Data from: Patient characteristics associated with tuberculosis treatment default: a cohort study in a high-incidence area of Lima, Peru
Background: Although tuberculosis (TB) is usually curable with antibiotics, poor adherence to medication can lead to increased transmission, drug resistance, and death. Prior research has shown several factors to be associated with poor adherence, but this problem remains a substantial barrier to global TB control. We studied patients in a high-incidence district of Lima, Peru to identify factors associated with premature termination of treatment (treatment default). Methods: We conducted a prospective cohort study of adult smear-positive TB patients enrolled between January 2010 and December 2011 with no history of TB disease. Descriptive statistics and multivariable logistic regression analyses were performed to determine risk factors associated with treatment default. Results: Of the 1233 patients studied, 127 (10%) defaulted from treatment. Patients who defaulted were more likely to have used illegal drugs (OR = 4.78, 95% CI: 3.05-7.49), have multidrug-resistant TB (OR = 3.04, 95% CI: 1.58-5.85), not have been tested for HIV (OR = 2.30, 95% CI: 1.50-3.54), drink alcohol at least weekly (OR = 2.22, 95% CI: 1.40-3.52), be underweight (OR = 2.08, 95% CI: 1.21-3.56), or not have completed secondary education (OR = 1.55, 95% CI: 1.03-2.33). Conclusions: Our study identified several factors associated with defaulting from treatment, suggesting a complex set of causes that might lead to default. Addressing these factors individually would be difficult, but they might help to identify certain high-risk patients for supplemental intervention prior to treatment interruption. Treatment adherence remains a barrier to successful TB care and reducing the frequency of default is important for both the patients' health and the health of the community.
Data from: Clinical characteristics and lipid lowering treatment of patients initiated on Proprotein convertase subtilisin/kexin type 9 inhibitors – a nationwide cohort study
Objectives: Given the novelty of proprotein convertase subtilisin–kexin type 9 inhibitors (PCSK9i), little is known regarding overall implementation or clinical characteristics among patients who initiate treatment. We aimed to assess the total number of patients initiated on PCSK9i along with a description of the clinical characteristics and lipid lowering treatment (LLT) of such patients. Setting: A register based descriptive cohort study of patients receiving a PCSK9i in the time period from 01-01-2016 to 31-03-2017 using a cross-linkage between three nationwide Danish registers. Information regarding PCSK9i prescriptions, patient demographics, concurrent pharmacotherapy, comorbidities, and previous coronary procedures was identified. Results: Overall, 137 patients initiated treatment with PCSK9i in the study period from 11 in the first quarter of 2016 to 40 in the first quarter of 2017. The majority had a history of ischemic heart disease (67.9%) with ischemic stroke and diabetes mellitus being present in 7.3% and 16.8% of patients, respectively. All patients initiated on PCSK9i had been previous prescribed statin treatment with Atorvastatin and Simvastatin being most frequently prescribed in 53% and 36% of patients, respectively. The majority of patients had received both statins and Ezetimibe (94.9%) and approximately half of these patients had also received bile acid sequestrant (45.3%). Clinical characteristics mainly differed in patients receiving triple LLT compared to patients not receiving triple LLT in the regards of heart failure. Conclusion: Patients treated with PCSK9i were rare, characterized by having ischemic heart disease and had received various and intensive conventional LLT prior to PCSK9i initiation in agreement with current international guidelines.
