PMID: 30657927.</p> <p>&nbsp;</p> <p>This is the abstract:</p> <p><strong>Objectives: </strong> In cardiac surgery, obesity is associated with a lower mortality risk. This study aims to investigate the association between body mass index (BMI) and operative mortality separately in female patients and male patients undergoing cardiac surgery and to separate the effects of weight and height in each gender-based cohort of patients.</p> <p><strong>Methods: </strong> A retrospective cohort study including 7939 consecutive patients who underwent cardiac surgery was conducted. The outcome measure was the operative mortality.</p> <p><strong>Results: </strong> In men, there was a U-shaped relationship between the BMI and the operative mortality, with the lower mortality rate at a BMI of 35 kg/m2. In women, the relationship is J-shaped, with the lower mortality at a BMI of 22 kg/m2. Female patients with obesity class II-III had a relative risk for operative mortality of 2.6 [95% confidence interval (CI) 1.37-4.81, P = 0.002]. The relationship between weight and mortality rate is a U-shaped bot in men and women, with the lower mortality rate at 100 kg for men and 70 kg for women. Height was linearly and inversely associated with the operative mortality in men and women. After correction for the potential confounders, height, but not weight, was independently associated with operative mortality in women (odds ratio 0.949, 95% CI 0.915-0.983; P = 0.004); conversely, in men, this association exists for weight (odds ratio 1.017, 95% CI 1.001-1.032; P = 0.034), but not height.</p> <p><strong>Conclusions: </strong> Contrary to men, in women obesity does not reduce the operative mortality in cardiac surgery, whereas the height seems to be associated with a lower mortality.</p>

Mortality in an Italian nursing home during COVID-19 pandemic: correlation with gender, age, ADL, vitamin D supplementation, and limitations of the diagnostic tests

<p><strong>Introduction: </strong> The COVID-19 pandemic caused an increased mortality in nursing homes due to its quick spread and the age-related high lethality.</p> <p><strong>Results: </strong> We observed a two-month mortality of 40%, compared to 6.4% in the previous year. This increase was seen in both COVID-19 positive (43%) and negative (24%) residents, but 8 patients among those testing negative on the swab, tested positive on serological tests. Increased mortality was associated with male gender, older age, no previous vitamin D supplementation and worse &quot;activities of daily living (ADL)&quot; scores, such as Barthel index, Tinetti scale and S.OS.I.A.</p> <p><strong>Conclusion: </strong> Our data confirms a higher geriatric mortality due to COVID-19. Negative residents also had higher mortality, which we suspect is secondary to preanalytical error and a low sensitivity of the swab test in poorly compliant subjects. Male gender, older age and low scores on ADL scales (probably due to immobility) are risk factors for COVID-19 related mortality. Finally, mortality was inversely associated with vitamin D supplementation.</p> <p><strong>Design: </strong> In this observational study, we described the two-month mortality among the 157 residents (age 60-100) of a nursing home after Sars-CoV-2 spreading, reporting the factors associated with the outcome. <p><strong>Abstract:</strong> In the ongoing global COVID-19 pandemic, male sex is a risk factor for severe disease and death, and the reasons for these clinical discrepancies are largely unknown. The aim of this work is to study the influence of sex on the course of infection and the differences in prognostic markers between genders in COVID-19 patients. Our cohort consisted of 64 adult patients (n = 34 men and n = 30 women) with PCR-proven SARS-CoV-2 infection. Further, a group of patients was characterized by a different severity degree (n = 8 high- and n = 8 low-grade individuals for both male and female patients). As expected, the serum concentrations of LDH, fibrinogen, CRP, and leucocyte count in men were significantly higher than in females. When serum concentrations of the inflammatory cytokines, including IL-6, IL-2, IP-10 and IL-4 and chemokines like MCP-1, were measured with multiplex ELISA, no significant differences between male and female patients were found. In COVID-19 patients, we recently attributed a new prognostic value to BPIFB4, a natural defensin against dysregulation of the immune responses. Here, we clarify that BPIFB4 is inversely related to the disease degree in men but not in women. Indeed, higher levels of BPIFB4 characterized low-grade male patients compared to high-grade ones. On the contrary, no significant difference was reported between low-grade female patients and high-grade ones. In conclusion, the identification of BPIFB4 as a biomarker of mild/moderate disease and its sex-specific activity would open an interesting field for research to underpin gender-related susceptibility to the disease.</p>

