Data from: A meta-analysis of plant interaction networks reveals competitive hierarchies as well as facilitation and intransitivity

The extent to which competitive interactions and niche differentiation structure communities has been highly controversial. To quantify evidence for key features of plant community structure, I recharacterized published data from interaction experiments as networks of competitive and facilitative interactions. I measured the network structure of 31 woody and herbaceous communities, including the intensity, distribution, and diversity of interactions at the species-pair and community level to determine the generality of competition, winner-loser relationships, and unequal interaction allocation. I developed novel methodology using meta-analysis to incorporate interaction uncertainty into estimates of structural metrics among independent networks. Plant communities were competitive, but intraspecific interactions were sometimes more intense than interspecific interactions. On the whole, interactions were imbalanced and communities were transitive. However, facilitation, balanced interactions, and intransitivity were common in individual communities. Synthesizing network metrics using meta-analysis is an original approach with which to generalize community structure in a systematic way.

Data from: Pharmacoeconomic study of anti-influenza virus drugs in Japan based on a network meta-analysis

<p><strong>Objectives:</strong> An analysis was conducted in Japan to determine the most cost-effective neuraminidase inhibitor for the treatment of influenza virus infections from the healthcare payer's standpoint.</p> <p><strong>Methods:</strong> This study reanalyzed the findings of a previous study that had some limitations (no probabilistic sensitivity analysis, quality of life scores measured by the EQ-5D-3L instead of the EQ-5D-5L, and the use of a decision tree model with only three health conditions) by using data from a network meta-analysis study. A decision tree model with eight health conditions was constructed, and costs were identified as medical costs and drug prices (the 2020 version of the Japanese medical fee index). The effectiveness outcomes were measured using EQ-5D-5L questionnaires for adult patients who had previously experienced influenza virus infections. The time horizon was 14 days. Both deterministic and probabilistic sensitivity analyses were performed to examine the robustness of the results.</p> <p><strong>Results:</strong> The base-case cost-effectiveness analysis revealed that oseltamivir outperformed laninamivir, zanamivir, and peramivir, making it the most cost-effective neuraminidase inhibitor. The deterministic and probabilistic sensitivity analyses showed robust results that validated oseltamivir as the most cost-effective among the four neuraminidase inhibitors.</p> <p><strong>Conclusions:</strong> This study thus reconfirmed oseltamivir's position as the most cost-effective neuraminidase inhibitor for the treatment of influenza virus infections in Japan from the standpoint of healthcare payment. These findings can help decision-makers and healthcare providers in Japan, including pharmacists, create and manage formularies.</p>

Efficacy of treatments for polycystic ovarian syndrome management in adolescents: a systematic review and network meta-analysis

<p><span>Limited evidence on treatment options for polycystic ovarian syndrome (PCOS) has led to considerable variation in healthcare practices. We aimed to compare the effects of metformin and/or oral contraceptive pills (OCPs) in combination with pioglitazone, spironolactone, flutamide, and lifestyle interventions among adolescents aged 11-19 years with PCOS.<b> </b>Literature searches were performed in Medline, Embase, and the Cochrane Central Register of Controlled Trials from database inception through December 2018, with no language restriction. Two reviewers screened titles and abstracts, assessed full text eligibility, and extracted information from eligible trials. Evidence was synthesized through network meta-analyses (NMA) using Bayesian random-effects approach. We identified 37 RCTs, in which 2400 patients were randomized. NMA showed no statistically important difference among all interventions to improve menstrual regulation, or body mass index. Moderate quality evidence showed hirsutism scores were reduced by multiple interventions that included single and combination medications namely; lifestyle intervention, metformin, OCP, spironolactone, pioglitazone, metformin-OCP, metformin-spironolactone, metformin-flutamide, against placebo. Moderate quality evidence showed OCP results in more dysglycaemia compared to metformin (odds ratio 2.98; 95%credible interval 1.02, 8.96), no intervention resulted in dysglycaemia reduction. In conclusion,<b> </b>metformin and OCP as monotherapy or in combination with other interventions compared to placebo can reduce hirsutism scores, but none of these medications lead to effective menstrual cycle regulation or weight reduction. However, the use of OCP leads to worse cardiometabolic risk factors. Further research into new treatment options is urgently needed. </span></p>

