The effect of Vitamin D levels on the course of COVID-19 in hospitalized patients – a 1-year prospective cohort study

<p>Background: The aim of the current study was to assess the patients with COVID-19 and the impact of vitamin D supplementation on the course of COVID-19.<br> Methods: This prospective cohort study included patients hospitalized due to COVID-19 between December 2020 and December 2021. Patients&#39; demographic, clinical, and laboratory parameters were analysed.&nbsp;<br> Results: 301 participants were enrolled in the study. 46 (15,3%) had moderate, and 162 (53,8%) had severe COVID-19. 14 (4,7%) patients died, and 30 (10,0%) were admitted to the ICU due to disease worsening. The majority needed oxygen therapy (n=224; 74,4%). Average vitamin 25(OH)D3 levels were below optimal at the admittance, and vitamin D deficiency was detected in 205 individuals. More male patients were suffering from vitamin D deficiency. Patients with the more severe disease showed lower levels of vitamin 25(OH)D3 in their blood. The most severe group of patients had more symptoms that lasted significantly longer with progressing disease severity. This group of patients also suffered from more deaths, ICU admissions, and treatments with dexamethasone, remdesivir, and oxygen.<br> Conclusion: Patients with the severe course of COVID-19 were shown to have increased inflammatory parameters, increased mortality, and higher incidence of vitamin D deficiency. The results suggest that the vitamin D deficiency might represent a significant risk factor for a severe course of COVID-19.</p> <p>&nbsp;</p>

Food composition database for nutrient intake: selected vitamins and minerals in selected European countries

<p>Following a request from the European Commission for a review of European dietary reference values (DRVs), the EFSA&rsquo;s Panel on Dietetic Products, Nutrition and Allergies (NDA) has prepared a number of&nbsp;Scientific Opinions on DRVs for micronutrients. The DATA Unit supported this activity by estimating the nutrient intake of a number of micronutrients in nine selected European countries and different age groups. In addition, the DATA Unit also provided information on average content of food sources of the respective nutrients per country based on the composition database, as well as main food group contributors to nutrient intakes and assessed the comparability of the provided data with pertinent published intake data.</p> <p>Intake estimates have been assessed using food consumption data from the EFSA Comprehensive Food Consumption Database (EFSA, 2011a) and the EFSA Nutrient composition database. Food composition data used to populate the Nutrient composition database were provided to EFSA through the EFSA procurement project &lsquo;<em>Updated food composition database for nutrient intake&rsquo;</em> (Roe at al., 2013). Data were provided following the EFSA specification for standard sample description for food and feed and were classified according to the FoodEx2 classification system of EFSA (EFSA, 2011b).</p> <p>The food composition data used in these assessments and here published cover the following vitamins and minerals: calcium (Ca); copper (Cu); cobalamin (vitamin B12); magnesium (Mg); niacin; phosphorus (P); potassium (K); riboflavin; thiamin; iron (Fe); selenium (Se); vitamin B6; vitamin K, zinc (Zn), and vitamin E<sup>1</sup>. The food composition dataset contains data from seven<sup>2</sup>&nbsp;countries: Finland, France, Germany, Italy, Netherlands, Sweden, and United Kingdom. This dataset version has been checked for outliers but is prior to data completion for missing foods and nutrient values.</p> <p><sup>1</sup>&nbsp;Vitamin E is defined as alpha-tocopherol (AT) only, however as most food composition databases in the EU contain values as alpha-tocopherol equivalents (TE), data on TE are also provided</p> <p><sup>2</sup> For the nutrient intake estimates of Ireland and Latvia present in the opinions of the EFSA Panel on Dietetic Products, Nutrition and Allergies (NDA), food composition data from UK and Germany were respectively used</p>

Data from: An insight into vitamin E and lipid nutrition of the plains-wanderer Pedionomus torquatus