Microenvironment characteristics and molecular classification in pheochromocytoma patients
<p>Pheochromocytomas (PCCs) are rare neuroendocrine tumors that originate from chromaffin cells in the adrenal gland. However, the cellular molecular characteristics and immune microenvironment of PCCs are incompletely understood. Here, we performed single-cell RNA sequencing (scRNA-seq) on 16 tissues from 4 sporadic unclassified PCC patients and 1 hereditary PCC patient with Von Hippel-Lindau (VHL) syndrome. We found that intra-tumoral heterogeneity was less extensive than the inter-individual heterogeneity of PCCs. Further, the unclassified PCC patients were divided into two types, metabolism-type (marked by NDUFA4L2 and COX4I2) and kinase-type (marked by RET and PNMT), validated by immunohistochemical staining. Trajectory analysis of tumor evolution revealed that metabolism-type PCC cells display phenotype of consistently active metabolism and increased metastasis potential, while kinase-type PCC cells showed decreased epinephrine synthesis and neuron-like phenotypes. Cell-cell communication analysis showed activation of the annexin pathway and a strong inflammation reaction in metabolism-type PCCs and activation of FGF signaling in the kinase-type PCC. Although multispectral immunofluorescence staining showed a lack of CD8<sup>+</sup> T cell infiltration in both metabolism-type and kinase-type PCCs, only the kinase-type PCC exhibited downregulation of <em>HLA-Ⅰ</em> molecules that possibly regulated by <em>RET</em>, suggesting the potential of combined therapy with kinase inhibitors and immunotherapy for kinase-type PCCs; in contrast, the application of immunotherapy to metabolism-type PCCs (with antigen presentation ability) is likely unsuitable. Our study presents a single-cell transcriptomics-based molecular classification and microenvironment characterization of PCCs, providing clues for potential therapeutic strategies to treat PCCs.</p>
Characteristics of COVID-19 patients predicting hospital readmission after ED discharge
<p><strong>Objective:</strong> We aimed at identifying baseline predictive factors for emergency department (ED) readmission, with hospitalization/death, in COVID-19 patients previously discharged from the ED. We also developed a disease progression velocity index.</p> <p><strong>Design and setting:</strong> Retrospective cohort study of prospectively collected data. The charts of consecutive COVID-19 patients discharged from the Reggio Emilia (Italy) ED (March 2 - March 31, 2020) were retrospectively examined. Clinical, laboratory, and computed tomography (CT) findings at first ED admission were tested as predictive factors using multivariable logistic models. We divided CT extension by days from symptom onset to build a synthetic velocity index.</p> <p><strong>Participants: </strong>450 patients discharged from the ED with diagnosis of COVID-19.</p> <p><strong>Main outcome measure:</strong> ED readmission within 14 days, followed by hospitalization/death.</p> <p><strong>Results:</strong> Of the discharged patients, 84 (18.7%) were readmitted to the ED and 61 (13.6%) were hospitalized and 10 (2.2%) died. Age (OR=1.05; 95% CI 1.03-1.08), Charlson Comorbidity Index 3 vs 0 (OR=11.61; 95% CI 1.76-76.58), days from symptom onset (OR for one day increase=0.81; 95% CI 0.73-0.90), and CT extension (OR for 1% increase=1.03; 95% CI 1.01-1.06) were associated in a multivariable model for readmission with hospitalization/death. A 2-day lag velocity index was a strong predictor (OR for unit increase=1.21, 95% CI 1.08-1.36); the model including this index resulted in less information loss.</p> <p><strong>Conclusions:</strong> A velocity index combining CT extension and days from symptom onset predicts disease progression in COVID-19 patients. For example, a 20% CT extension 3 days after symptom onset has the same risk as does 50% after 10 days.</p>

Clinical Features, Socio-Cultural Characteristics, Sleep Patterns, and Depression in Fibromyalgia Patients from India: A Cross-Sectional Study
<p><strong>Introduction</strong>: Fibromyalgia is a complex and chronic disease with significant regional variation and lack of studies from Indian population.</p> <p><strong>Objective</strong>: Aim to investigate the clinical features of fibromyalgia patients in India, including the prevalence and distribution of comorbidities, sleep patterns, and depression.</p> <p><strong>Method</strong>: Cross-sectional analysis of consecutive patients attending outpatient Rheumatology clinic from 2019-2020 satisfying ACR2016 criteria for fibromyalgia.</p> <p><strong>Results:</strong></p> <p>Of the 121 patients enrolled in the study, the majority (93.4%) were female, with a female-to-male ratio of 14:1. The mean age of the patients was 45 ±11 years. The socio-cultural profile of the patients revealed that the majority were married (88%) and homemakers (68.8%), lived in nuclear families (56%) and were middle to upper middle class (68.6%). Contrary to the existing literature, a higher prevalence of fibromyalgia has been observed in people with a higher educational status. Common clinical symptoms were extensive body aches (100%), fatigue (88%), difficulty concentrating (69.4%), irritability and gastrointestinal complaints (58.5%). We observed a high prevalence of primary headache (76%), mainly migraine (42%) and compulsive behaviour (OCB) (71%). Sleep disturbances and depression were found to be significant factors associated with fibromyalgia. The patients reported various sleep problems, such as snoring, waking up at night, daytime sleepiness, and taking daytime naps. Most of the patients had mild (50.8%) to moderate (29%) depression, while a smaller proportion experienced severe (6.6%) symptoms. Most of the patients had low vitamin D(91%).</p> <p><strong>Conclusion</strong>: The demographic profile of Indian patients was similar to that reported in the literature but had varied socio-cultural profiles needing further community-based studies. The high prevalence of psychological comorbidities and sleep disturbances highlights their importance in managing FM patients.</p>
