BRAIN Journal-Prediction of Thyroid Disease Using Data Mining Techniques-Figure 2. Attributes of the classification models used in the experiments

The authors used for their experiments a data set (UCI, 2016) containing 756 records about persons with thyroid dysfunctions. The classification model has 22 attributes; the class attribute is the target and it has three possible values: hypothyroidism, hyperthyroidism and normal. The current data set was extracted and preprocessed from the original file. A description of the attributes used in the experiments is given in Figure 2 (an extract from thyroid.arff test file).

BRAIN Journal-Prediction of Thyroid Disease Using Data Mining Techniques-Figure 3. KNIME Diagram

The proposed KNIME diagram representing the data mining models is given in Figure 3. The nodes that constitute the model diagram are: ARFF Reader – the input node used to load the data set in arff format, Partitioning – the node with the role of data set partition (for training and for the validation of the classification model), Naive Bayes Learner and Decision Tree Learner – the nodes used to build the classification model, Naive Bayes Predictor and Decision Tree Predictor – the nodes used to validate the model, Scorer – the node reports a confusion matrix and the accompanying quality measures in its view, Normalizer – the data set are normalized to be able to apply the neural network models, Multilayer Perceptron and RBFNetwork – the nodes corresponding to the neural network classification models, Weka Predictor – a node implemented in Weka to validate the models.

BRAIN Journal-Prediction of Thyroid Disease Using Data Mining Techniques-Figure 1. Factors that Affect Thyroid Function (The Institute for Functional Medicine, 2014)

In Figure 1 are presented the main factors that affect the thyroid function. It is obvious that factors such as stress, infection, toxins, trauma and certain medication are directly responsible for the improper production of thyroid hormones. Symptoms identification and the early detection of abnormal values of thyroid hormones after clinical investigation will help in establishing the proper diagnostic and to prescribe the right medication. The patient must periodically evaluate his clinical state in order to receive the treatment as long as he needs it.

Gut microbiome in the Graves' disease: comparison before and after anti-thyroid drug treatment

Several studies have explored the link between the gut microbiome and Graves's disease (GD) but lacked data on microbiome changes following anti-thyroid drug (ATD) treatment. Stool samples from 29 newly diagnosed GD patients were analyzed before and after 6 months of ATD treatment. Results showed a significant decrease in microbial diversity in GD patients, which increased after treatment, resembling healthy levels. GD patients had lower levels of Firmicutes and higher levels of Bacteroidota, which normalized post-treatment. Specific microbial changes were associated with hyperthyroid symptoms. The study suggests the gut microbiome's involvement in GD pathogenesis and potential restoration with treatment.

Comparison of different thyroid surgical procedures and their outcomes/complications for benign disease in relation to expertise of the surgeon in a public hospital of a developing country over 2 decades

Objective: To compare the frequency of postoperative complications with thyroid surgical procedures and the expertise of the surgeon.

Design: Retrospective cross-sectional study from 1999 to 2018.

Setting: A public sector tertiary care teaching hospital.

Participants and methods: Patients undergoing thyroid surgery (lobectomy with isthmusectomy, subtotal thyroidectomy (STT), near total thyroidectomy (NTT), or total thyroidectomy (TT)) were included. Expertise level 1, 2 and 3 (L1, L2, L3) of the surgeon was based on years of experience or number of thyroid surgeries to their credit.

Primary and Secondary Outcome Measures: Postoperative complications (hypocalcemia, recurrent laryngeal nerve (RLN) damage, airway obstruction, hemorrhage and mortality) were measured against type of thyroid surgery and expertise of surgeon.

Results: 833 thyroid surgeries were performed on 695 (83.43%) females and 138 (16.57%) males. 502 (60.26%) STT, 228 (27.37%) TT, 61 (7.32%) NTT, 42 (5.04%) lobectomies with isthmusectomies were performed, with LI, 2, and 3 surgeons performing 21.25%, 45.74% and 33.01%, respectively. L1, 2 and 3 surgeons caused 49.47%, 33.45% and 17.08% of adverse events respectively. The permanent hypocalcemia, RLN damage and mortality were significantly (p<0.05) more common in surgeries performed by L1 compared with L2 and L3. Transient and permanent hypocalcemia, transient and permanent RLN damage and mortality were significantly (p<0.01) more common for total thyroidectomy compared to subtotal thyroidectomy.

Conclusion: Minimizing the occurrence of complications like permanent hypocalcemia, RLN damage and mortality, expertise of the surgeon and anticipated difficulty of the procedure needs to be taken into account while selecting a thyroid procedure.

Keywords: Thyroid surgery, Total thyroidectomy, Subtotal thyroidectomy, near total thyroidectomy, Lobectomy with isthmusectomy, hypoparathyroidism, hypocalcemia, recurrent laryngeal nerve, postoperative complications

Unfavorable triglyceride-rich particle profile in subclinical thyroid disease: A cross-sectional analysis of ELSA-Brasil

Introduction: Subclinical thyroid disorders have been associated with atherosclerosis and increased cardiovascular risk. As triglyceride-rich lipoprotein particles (TRLP) have recently emerged as a casual factor for atherogenesis, the aim of this study was to evaluate the relationship between subclinical hypo and hyperthyroidism and TRLP subfractions.

Methods: We selected 5,066 participants from the ELSA-Brasil cohort with available data of thyroid function and lipid profile measured by NMR. Individuals were divided into three groups by baseline thyroid function (subclinical hypothyroidism, euthyroidism, and subclinical hyperthyroidism). TRLP subfractions were analyzed through Nuclear Magnetic Resonance (NMR) spectroscopy. To examine the association between TRLP subfractions and thyroid function, we conducted univariate and multivariate linear regression models adjusted for demographic characteristics, BMI, diabetes, smoking status and alcohol use.

Results: Of 3,304 individuals, 54% were women, with a mean age of 50.6±8,7 years, 51% white and 53% with at least college education. Of these individuals, 92% were euthyroid, whereas 6.8% had subclinical hypothyroidism and 1.2% had subclinical hyperthyroidism. The univariate linear regression showed that Very Small TRLP (p=0.026) and Very Large TRLP (p=0.008) were statistically increased in subclinical hypothyroidism when compared to euthyroidism. In subclinical hyperthyroidism, there was a reduction in total TRLP (p=0.003), seemingly driven by reduced Very Small-TRLP (p=0.067). The findings were confirmed when adjusted for demographic characteristics, as well as comorbidities.

Conclusions: This study suggests that subclinical hypothyroidism is associated to very small and very large TRLP, which are related to an unfavorable atherogenic profile. Subclinical hyperthyroidism is associated to lower very small TRLP.
