients are predicted by clinical and peripheral blood signatures representing distinct lung pathologies

<p><span>COVID-19 is characterized by a broad range of symptoms and disease trajectories. Understanding the correlation between clinical biomarkers and lung pathology over the course of acute COVID-19 is necessary to understand its diverse pathogenesis and inform more precise and effective treatments. Here, we present an integrated analysis of longitudinal clinical parameters, peripheral blood biomarkers, and lung pathology in COVID-19 patients from the Brazilian Amazon. We identified core clinical and peripheral blood signatures differentiating disease progression between recovered patients from severe disease and fatal cases. Signatures were heterogenous among fatal cases yet clustered into two patient groups: &ldquo;early death&rdquo; (&lt; 15 days of disease until death) and &ldquo;late death&rdquo; (&gt; 15 days). Progression to early death was characterized systemically and in lung histopathology by rapid, intense endothelial and myeloid activation/chemoattraction and presence of thrombi, associated with SARS-CoV-2<sup>+</sup> macrophages. In contrast, progression to late death was associated with fibrosis, apoptosis and abundant SARS-CoV-2<sup>+</sup> epithelial cells in post-mortem lung, with cytotoxicity, interferon and Th17 signatures only detectable in the peripheral blood 2 weeks into hospitalization. Progression to recovery was associated with higher lymphocyte counts, Th2 and anti-inflammatory-mediated responses. By integrating ante-mortem longitudinal systemic and spatial single-cell lung signatures, we defined an enhanced set of prognostic clinical parameters predicting disease outcome for guiding more precise and optimal treatments.</span><span> Finally, this study represents a major advance in the investigation of acute respiratory infections by integrating serial clinical data and peripheral blood samples with histopathological and </span><span>spatially-resolved single-cell </span><span>analyses of post-mortem lung samples.</span></p>
