Factors associated with hypertension and diabetes in rural communities in the Asante Akim North Municipality of Ghana

<p>Hypertension and diabetes are leading non-communicable diseases driving an epidemic of cardiovascular diseases in Ghana. Understanding the factors associated with the occurrence of hypertension and diabetes especially in rural settings is crucial in designing interventions to improve awareness, detection, and control. The study assessed factors associated with hypertension and diabetes in Asante Akim North Municipality, a rural community in the central belt of Ghana.  A cross-sectional study design involving 2,576 participants. Data were collected using a structured questionnaire to cover socio-demographic characteristics, blood pressure measurement, anthropometric measurement, biochemical parameters, and modifiable risk factors of hypertension and diabetes. Descriptive statistics on the outcome were done. Factors associated with hypertension or diabetes were assessed using a multivariable logistic regression model. he study found that participants' mean age was 35 years (SD) with<strong> </strong>a slight female preponderance representing 53.88%. Age, marital status, educational status, and occupation positively predict the occurrence of hypertension. The prevalence of hypertension and diabetes was found to be 30.9% and 17.7% respectively. The Community members above 18 years of age have increased odds of developing hypertension [18-44 years (aOR=1.99, CI=1.06-3.71, p&lt;0.05), 45-64 years (aOR=6.12, CI=3.15-11.9, p&lt;0.001) and &gt;64 years (aOR=14.55, CI=7.17-29.53, p&lt;0.001)]. Community members who were fishing/farming (aOR=0.45, CI=0.26-0.76, p&lt;0.01)<em> </em>and being a student/apprentices <em>(aOR=0.11, CI=0.02-0.56)</em> were at reduced odds of developing diabetes. Participants who take snacks <em>(aOR=</em><em>0.64, CI=0.41-0.99, p&lt;0.05)</em> during working hours had reduced odds of developing diabetes. The study concludes that<strong> c</strong>ommonly known risk factors such as dietary behaviour, alcohol intake, tobacco use, and physical activities were not associated with hypertension or diabetes in rural Ghana. Social demographic characteristics and poor health screening practices were the main predictors of hypertension and diabetes in rural areas. Urgent steps at improving health education and population-level screening would be pivotal in controlling hypertension and diabetes.</p>

Predictors of hypertension among diabetic patients in the Ejisu Municipality of Ghana

<p><strong>Introduction: </strong>The co-existence of hypertension with diabetes mellitus among diabetic patients is a setback to public health. About 40–75% of diabetic patients present with hypertension. The co-existence of hypertension and diabetes can accelerate complications such as stroke, myocardial infarction, nephropathy, and mortality. Available data indicate the devastating effects of hypertension and diabetes on individuals, families, and the economy as catastrophic. Therefore, knowing the predictors of hypertension among diabetic patients would inform the lifestyle and management of the two conditions.</p> <p><strong>Objective:</strong> The study focused on predictors of hypertension among diabetic patients in the Ejisu Municipality of Ghana.</p> <p><strong>Methods: </strong>The study employed a quantitative approach with a sample size of 120. Data were collected on sociodemographic characteristics, family history, 24-hour dietary recall, blood pressure, fasting blood glucose, glycated haemoglobin, total lipid profile, and anthropometrics. Data were analyzed using SPSS version 27.</p> <p><strong>Results:</strong> Out of 120 respondents, 85% were females with 77.5% above 50 years of age. A majority (66.7%) had a family history of diabetes with 76.7% having hypertension as a comorbidity. Fasting blood glucose was found to be 8.519 times more likely to present with hypertension. Systolic blood pressure, carbohydrate, and sodium intakes were 6.1%, 2.9%, and 0.1% respectively. However, diabetic patients with high HbA1c were 97% less likely not to present with hypertension.</p> <p><strong>Conclusion:</strong> Hypertension was found to be the most common comorbidity among diabetic patients in Ghana. Glycaemic control, systolic blood pressure, and dietary factors specifically carbohydrate and sodium intake were significant predictors of hypertension among the study participants.</p>

