Uterine scarring leads to adverse pregnant consequence through impairing the response of endometrium to steroids

<p>Uterine surgical scarring is an increasing risk factor for adverse pregnant consequences that threaten fetal-maternal health. The detailed molecular features of scar implantation remain largely unknown. We aim to study the pathologic features of uterine surgical scarring and the mechanisms of compromised pregnancy outcomes of scar implantation. We generated a mouse model of uterine surgical scarring with a uterine incision penetrating the myometrium to endometrium to examine the pathologic changes and transcriptome profiles of uterine scarring at various post-surgery (PS) time points, as well as features of the feto-maternal interface during scar implantation. We found that uterine surgical scar recovery was consistently poor at PS3 until PS90, as shown by a reduced number of endometrial glands, inhibition of myometrial smooth muscle cell growth but excessive collagen fiber deposition, and massive leukocyte infiltration. Transcriptome annotation indicated significant chronic inflammation at the scarring site. At the peri-implantation and postimplantation stages, abnormal expression of various steroid-responsive genes at the scarring site was in parallel with lumen epithelial cell hyperplasia, inappropriate luminal closure, and disorientation of the implanted embryo, restricted stromal cell proliferation, and defective decidualization. High embryonic lethality (around 70%) before E10.5 was observed, and the small amount of survival embryos at E10.5 exhibited restricted growth and aberrant placenta defects including overinvasion of trophoblast cells into the decidua and insufficient fetal blood vessel branching in the labyrinth. The findings indicate that chronic inflammation and compromised responses to steroids in uterine scar tissues are the pivotal molecular basis for adverse pregnancy consequences of scar implantation.</p>

Data from: Of 11 candidate steroids, corticosterone concentration standardized for mass is the most reliable steroid-biomarker of nutritional stress across different feather types

<p>1. Measuring corticosterone in feathers has become an informative tool in avian ecology, enabling researchers to investigate carry-over effects and responses to environmental variability. Few studies have, however, explored whether corticosterone is the only hormone expressed in feathers, and is the most indicative of environmental stress. Essential questions remain as to how to compare hormone concentrations across different types of feathers and whether preening adds steroids, applied after feather growth. 2. We used liquid chromatography coupled to tandem mass spectrometry to quantify a suite of 11 steroid hormones in back, breast, tail, and primary feathers naturally grown at overlapping time intervals by rhinoceros auklet Cerorhinca monocerata captive-reared fledglings and wild-caught juveniles. The captive-reared birds were raised on either a restricted or control diet. Measured steroids included intermediates in the adrenal steroidogenesis pathway to glucocorticoids and the sex steroids pathway to androgens and estrogens. 3. Corticosterone was detected in the majority of feathers of each type. We also detected cortisone in back feathers, androstenedione in breast feathers, and testosterone in primary feathers. Captive fledglings raised on a restricted diet had higher concentrations of corticosterone in all four feather types than captive fledglings raised on a control diet. Corticosterone concentrations were reliably repeatable across feather types when standardized for feather mass, but not for feather length. Of the seven hormones looked for in uropygial gland secretions, only corticosterone was detected in one out of 23 samples. 4. We conclude that corticosterone is the best feather-steroid biomarker for detection of developmental nutritional stress, as it was the only hormone to manifest a signal of nutritional stress, and that exposure to stress can be compared among different feather types when corticosterone concentrations are standardized by feather mass.</p>

Effectiveness of steroid therapy on pneumonic chronic obstructive pulmonary disease exacerbation: a multi-centred retrospective cohort study

