Data from: How do patients and health workers interact around malaria rapid diagnostic testing, and how are the tests experienced by patients in practice? A qualitative study in western Uganda

Background: Successful scale-up in the use of malaria rapid diagnostic tests (RDTs) requires that patients accept testing and treatment based on RDT results and that healthcare providers treat according to test results. Patient-provider communication is a key component of quality care, and leads to improved patient satisfaction, higher adherence to treatment and better health outcomes. Voiced or perceived patient expectations are also known to influence treatment decision-making among healthcare providers. While there has been a growth in literature on provider practices around rapid testing for malaria, there has been little analysis of inter-personal communication around the testing process. We investigated how healthcare providers and patients interact and engage throughout the diagnostic and treatment process, and how the testing service is experienced by patients in practice. Methods: This research was conducted alongside a larger study which explored determinants of provider treatment decision-making following negative RDT results in a rural district (Kibaale) in mid-western Uganda, ten months after RDT introduction. Fifty-five patients presenting with fever were observed during routine outpatient visits at 12 low-level public health facilities. Observation captured communication practices relating to test purpose, results, diagnosis and treatment. All observed patients or caregivers were immediately followed up with in-depth interview. Analysis followed the 'framework' approach. A summative approach was also used to analyse observation data. Results: Providers failed to consistently communicate the reasons for carrying out the test, and particularly to RDT-negative patients, a diagnostic outcome or the meaning of test results, also leading to confusion over what the test can detect. Patients appeared to value testing, but were frustrated by the lack of communication on outcomes. RDT-negative patients were dissatisfied by the absence of information on an alternative diagnosis and expressed uncertainty around adequacy of proposed treatment. Conclusions: Poor provider communication practices around the testing process, as well as limited inter-personal exchange between providers and patients, impacted on patients' perceptions of their proposed treatment. Patients have a right to health information and may be more likely to accept and adhere to treatment when they understand their diagnosis and treatment rationale in relation to their perceived health needs and visit expectations.

Percussion test: description and diagnostic accuracy of a new manual test for bone marrow edema of the knee - RAW DATASET

<p>Background</p> <p>Prompt diagnosis of bone marrow lesion (BML) is difficult but critical for correct treatment. Magnetic resonance imaging is the gold standard, although expensive and time consuming. Simple and reliable clinical test for BML detection is lacking. Aim of the study is to describe a new manual clinical test called Percussion Test (PT) and to statistically determine its diagnostic accuracy in BML, compared to MRI imaging.</p> <p>Methods</p> <p>After evaluation of the inclusion and exclusion criteria, 218 consecutive patients with unilateral knee pain and age comprised between 18 and 80&thinsp;years old were enrolled in our observational prospective study. Informed consent was obtained for each patient. After medical history collection, PT was performed by a single operator as described. MRI was performed in the affected knee to detect the presence of BML. Coherence in PT and MRI assessment was evaluated in each quadrant of the knee via contingency tables, as sensitivity, specificity, NPV, PPV and diagnostic accuracy were calculated.</p> <p>Results</p> <p>No correlation with a positive PT was demonstrated for the covariables gender (<em>p</em>&nbsp;=&amp;thinsp;0.156), age (<em>p</em>&nbsp;=&amp;thinsp;0.272) and BMI (<em>p</em>&nbsp;=&amp;thinsp;0.639).</p> <p>PT showed a sensitivity ranging from 60.6 (40.6&ndash;80.6) to 79.5 (63.0&ndash;96.0) and a specificity ranging from 85.7 (80.0&ndash;91.5) to 96.0 (93.1&ndash;98.9) depending on knee quadrant. Diagnostic accuracy ranged from 81.6 (75.9&ndash;86.6) to 89.4 (84.6&ndash;93.2), and&nbsp;<em>p</em>-value was &lt;&thinsp;0.00001 in a chi-squared analysis for all quadrants.</p> <p>Conclusions</p> <p>PT showed sensitivity and specificity values that are comparable with other clinical tests routinely adopted in clinical practice. In the absence of other reliable clinical test, PT has the potential to become a useful bedside tool in the diagnosis and management of BMLs.</p>

This is done on (known) sick or infected and healthy or non-infected persons. In the case of sick persons, the statistical measures of true-positive and false-positive are also obtained. In the case of healthy persons, the statistical measures true-negative and false-negative are also obtained.</p> <p>After the test has been performed, <strong>the positive predictive value</strong> indicates the probability that, if the test result is positive, the person is actually sick or infected (post-test probability).</p> <p>After the test has been carried out, <strong>the negative predictive value</strong> indicates the probability that, in the case of a negative test result, the person is actually healthy or non-infected (post-test probability).</p>