Differential DNA methylation may provide a mechanistic understanding of these long-term associations. We aimed to quantify the associations of MEA with offspring DNA methylation levels at birth, in childhood and in adolescence. Using 37 studies from high-income countries, we performed meta-analysis of epigenome-wide association studies (EWAS) to quantify the associations of completed years of MEA at the time of pregnancy with offspring DNA methylation levels at birth (n&thinsp;=&thinsp;9 881), in childhood (n&thinsp;=&thinsp;2 017), and adolescence (n&thinsp;=&thinsp;2 740), adjusting for relevant covariates. MEA was found to be associated with DNA methylation at 473 cytosine-phosphate-guanine sites at birth, one in childhood, and four in adolescence. We observed enrichment for findings from previous EWAS on maternal folate, vitamin-B12 concentrations, maternal smoking, and pre-pregnancy BMI. The associations were directionally consistent with MEA being inversely associated with behaviours including smoking and BMI. Our findings form a bridge between socio-economic factors and biology and highlight potential pathways underlying effects of maternal education. The results broaden our understanding of bio-social associations linked to differential DNA methylation in multiple early stages of life. The data generated also offers an important resource to help a more precise understanding of the social determinants of health.</p>

Sleep duration, sleep quality, and their association with hypertension of petroleum workers

<p><strong>Objective </strong></p> <p>Explore Sleep duration, sleep quality, and their association with hypertension of petroleum workers.</p> <p><strong>Design</strong></p> <p>Setting and participants Multistage, random-cluster sampling design was used to select study samples, conducted a cross-sectional survey 1,500 prtroleum workers in 6 oil field bases in Karamay, Xinjiang. Unified blood pressure test by occupational physicians at the central hospital. Excluding 53 unfinished questionnaires and 27 unfinished blood pressure tests, 1420 participants were finally included as the subjects.</p> <p><strong>Outcomes</strong></p> <p>Main outcome measures were defined by dividing the referral process into three phases: (1) Ascertainment of hypertension; (2) Subjective sleep status was evaluated by the standard Pittsburgh Sleep Quality Index (PSQI); (3) The logistic regressions analysis were performed to evaluate the associations of sleep duration and sleep quality with hypertension.</p> <p><strong>Results </strong></p> <p>First, short sleep duration(OR=1.64, 95%CI=1.16-2.31) and poor sleep quaity(OR=3.35, 95%CI=2.40-4.67)) exerted a statistically significant effect on hypertension. Second, stratified analysis, (1) stratified by gender, Short sleep duration and poor sleep quality in female and male can increase the risk of hypertension, the OR values of female were 2.05 (95%CI:1.35-3.12) and 5.59 (95%CI:3.67-8.49), respectively. The OR values of male were 2.00 (95%CI:1.20-3.34) and 2.37 (95%CI:1.42-3.95), respectively (2) stratified by age, Among short sleepers, adults aged 30-44 years had higher adjusted odds of hypertension (OR: 2.19, 95% CI: 1.34-3.59) than older adults aged 45-65 years (OR: 2.70, 95% CI:1.60-4.54). Among poor sleepers, the risk of hypertension increasing with age, the OR values were: 3.43(95%CI: 1.52-7.74); 4.06(95%CI: 2.48-6.62); 4.35(95%CI: 2.60-7.27). (3) Stratified by shift work, we observed slightly increased relations of hypertension prevalence with the extension of shift work compared with non-shift workers, the OR (95% CI) were 2.28(1.20-4.35) and 3.86(2.08-7.15) for shift workers with short sleep duration and poor sleep quality, respectively.</p> <p><strong>Conclusion</strong></p> <p>Short sleep duration and poor sleep quality are associated with hypertension prevalence of petroleum workers.</p>

Effects of the oral angiotensin II type 2 receptor agonist C21 in Sugen-hypoxia induced pulmonary hypertension in rats

<p><span><span>Substantial evidence supports involvement of the renin-angiotensin system in pulmonary hypertension (PH), and the angiotensin II type 2 receptor (AT<sub>2</sub>R) is known to exert tissue-protective actions. The effect of the selective AT<sub>2</sub>R agonist C21 (also known as Compound 21 or buloxibutid) was evaluated in the rat Sugen-hypoxia PH model. After a single injection of Sugen 5416 and hypoxia for 21 days, </span><span>C21</span><span> (2 or 20 mg/kg) or vehicle was administered perorally twice daily from Day 21 to Day 55. On Day 56, hemodynamic assessments were performed, and lung and heart tissue were prepared for quantification of cardiac and vascular remodeling and fibrosis. Treatment with C21 20 mg/kg improved cardiac output and stroke volume and decreased right ventricular hypertrophy (all p&lt;0.05). Treatment with C21 2 mg/kg significantly decreased vessel wall and muscular layer thickness and increased the luminal opening in vessels &gt;100 </span><span>μ</span><span>m (all p&lt;0.05). There were no significant differences between the two C21 doses on any parameter, and <em>post hoc</em> analyses </span><span>comparing the merged C21 groups with the vehicle group showed that C21 treatment reduced vascular remodeling (reduced endothelial proliferation and thickening of the vascular wall) in vessels of all sizes; moreover, the diastolic pulmonary artery pressure and right ventricular pressure were reduced along with reduction of right ventricular hypertrophy. Sugen 5416 and hypoxia increased pulmonary collagen deposition, which was counteracted by C21 20 mg/kg. In conclusion, the effects of C21 on vascular remodeling, hemodynamic alterations, and fibrosis suggest that AT<sub>2</sub>R agonists may have a role in Group 1 and 3 PH treatment.</span><br></span></p>