<p><span>Background</span></p> <p><span>To date, no consensus exists on the effects of steroid use on pneumonic chronic obstructive pulmonary disease (COPD) owing to trial design issues in previous trials involving these conditions. Therefore, we aimed to evaluate steroid effectiveness in pneumonic COPD exacerbation patients.</span></p> <p><span>Methods</span></p> <p><span>This multi-centred, retrospective, observational study was conducted across five acute general hospitals in Japan. We analysed the association between parenteral/oral steroid therapy and time to clinical stability in pneumonic COPD exacerbation. </span></p> <p><span>We used a validated algorithm derived from the 10th revision of the International Classification of Diseases and Related Health Problems (ICD-10) to include pneumonic COPD exacerbation patients. We excluded patients with other hypoxia causes (asthma exacerbation, pneumothorax, heart failure) and complicated pneumonia (obstructive pneumonia, empyema), those who required tracheal intubation/vasopressors, and those who were clinically stable on the admission day. </span></p> <p><span>The primary outcome was time to clinical stability. Multiple imputation was used for missing data. Propensity scores within each imputed dataset were calculated using potential confounding factors. The Fine and Gray model was used within each dataset to account for the competing risk of death and hospital discharge without clinical stability, and we combined the results.</span></p> <p><span>Results</span></p> <p><span>Altogether, 1237 patients were included. The pooled estimated subdistribution hazard ratio of time to clinical stability in steroid versus non-steroid users was 0.89 (95% confidence interval, 0.78<a name="_Hlk33128668"> to</a> 1.03). However, there were potentially unmeasured confounders, and we could not assess longer-term outcomes.</span></p> <p><span>Conclusions</span></p> <p><span>The current study recommends that steroid therapy should not be used routinely for pneumonic COPD exacerbation.</span></p>

Data from: Non-steroidal anti-inflammatory drugs use in older adults decreases risk of Alzheimer's disease mortality

Alzheimer disease (AD) mortality risk in a large cohort of subjects treated or not with non-steroidal anti-inflammatory drugs (NSAIDs) is unknown. Our objective was to determine whether NSAIDs use is associated with decreased risk of AD mortality. Methods: In this prospective, population-based study (Neurological Disorders in Central Spain [NEDICES]) of 5,072 people without AD (aged 65 years and older), sociodemographic, comorbidity factors, and current medications were recorded at baseline. Community-dwelling older adults were followed for a median of 12.7 years, after which the death certificates of deceased participants were examined. 2,672 (52.7%) of 5,072 participants died, including 504 (18.9%) NSAIDs users and 2,168 (81.1%) non-users. Of the 2,672 deceased participants, 113 (4.2%) had AD as a cause of death (8 [1.6%] among NSAIDs users and 105 [4.8%] among non-users, chi-square = 10.70, p=0.001). In an unadjusted Cox model, risk of AD mortality was decreased in NSAIDs users (hazard ratio [HR] for AD mortality = 0.35, 95% confidence interval [CI] 0.17–0.72, p=0.004) when compared to non-users. After adjusting for numerous demographic factors and co-morbidities, the HR for AD mortality in NSAIDs users was 0.29, 95% CI 0.12–0.73, p=0.009. Stratified analyses showed a significantly decreased risk of AD mortality with aspirin, whereas non-aspirin NSAIDs only showed a statistical trend toward significance in the adjusted Cox regression models. NSAIDs use was associated with 71% decreased risk of AD mortality in older adults. Our results support the hypothesis that NSAIDs use is a protective factor of developing AD.

Data from: Managing anabolic steroids in pre-hibernating Arctic ground squirrels: obtaining their benefits and avoiding their costs

Androgens have benefits, such as promoting muscle growth, but also significant costs, including suppression of immune function. In many species, these trade-offs in androgen action are reflected in regulated androgen production, which is typically highest only in reproductive males. However, all non-reproductive Arctic ground squirrels, irrespective of age and sex, have high levels of androgens prior to hibernating at sub-zero temperatures. Androgens appear to be required to make muscle in summer, which, together with lipid, is then catabolized during overwinter. By contrast, most hibernating mammals catabolize only lipid. We tested the hypothesis that androgen action is selectively enhanced in Arctic ground squirrel muscle because of an upregulation of androgen receptors (ARs). Using Western blot analysis, we found that Arctic ground squirrels have AR in skeletal muscle more than four times that of Columbian ground squirrels, a related southern species that overwinters at approximately 0°C and has low pre-hibernation androgen levels. By contrast, AR in lymph nodes was equivalent in both species. Brain AR was also modestly but significantly increased in Arctic ground squirrel relative to Columbian ground squirrel. These results are consistent with the hypothesis that tissue-specific AR regulation prior to hibernation provides a mechanism whereby Arctic ground squirrels obtain the life-history benefits and mitigate the costs associated with high androgen production.