Women have a larger platelet count, but gender-based differences in platelet function remain debated. We performed a study addressing gender-based differences in platelet function using point-of-care platelet function tests (PFT).</p> <p><strong>Methods: </strong> The patient population consisted of 760 cardiac surgery patients where preoperative PFT (multiple-electrode aggregometry [MEA]) were available. Platelet count and function at the ADPtest and TRAPtest were compared in the overall population and separately in patients with or without residual effects of P2Y12 inhibitors.</p> <p><strong>Results: </strong> Women had a significantly (P = 0.001) higher platelet count but a non-significantly higher platelet reactivity to ADP. In clopidogrel-treated patients, the platelets ADP reactivity was significantly (P = 0.031) higher in women, and platelet count was the main determinant of platelet hyper-reactivity. Within patients under full clopidogrel effects, women with a platelet count &ge; 200,000 cells/&mu;L had a significantly (P = 0.023) higher rate of high-on-treatment platelet reactivity (HTPR, 45.5%) with respect to males with a platelet count &lt; 200,000 cells/&mu;L (11.9%), with a relative risk of 6.2 (95% confidence interval 1.4-29).</p> <p><strong>Conclusions: </strong> Our findings confirm that women have a larger platelet count than men, and that this is associated to a trend towards a higher platelet reactivity. HTPR is largely represented in women with a high platelet count. This generates the hypothesis that women requiring P2Y12 inhibitors could potentially benefit from larger doses of drug or should be treated with anti-platelet agents with a low rate of HTPR.</p>

<p>Raw data of the manuscript &quot;Gender and age normalization and ventilation efficiency during exercise in heart failure with reduced ejection fraction.&quot;</p> <p>&nbsp;</p> <p><strong>Aims:&nbsp;</strong>Ventilation vs. carbon dioxide production (VE/VCO<sub>2</sub>&nbsp;) is among the strongest cardiopulmonary exercise testing prognostic parameters in heart failure (HF). It is usually reported as an absolute value. The current definition of normal VE/VCO<sub>2</sub>&nbsp;slope values is inadequate, since it was built from small groups of subjects with a particularly limited number of women and elderly. We aimed to define VE/VCO<sub>2</sub>&nbsp;slope prediction formulas in a sizable population and to test whether the prognostic power of VE/VCO<sub>2</sub>&nbsp;slope in HF was different if expressed as a percentage of the predicted value or as an absolute value.</p> <p><strong>Methods and results:&nbsp;</strong>We calculated the linear regressions between age and VE/VCO<sub>2</sub>&nbsp;slope in 1136 healthy subjects (68% male, age 44.9 &plusmn; 14.5, range 13-83 years). We then applied age-adjusted and sex-adjusted formulas to predict VE/VCO<sub>2</sub>&nbsp;slope to HF patients included in the metabolic exercise test data combined with cardiac and kidney indexes score database, which counts 6112 patients (82% male, age 61.4 &plusmn; 12.8, left ventricular ejection fraction 33.2 &plusmn; 10.5%, peakVO<sub>2</sub>&nbsp;14.8 &plusmn; 4.9, mL/min/kg, VE/VCO<sub>2</sub>&nbsp;slope 32.7 &plusmn; 7.7) from 24 HF centres. Finally, we evaluated whether the use of absolute values vs. percentages of predicted VE/VCO<sub>2</sub>&nbsp;affected HF prognosis prediction (composite of cardiovascular mortality + urgent transplant or left ventricular assist device). We did so in the entire cardiac and kidney indexes score population and separately in HF patients with severe (peakVO<sub>2</sub>&nbsp;&lt; 14 mL/min/kg, n = 2919, 61.1 events/1000 pts/year) or moderate (peakVO<sub>2</sub>&nbsp;&ge; 14 mL/min/kg, n = 3183, 19.9 events/1000 pts/year) HF. In the healthy population, we obtained the following equations: female, VE/VCO<sub>2</sub>&nbsp;= 0.052 &times; Age + 23.808 (r = 0.192); male, VE/VCO<sub>2</sub>&nbsp;= 0.095 &times; Age + 20.227 (r = 0.371) (P = 0.007). We applied these formulas to calculate the percentages of predicted VE/VCO<sub>2</sub>&nbsp;values. The 2-year survival prognostic power of VE/VCO<sub>2</sub>&nbsp;slope was strong, and it was similar if expressed as absolute value or as a percentage of predicted value (AUCs 0.686 and 0.690, respectively). In contrast, in severe HF patients, AUCs significantly differed between absolute values (0.637) and percentages of predicted values (0.650, P = 0.0026). Moreover, VE/VCO<sub>2</sub>&nbsp;slope expressed as a percentage of predicted value allowed to reclassify 6.6% of peakVO<sub>2</sub>&nbsp;&lt; 14 mL/min/kg patients (net reclassification improvement = 0.066, P = 0.0015).</p> <p><strong>Conclusions:&nbsp;</strong>The percentage of predicted VE/VCO<sub>2</sub>&nbsp;slope value strengthens the prognostic power of VE/VCO<sub>2</sub>&nbsp;in severe HF patients, and it should be preferred over the absolute value for HF prognostication. Furthermore, the widespread use of VE/VCO<sub>2</sub>&nbsp;slope expressed as percentage of predicted value can improve our ability to identify HF patients at high risk, which is a goal of utmost clinical relevance.</p>
