BONE STRAIN INDEX AS A PREDICTOR OF FURTHER VERTEBRAL FRACTURE IN OSTEOPOROTIC WOMEN

<p>Background:&nbsp;Osteoporosis is an asymptomatic disease of high prevalence and incidence, leading to bone fractures burdened by high mortality and disability, mainly when several subsequent fractures occur. A fragility fracture predictive model, Artificial Intelligence-based, to identify dual X-ray absorptiometry (DXA) variables able to characterise those patients who are prone to further fractures called Bone Strain Index, was evaluated in this study.</p> <p>Methods:&nbsp;In a prospective, longitudinal, multicentric study 172 female outpatients with at least one vertebral fracture at the first observation were enrolled. They performed a spine X-ray to calculate spine deformity index (SDI) and a lumbar and femoral DXA scan to assess bone mineral density (BMD) and bone strain index (BSI) at baseline and after a follow-up period of 3 years in average. At the end of the follow-up, 93 women developed a further vertebral fracture. The further vertebral fracture was considered as one unit increase of SDI. We assessed the predictive capacity of supervised Artificial Neural Networks (ANNs) to distinguish women consistently with a further fracture from those without a further fracture and to identify the variables expressing the maximal amount of relevant information to discriminate the two groups. ANNs were allowed to choose relevant input data automatically (TWIST-system). Moreover, we constructed a semantic connectivity map with Auto Contractive Map to offer some insight regarding the complex biological connections among the studied variables and the two conditions (further fracture vs no further fracture).</p> <p>Results:&nbsp;TWIST system selected 5 out of 13 available variables: age, menopause age, BMI, FTot BMC, FTot BSI. With training testing procedure, ANNs reached predictive accuracy of 79.36%, with a sensitivity of 75% and a specificity of 83.72%.&nbsp;The semantic connectivity map highlighted the role of BSI in predicting the risk of a further fracture.</p> <p>Conclusions:&nbsp;Artificial Intelligence is a useful method to analyse a complex system like that regarding osteoporosis, able to identify patients prone to a further fragility fracture. BSI appears to be a useful DXA index in identifying those patients who are at risk of further vertebral fractures.</p>

METHODS. Bovine ilia were used in the simulations because their histological structure (a fibrolamellar cortex overlying cancellous bone26) was found to match that of the Triceratops ilium. Bone sections 10 x 50 x 縠 3.0 cm with cortices ranging from 0.5 to 5.5 mm in depth (the range of initial cortical-thickness estimates based on gross morphology) were mounted on a servohydraulic mechanical loading frame (MTS Bionix, Minneapolis) and penetrated with an aluminium-bronze T. rex tooth replica. The replica was cast from an actual adult T. rex maxillary tooth, after casts made from some ofthe deeper bite marks revealed the size and shape of the teeth that had impacted the pelvis8 • The replica was penetrated into the ilia sections at 1 mm s-1 to a depth of 11.5 mm, equivalent to the maximum depth of the deepest ilium bite mark8 • Forces were measured with an MTS 25 N strain-gauge-based axial load cell accurate to 0.2%. The forces increased with increasing penetration depth even after the cortical layer had been perforated and the underlying cancellous bone was being crushed. Data from: Differential changes in bone strength of two inbred mouse strains following administration of a sclerostin-neutralizing antibody during growth

Administration of sclerostin-neutralizing antibody (Scl-Ab) treatment has been shown to elicit an anabolic bone response in growing and adult mice. Prior work characterized the response of individual mouse strains but did not establish whether the impact of Scl-Ab on whole bone strength would vary across different inbred mouse strains. Herein, we tested the hypothesis that two inbred mouse strains (A/J and C57BL/6J (B6)) will show different whole bone strength outcomes following sclerostin-neutralizing antibody (Scl-Ab) treatment during growth (4.5 – 8.5 weeks of age). Treated B6 femurs showed a significantly greater stiffness (S) (68.8% vs. 46.0%) and maximum load (ML) (84.7% vs. 44.8%) compared to A/J. Although treated A/J and B6 femurs showed greater cortical area (Ct.Ar) similarly relative to their controls (37.7% in A/J and 41.1% in B6), the location of new bone deposition responsible for the greater mass differed between strains and may explain the greater whole bone strength observed in treated B6 mice. A/J femurs showed periosteal expansion and endocortical infilling, while B6 femurs showed periosteal expansion. Post-yield displacement (PYD) was smaller in treated A/J femurs (-61.2%, p &lt; 0.001) resulting in greater brittleness compared to controls; an effect not present in B6 mice. Inter-strain differences in S, ML, and PYD led to divergent changes in work-to-fracture (Work). Work was 27.2% (p = 0.366) lower in treated A/J mice and 66.2% (p &lt; 0.001) greater in treated B6 mice relative to controls. Our data confirmed the anabolic response to Scl-Ab shown by others, and provided evidence suggesting the mechanical benefits of Scl-Ab administration may be modulated by genetic background, with intrinsic growth patterns of these mice guiding the location of new bone deposition. Whether these differential outcomes will persist in adult and elderly mice remains to be determined.