Lipidomics for diagnosis and prognosis of pulmonary hypertension

<p>Pulmonary hypertension (PH) is associated with high morbidity and mortality with an urgent need for diagnostic and prognostic biomarkers.</p> <p>A training cohort of PH patients, disease controls without PH, and healthy controls was investigated using metabolomics and machine learning. Specific free fatty acid (FFA)/lipid-ratio biomarkers were diagnostic and predictive for PH survival with an area under the curve (AUC) of 0.89. FFA/lipid-ratio performance was independently validated in PH patients from other centers(AUC 0.90). Survival could be predicted in an age-independent manner and a combination with established clinical scores (FPHR4p, COMPERA 2.0) increased the scores hazard risk.</p> <p>Our mechanistic studies in healthy and diseased pulmonary artery endothelial and smooth muscle cells indicate a functional involvement of increased FFA levels in pathophysiology of PH. In conclusion, lipidomic changes in PH can be used as a novel diagnostic and prognostic approach and may help the discovery of new therapeutic targets.</p>

miRNAs in primary open-angle glaucoma and ocular hypertension

<p>Glaucoma is a chronic neurodegenerative disease, which is the leading cause of irreversible blindness worldwide. As a response to high intraocular pressure, the clinical and molecular glaucoma biomarkers indicate the biological state of the visual system. Classical and uncovering novel biomarkers of glaucoma development and progression, follow-up, and monitoring the response to treatment are key objectives to improve vision outcomes. While the glaucoma imaging field has successfully validated biomarkers of disease progression, there is still a considerable need for developing new biomarkers of early glaucoma, that is, at the preclinical and initial glaucoma stages. Outstanding clinical trials and animal-model study designs, innovative technology, and analytical approaches in bioinformatics are essential tools to successfully uncover novel glaucoma biomarkers with a high potential for translation into clinical practice. To better understand the clinical and biochemical–molecular–genetic glaucoma pathogenesis, we conducted an analytical, observational, and case-comparative/control study in 358 primary open-angle glaucoma (POAG) patients and 226 comparative-control individuals to collect tears, aqueous humor, and blood to process for identifying POAG biomarkers by exploring several biological pathways, such as inflammation, neurotransmitter/neurotrophin alteration, oxidative stress, gene expression, miRNAs fingerprint and its biological targets, and vascular endothelial dysfunction. We are incredibly enthusiastic about gathering as much information as possible on POAG biomarkers to learn how this information can be used to better steer the diagnosis and therapy of glaucoma to prevent blindness in the predictable future.</p>

Prospective cohort study of a community-based primary care program's effects on pharmacotherapy quality in low-income Peruvians with type 2 diabetes and hypertension

<p>A door-to-door survey was conducted to enumerate all household members by age and sex in a low-income community in Peru. 856 adults 35 years and older were eligible to participate in screening for type 2 diabetes and hypertension. 709 (83%) participated in screening. 130 (18.3%) were diagnosed with hypertension and/or type 2 diabetes of which 109 (84%) participated at program onset and 22 were added later from earlier non-participants in screening or program onset to form the cohort of 131 patients with diabetes and/or hypertension. The primary care program had components of the Chronic Care Model, community health workers, and freely accessible visits and medications. The program operated between September 2011 and May 2014, and consisted of two care periods (separated by a six-month hiatus), first a 10-month home-care period, then a 17-month clinic-care period. The dataset is two files corresponding to two exposures: the 27-month program overall (post- versus pre-) (N=262 observations, 131 pairs with patients as self-controls) and care period (clinic versus home), N=211 (109 home and 102 clinic observations, &gt;131 because 80 patients participated in both care periods). Exposures were evaluated for their effects on guidelines-based pharmacotherapy standards: hypoglycemic and antihypertensive medications, low-dose aspirin, and first-line angiotensin converting enzyme inhibitor (ACEi) treatment of diabetes with elevated blood pressure.</p>
