Data from: Safety of single low-dose primaquine in glucose-6-phosphate dehydrogenase deficient falciparum-infected African males: two open-label, randomized, safety trials

Background: Primaquine (PQ) actively clears mature Plasmodium falciparum gametocytes but in glucose-6-phosphate dehydrogenase deficient (G6PDd) individuals can cause hemolysis. We assessed the safety of low-dose PQ in combination with artemether-lumefantrine (AL) or dihydroartemisinin-piperaquine (DP) in G6PDd African males with asymptomatic P. falciparum malaria. Methods and findings: In Burkina Faso, G6PDd adult males were randomized to treatment with AL alone (n = 10) or with PQ at 0.25 (n = 20) or 0.40 mg/kg (n = 20) dosage; G6PD-normal males received AL plus 0.25 (n = 10) or 0.40 mg/kg (n = 10) PQ. In The Gambia, G6PDd adult males and boys received DP alone (n = 10) or with 0.25 mg/kg PQ (n = 20); G6PD-normal males received DP plus 0.25 (n = 10) or 0.40 mg/kg (n = 10) PQ. The primary study endpoint was change in hemoglobin concentration during the 28-day follow-up. Cytochrome P-450 isoenzyme 2D6 (CYP2D6) metabolizer status, gametocyte carriage, haptoglobin, lactate dehydrogenase levels and reticulocyte counts were also determined. In Burkina Faso, the mean maximum absolute change in hemoglobin was -2.13 g/dL (95% confidence interval [CI], -2.78, -1.49) in G6PDd individuals randomized to 0.25 PQ mg/kg and -2.29 g/dL (95% CI, -2.79, -1.79) in those receiving 0.40 PQ mg/kg. In The Gambia, the mean maximum absolute change in hemoglobin concentration was -1.83 g/dL (95% CI, -2.19, -1.47) in G6PDd individuals receiving 0.25 PQ mg/kg. After adjustment for baseline concentrations, hemoglobin reductions in G6PDd individuals in Burkina Faso were more pronounced compared to those in G6PD-normal individuals receiving the same PQ doses (P = 0.062 and P = 0.022, respectively). Hemoglobin levels normalized during follow-up. Abnormal haptoglobin and lactate dehydrogenase levels provided additional evidence of mild transient hemolysis post-PQ. Conclusions: Single low-dose PQ in combination with AL and DP was associated with mild and transient reductions in hemoglobin. None of the study participants developed moderate or severe anemia; there were no severe adverse events. This indicates that single low-dose PQ is safe in G6PDd African males when used with artemisinin-based combination therapy.

A randomized clinical trial to compare P. falciparum gametocytaemia and infectivity following blood-stage or mosquito bite induced controlled malaria infection

<p>For malaria elimination efforts, it is important to better understand parasite transmission to mosquitoes and to develop models to allow early clinical evaluation of transmission-blocking interventions. We previously described a Controlled Human Malaria Infection protocol for induction of gametocytemia in malaria naïve volunteers by mosquito bite (CHMI-trans) (Reuling et al., 2018).  Here, we compared gametocyte production and infectivity in the CHMI-trans model after bites of <em>Plasmodium falciparum (Pf)- </em>infected mosquitoes  to that after intravenous administration of  <em>Pf-</em>infected-erythrocytes.  Volunteers received (sub) curative treatments with gametocyte-permissive piperaquine or sulfadoxine-pyrimethamine. Blood-stage inoculation induced considerably higher gametocyte densities compared to mosquito bitesthat was predicted by <em>Pf</em>AP2-G transcripts indicative of gametocyte commitment, and resulted in <em>Pf</em>-positive mosquito infections in 9/12 volunteers versus 0/12 volunteers after mosquito bite inoculation. Current findings firmly establish the CHMI-trans with intravenous administration of asexual parasites as a model for early clinical evaluation of interventions that aim to interrupt <em>Pf</em>-transmission.</p>

Recent HIV infections among newly diagnosed individuals living with HIV in rural Lesotho: Secondary data from the VIBRA cluster-randomized trial

<p>These are pseudo-anonymised data from a secondary analysis of the VIBRA randomised trial.</p> <p>HIV recency assays are used to distinguish recently acquired infection from long-term infection among individuals newly diagnosed with HIV. Since 2015, the World Health Organisation recommends the use of an algorithm to assess recency of infections which is based on an HIV recency assay and viral load (VL) quantification. We determined the proportion of recent HIV infections among participants of the VIBRA (Village-Based Refill of Antiretroviral therapy) cluster-randomized trial in Lesotho and assessed risk factors for these recent infections.</p> <p>The VIBRA trial recruited individuals living with HIV and not taking antiretroviral therapy during a door-to-door HIV testing campaign in two rural districts (Butha-Buthe and Mokhotlong). Samples were collected from participants newly diagnosed and tested for HIV recency using the Asant&eacute; HIV-1 Rapid Recency Assay and VL using the Roche Cobas System. Clinical and socio-demographic data were extracted from the trial database. Univariate analysis was conducted to determine factors associated with recent compared to long-term infection.</p> <p>There is one dataset containing all data presented in this dtuy. The data codebook explains the data available in the dataset.</p>

Growth, mortality and infection success data from Procambarus virginalis infection trials

<p><span>The dataset STRIVE consists of two files which contain: </span><span>(1) measurements of wet weight (g) and total length (mm) of juvenile marbled crayfish (<em>Procambarus virginalis</em>) used in infection trials performed at the Department of Biology, Faculty of Science, University of Zagreb; and (2) records of juvenile marbled crayfish mortality and detection of crayfish plague pathogen (<em>Aphanomyces astaci</em>) in individuals used in infection trials under (1). Two sets of experiments were performed: Experiment 1 where juvenile marbled crayfish were continuously exposed to two concentrations of <em>A. astaci</em> (7500 and 15000 zoospore/mL) and Experiment 2 where juveniles were exposed to a higher pathogen load (15000 zoospores/mL) but under differing feeding regime (once a week vs. five times a week). In both experiments, all infected groups had respective non-infected control groups. Infections were repeated every two weeks, and individuals were measured and weighed before consecutive infections. Wet weight was measured using a digital scale, while the total length was measured from photographs using the image processing program, ImageJ. All crayfish were tested at the end of experiments for the presence of <em>A. astaci</em> in the crayfish cuticle using PCR-based assay with specific primers which amplify the ITS (Internal Transcribed Spacer) region of the <em>A. astaci</em> 5.8 rRNA gene. </span></p>