Dataset of Insulin sensitizers for improving endocrine metabolism in overweight women with PCOS: a network meta-analysis

<p><span><b>Objective: </b>To evaluate the efficacy of insulin sensitizers on menstrual frequency, sex hormone and metabolic parameters in overweight women with PCOS. </span></p> <p><span><b>Methods:</b> We searched multiple databases from inception to September 2019 for RCTs. Network meta-analysis was conducted using multivariate random effects method. </span></p> <p><span><b>Results:</b> 14 trials reporting on 619 women were included. Compared with metformin, metformin + TZDs was more superior in menstrual recovery (WMD 3.68, 95% CrI 1.65 to 8.20), metformin + GLP-1 receptor agonists was more effective in decreasing AND (WMD -2.53, 95% CrI -3.96 to -1.09), both metformin + GLP-1 receptor agonists (WMD 9.22, 95% CrI 5.46 to 12.98) and metformin + TZDs (WMD 4.30, 95% CrI 0.78 to 7.82) were more effective in increasing SHBG, while TZDs was less effective in decreasing BMI (WMD 1.69, 95% CrI 0.72 to 2.66). Compared with GLP-1 receptor agonists, metformin + GLP-1 receptor agonists was associated with higher SHBG (WMD 7.80, 95% CrI 4.75 to 10.85), lower FT (WMD -1.77, 95% CrI -3.25 to -0.29), lower AND (WMD -2.70, 95% CrI -3.91 to -1.50) and lower FG (WMD -0.41, 95% CrI -0.73 to -0.08).</span></p> <p><span><b>Conclusion: </b>For overweight women with PCOS, both metformin combined with GLP-1 receptor agonists and metformin combined with TZDs appear superior to monotherapy in improving hyperandrogenemia. Metformin combined with TZDs could be particularly effective in promoting the recovery of menstruation. Metformin combined with GLP-1 receptor agonists has additional advantage in improving fasting glucose when compared with GLP-1 receptor agonists alone. TZDs is inferior to metformin in decreasing BMI.</span></p>

Data from: Patent foramen ovale closure, antiplatelet therapy or anticoagulation in patients with patent foramen ovale and cryptogenic stroke: a systematic review and network meta-analysis incorporating complementary external evidence

Objective: To examine the relative impact of three management options in patients less than 60 years old with cryptogenic stroke and a patent foramen ovale (PFO): PFO closure plus antiplatelet therapy, antiplatelet therapy alone, and anticoagulation alone. Design: Systematic review and network meta-analysis (NMA) supported by complementary external evidence Data sources: Medline, EMBASE, and Cochrane CENTRAL. Study selection: Randomised controlled trials (RCTs) addressing PFO closure and/or medical therapies in patients with PFO and cryptogenic stroke. Review methods: We conducted an NMA complemented with external evidence and rated certainty of evidence using the GRADE system. Results: Ten RCTs in eight studies proved eligible (n=4416). PFO closure versus antiplatelet therapy probably results in substantial reduction in ischaemic stroke recurrence (risk difference per 1000 patients over 5 years [RD]: -87, 95% credible interval [CrI] -100 to -33; moderate certainty). Compared with anticoagulation, PFO closure may confer little or no difference in ischaemic stroke recurrence (low certainty) but probably has a lower risk of major bleeding (RD -20, 95% Crl -27 to -2, moderate certainty). Relative to either medical therapy, PFO closure probably increases the risk of persistent atrial fibrillation (RD 18, CI +5 to +56, moderate certainty) and device-related adverse events (RD +36, 95% CI +23 to +50, high certainty). Anticoagulation, compared to antiplatelet therapy, may reduce the risk of ischaemic stroke recurrence (RD -71, 95% CrI -100 to +17, low certainty), but probably increases the risk of major bleeding (RD +12, CrI -5 to +65, moderate certainty). Conclusions: In patients less than 60 years old, PFO closure probably confers an important reduction in ischaemic stroke recurrence compared to antiplatelet therapy alone but may make no difference compared to anticoagulation. PFO closure incurs a risk of persistent atrial fibrillation and device-related adverse events. Compared to alternatives, anticoagulation probably increases major bleeding.