Data from: Feather steroid hormone concentrations in relation to age, sex, and moulting time in a long distance migratory passerine

In birds, concentrations of testosterone (T) and corticosterone (Cort) are closely connected with many morphological, behavioural and other physiological traits, including reproduction, metabolism, immunity, and fitness. The direction of the effect of these hormones on above-mentioned traits, and the potential feedback between hormones are in general unclear; in addition, knowledge on how age and sex can affect T and Cort concentrations is still inconsistent. Our study used a novel method to analyse testosterone and corticosterone in feathers (Tf, Cortf) based on the pre-column chemical derivatization of hormones before LC-MS/MS analysis. Unlike previously used methods (RIA, EIA), our analytical procedure allows simultaneous analysis of both hormones from small amounts of feathers (4-25 mg) and thus overcomes the problem of insufficient detection limits. We applied this method to reveal associations between Tf and Cortf hormone concentrations and feather growth, age, and sex in feathers grown during the post-breeding (flanks) and pre-breeding (tails) periods in barn swallows (Hirundo rustica). There was neither a correlation between pre-breeding and post-breeding Tf, nor between pre-breeding and post-breeding Cortf. Tail Cortf concentrations were negatively associated with tail feather growth rates. Feather hormone concentrations were correlated in the pre-breeding period, negatively in males but positively in females. Both Cortf and Tf were higher in young birds compared to older ones, indicating either an age-related decrease in hormone concentrations within individuals, or the selective disappearance of individuals with high steroid concentrations. Males and females did not differ in Cortf, but Tf concentrations were higher in males than females, particularly during the pre-breeding period. In this study, we provide an effective method for analysing hormones in feathers in an ecological context, especially in situations when the total amount of feathers available for the analysis is limited.

A retrospective cohort study on effects of antenatal steroids on respiratory morbidity for term elective Caesarean sections in South Asian women

<p>Respiratory distress (RD) is higher among newborns born by caesarean section (CS) at term. RCOG recommend steroid administration for CS prior to 39 weeks. Evidence for effectiveness of steroids for neonatal RD at term is inconclusive. The racial differences are yet to be studied. </p> <p>A single center retrospective cohort study was conducted in Colombo, Sri Lanka form December 2016 to February 2019. All mothers delivered by CS between 37+0 and 38+6 weeks were included in the study. Mothers with severe maternal hypertension, severe fetal rhesus sensitization, evidence of intrauterine infection, multiple pregnancies and those who received steroids due to other indications at a prior gestation and were excluded. Cohort was subdivided according to administration of IM dexamethasone prior to CS. Primary outcomes measured were RD, admissions to neonatal intensive care unit (NICU) and special care baby unit (SCBU). Neonatal infections and maternal duration of hospital stay was recorded as secondary outcome measures. </p> <p>560 patients were included in the study. 23.2% of patients received antenatal corticosteroids. Incidence of RD, NICU admissions and SCBU admissions in the study cohort was 10%, 0.9%, 2.7% respectively. RR for developing RD in the steroid group compared to non-steroid group was 2.67 (1.64-4.35). NICU and SCBU admissions was needed by 4.6% in steroid group and 3.3% in non-steroid group without a significant difference (p=0.464). A significantly higher number of babies in the steroid group needed IV antibiotics. Average days the mothers were admitted in the hospital was 2.45 days (SD 1.424) for steroid group and 1.4 days (SD 0.856) for the non-steroid group (p &lt; 0.001).</p> <p>There is a significant increase in the respiratory morbidity in the dexamethasone administered south Asian mothers at term prior to CS. However, this effect has no clinical significance since the admission to NICU and SCBU were not significantly different.</p>
