Subjective sleep quality and sleep architecture in patients with migraine: a meta-analysis

<p>Objectives: Sleep disturbance is often associated with migraine. However, there is a paucity of research investigating objective and subjective measures of sleep in migraine patients. This meta-analysis aims to determine whether there are differences in subjective sleep quality measured using the Pittsburgh Sleep Quality Index (PSQI), and objective sleep physiology measured using polysomnography between adult and pediatric patients, and healthy controls.</p> <p>Methods: This review was pre-registered on PROSPERO (CRD42020209325). A systematic search of five databases (Embase, MEDLINE®, Global Health, APA PsycINFO, APA PsycArticles, last searched: 12/17/2020) was conducted to find case-controlled studies which measured polysomnography and/or PSQI in patients with migraine. Pregnant participants and those with other headache disorders were excluded. Effect sizes (Hedges' <i>g</i>) were entered into a random effects model meta-analysis. Study quality was evaluated with the Newcastle Ottawa Scale, and publication bias with Egger's regression test.</p> <p>Results: 32 studies were eligible, of which 21 measured PSQI and/MIDAS in adults, 6 measured PSG in adults and 5 in children. The overall mean study quality score was 5/9, and this did not moderate any of the results, and there was no risk of publication bias. Overall, adults with migraine had higher PSQI scores than healthy controls (<i>g</i>=0.75, <i>p</i> &lt; .001, 95% confidence interval [95%CI]: 0.54 - 0.96). This effect was larger in those with chronic rather than episodic condition (<i>g</i>=1.03, <i>p </i>&lt; .001, 95%CI: 0.37 - 1.01, <i>g </i>= 0.63, <i>p </i>&lt; .001, 95%CI: 0.38 - 0.88 respectively). For polysomnographic studies, adults and children with migraine displayed a lower percentage of REM sleep (<i>g</i>=-0.22, <i>p</i> = 0.017, 95%CI: -0.41 - -0.04, <i>g</i> = -0.71, <i>p</i> = 0.025, 95%CI: -1.34 - -0.10 respectively) than controls. Pediatric patients displayed less total sleep time (<i>g</i>=-1.37, <i>p </i>= 0.039, 95%CI: -2.66 - -0.10), more wake (<i>g</i>=0.52, <i>p </i>&lt; .001, 95%CI: 0.08 – 0.79) and shorter sleep onset latency (<i>g</i>=-0.37, <i>p </i>&lt; .001, 95%CI: -0.54 - -0.21) than controls.</p> <p>Discussion: People with migraine have significantly poorer subjective sleep quality and altered sleep physiology compared to healthy individuals. Further longitudinal empirical studies are required to enhance our understanding of this relationship.</p>

Data from: Association between night-shift work, sleep quality, and health-related quality of life : a cross-sectional study among manufacturing workers in a middle-income setting

<p>Objectives: Night-shift work may adversely affect health. This study aimed to determine the impact of night-shift work on health-related quality of life (HRQoL), and assess whether sleep quality was a mediating factor.</p> <p>Design: Cross-sectional study.</p> <p>Setting: 11 manufacturing factories in Malaysia.</p> <p>Participants: 177 night-shift workers aged 40 to 65 years old were compared with 317 non-night-shift work.</p> <p>Primary and secondary outcomes: Participants completed a self-administered questionnaire on socio-demography and lifestyle factors, short Form-12v2 Health Survey (SF-12), and the Pittsburgh Sleep Quality Index (PSQI). Baron and Kenny's method, Sobel test and multiple mediation model with bootstrapping were used to determine whether PSQI score or its components mediated the association between night-shift work and HRQoL.</p> <p>Results: Night-shift work was associated with sleep impairment and HRQoL. Night-shift workers had significantly lower mean scores in all the eight SF-12 domains (p&lt;0.001). Compared to non-night shift workers, night-shift workers were significantly more likely to report poorer sleep quality, longer sleep latency, shorter sleep duration, sleep disturbances, and daytime dysfunction (p&lt;0.001). Mediation analyses showed that PSQI global score mediated the association between night-shift work and HRQoL. "Subjective sleep quality" (indirect effect=-0.24, standard error [SE]=0.14, bias corrected 95%Confidence Interval [BC 95%CI]: -0.58 to -0.01) and "sleep disturbances" (indirect effect=-0.79, SE=0.22, BC 95%CI: -1.30 to -0.42) were mediators for the association between night-shift work and physical wellbeing, whereas "sleep latency" (indirect effect=-0.51, SE=0.21, BC 95%CI: -1.02 to -0.16) and "daytime dysfunction" (indirect effect=-1.11, SE=0.32, BC 95%CI: -1.86 to -0.58) were mediators with respect to mental wellbeing.</p> <p>Conclusion: Sleep quality partially explains the association between night-shift work and poorer HRQoL. Organisations should treat the sleep quality of night-shift workers as a top priority area for action in order to improve their employees' overall wellbeing.</p>

<p><strong>Objective </strong></p> <p>Explore Sleep duration, sleep quality, and their association with hypertension of petroleum workers.</p> <p><strong>Design</strong></p> <p>Setting and participants Multistage, random-cluster sampling design was used to select study samples, conducted a cross-sectional survey 1,500 prtroleum workers in 6 oil field bases in Karamay, Xinjiang. Unified blood pressure test by occupational physicians at the central hospital. Excluding 53 unfinished questionnaires and 27 unfinished blood pressure tests, 1420 participants were finally included as the subjects.</p> <p><strong>Outcomes</strong></p> <p>Main outcome measures were defined by dividing the referral process into three phases: (1) Ascertainment of hypertension; (2) Subjective sleep status was evaluated by the standard Pittsburgh Sleep Quality Index (PSQI); (3) The logistic regressions analysis were performed to evaluate the associations of sleep duration and sleep quality with hypertension.</p> <p><strong>Results </strong></p> <p>First, short sleep duration(OR=1.64, 95%CI=1.16-2.31) and poor sleep quaity(OR=3.35, 95%CI=2.40-4.67)) exerted a statistically significant effect on hypertension. Second, stratified analysis, (1) stratified by gender, Short sleep duration and poor sleep quality in female and male can increase the risk of hypertension, the OR values of female were 2.05 (95%CI:1.35-3.12) and 5.59 (95%CI:3.67-8.49), respectively. The OR values of male were 2.00 (95%CI:1.20-3.34) and 2.37 (95%CI:1.42-3.95), respectively (2) stratified by age, Among short sleepers, adults aged 30-44 years had higher adjusted odds of hypertension (OR: 2.19, 95% CI: 1.34-3.59) than older adults aged 45-65 years (OR: 2.70, 95% CI:1.60-4.54). Among poor sleepers, the risk of hypertension increasing with age, the OR values were: 3.43(95%CI: 1.52-7.74); 4.06(95%CI: 2.48-6.62); 4.35(95%CI: 2.60-7.27). (3) Stratified by shift work, we observed slightly increased relations of hypertension prevalence with the extension of shift work compared with non-shift workers, the OR (95% CI) were 2.28(1.20-4.35) and 3.86(2.08-7.15) for shift workers with short sleep duration and poor sleep quality, respectively.</p> <p><strong>Conclusion</strong></p> <p>Short sleep duration and poor sleep quality are associated with hypertension prevalence of petroleum workers.</p>