<p>Vitamin E, as α-tocopherol, is an essential antioxidant protecting the body from free radicals. The vitamin E requirement of managed wildlife species is known to be greater than their wild counterparts, predominantly due to higher dietary lipid content and potentially stressful environments. The plains-wanderer (<em>Pedionomus torquatus</em>, Family Pedionomidae [monotypical]) is a critically endangered, superficially quail-like bird that is the focus of an ongoing captive breeding program in Australia. It is estimated that plains-wanderers have a high vitamin E requirement (compared to domestic poultry species) to offset a high lipid diet and their naturally flighty temperament. This study therefore aims to gain a greater understanding of the nutritional status and vitamin E requirements of plains-wanderers in managed environments. Total lipid and α-tocopherol intake were quantified for 26 zoo-managed plains-wanderers over a series of diet intake trials in addition to measurement of plasma α-tocopherol and cholesterol concentrations. Plains-wanderers that consumed higher portions of dietary fat had significantly lower circulating α-tocopherol concentrations than birds that consumed lower total dietary fat (p &lt; 0.001). Additionally, plasma cholesterol concentrations of managed plains-wanderers were found to be significantly greater than all other bird species reviewed, irrespective of Family or feeding type. We also present the first published data quantifying the nutritional makeup of stomach contents of a wild plains-wanderer for use as a potential guide for diet formulation. This study forms a vital foundational insight into the nutritional management of plains-wanderers, but further research is required to understand their dietary habits and cholesterol metabolism.  </p>

Transcobalamin receptor antibodies in autoimmune vitamin B12 central deficiency

<p>Vitamin B12 is critical for hematopoiesis and myelination. Deficiency can cause neurologic deficits including loss of coordination and cognitive decline. However, diagnosis relies on vitamin B12 measurement in the blood which may not accurately reflect levels in the brain. Using programmable phage display, we identified an autoantibody targeting the transcobalamin receptor (CD320) in a patient with progressive tremor, ataxia, and scanning speech. Anti-CD320 impaired cellular uptake of cobalamin (B12) <em>in vitro</em>. Despite normal serum levels, B12 was nearly undetectable in her cerebrospinal fluid (CSF). Immunosuppressive treatment and high-dose systemic B12 supplementation were associated with increased CSF B12 levels and clinical improvement. Optofluidic screening enabled rapid isolation of a patient-derived monoclonal antibody that impaired B12 transport across an <em>in vitro</em> model of the blood-brain barrier. Autoantibodies targeting the same epitope of CD320 were identified in 7 other patients with neurologic deficits of unknown etiology, in 6% of healthy controls, and in 21.4% of a neuropsychiatric lupus cohort. In 132 paired serum and CSF samples, detection of anti-CD320 in the blood predicted B12 deficiency in the brain. However, these individuals did not display any hematologic signs of B12 deficiency despite systemic CD320 impairment. Using a genome-wide CRISPR screen, we discovered that the LDL receptor serves as an alternative B12 uptake pathway in hematopoietic cells. These findings dissect the tissue-specificity of B12 transport and elucidate an autoimmune neurologic condition that may be amenable to immunomodulatory treatment and nutritional supplementation.</p>

The role of rehabilitation and vitamin D supplementation on motor and psychological outcomes in poststroke patients.

<p>Post-Stroke depression affects between 12% and 72% of patients who have suffered a stroke. The association between low serum levels of 25-hydroxyvitamin D (25(OH) D) and increased risk of depression is reported in both stroke and non-stroke patients. Similarly, high 25(OH) D levels might be associated with greater functional improvement during rehabilitation program.</p> <p>We wanted to investigate the effects of an intensive rehabilitation on poststroke outcomes. We wondered if the daily rehabilitation of motor and cognitive functions could also have an effect on mood and functional abilities in addition to or as an alternative to vitamin D supplementation.</p> <p>We conducted a 12-week, randomized trial, double blind, parallel, monocentric clinical trial of 40 patients undergoing intensive neuro-rehabilitation treatment at a specialized care facility for ischemic or hemorrhagic brain stroke. Participants were randomly assigned, in a 1:1 ratio, to 1 of 2 parallel groups: in the experimental group, 2000 IU/day of oral cholecalciferol was administered; in the control group patients were not taking vitamin D supplementation. Patients underwent a text evaluation to investigate psychological and motor outcomes.</p> <p>Significant intra-group difference in outcomes measures was found but not between control group and experimental group. In the vitamin D group, we highlighted significant differences between T0 and T1 in calcium (<em>P</em> &lt; .001), vitamin D (<em>P</em> &lt; .001), in Montgomery Aasberg Depression Rating Scale (<em>P</em> = .001), and in Functional Independent Measures (<em>P</em> &lt; .001). In the health control group, we found a significant difference in calcium (<em>P</em> = .003), vitamin D (<em>P</em> &lt; .001), Montgomery Aasberg Depression Rating Scale (<em>P</em> = 0.006), in general self-efficacy (<em>P</em> = .009), and in Functional Independent Measures (<em>P</em> &lt; .001).</p> <p>Our results show that the beneficial effect on mood and functional recovery is mainly due to neurorehabilitation rather than vitamin D supplementation.</p>
